Active middle ear implant after lateral petrosectomy and radiotherapy for ear cancer.

BACKGROUND: Tumor of the temporal bone is a rare disease with a very poor prognosis. Surgery and postoperative radiotherapy are usually the recommended treatments for squamous cell carcinoma (SCC) of the external and middle ear, which may cause conductive hearing loss. The purpose of this study was to evaluate the audiologic results and compliance of active middle ear implant (AMEI) and establish the feasibility of the procedure in a patient treated for middle ear cancer. METHODS: A 73-year-old patient treated with lateral petrosectomy, neck dissection, reconstruction/obliteration by pedicled…

Ear piercing affects earprints: the role of ear piercing in human identification.

Previous research conducted into the use of the human ear in the field of forensic identification has focused upon the use of grids and manual methods to measure and catalogue the different anatomical features of the ear. To date, few have considered the importance of the presence of ear piercings and their possible role in human identification. This study aims to highlight the common distribution of piercings of both ears in both genders and to explore the effect of piercings on earprints. The presence of a piercing may, in part, help to explain why partial and not whole earprints are sometim…

Chronic ear pathology in a model of neonatal amniotic fluid ear inoculation.

Human histopathologic studies have demonstrated that amniotic fluid cellular contents, keratinized squamous epithelial cells and lanugo hair … osteoneogenesis, tympanosclerosis, and cholesteatoma; the control side demonstrated minimal middle ear changes. We conclude that in this model autologous keratinized tissue provokes a foreign body response similar to the granulation tissue observed in human infants and, further, that over a prolonged period the middle ear demonstrates more severe pat …

Complicated cholesteatomas: CT findings in inner ear complications of middle ear cholesteatomas.

Patients with facial palsy and middle ear disease, which may be chronic but clinically occult, may have a cholesteatoma with extension medially along the facial canal. In two patients, axial computed tomographic (CT) scans demonstrated involvement of the medial petrous bone. Patients with vertigo and chronic middle ear disease may have a cholesteatoma with a "fistula" between the middle and inner ears. Although the fistula usually involves the lateral semicircular canal, the cholesteatoma may pass through the oval window. In two patients, coronal CT scans showed extension to the oval window in…

Red colour of a painful ear: red ear syndrome in paediatric age.

A middle childhood boy with no disclosed medical history presented with paroxysmal episodes of pain in the left ear accompanied by redness of the earlobe over a 3-month period. The attacks occurred suddenly, lasted for hours and had a variable frequency. He denied experiencing a headache or other neurological symptoms and had previously attempted treatment with gabapentin, prednisolone, ibuprofen and topical lidocaine without success. Suspecting red ear syndrome (RES), we requested cervical and brain MRI and initiated a therapeutic trial with indomethacin 75 mg/day. The MRI revealed a loop of …

Desmoid tumour of the external ear and mastoid following middle-ear surgery.

BACKGROUND: Desmoid tumours (aggressive fibromatosis) are rare, locally invasive, benign tumours. The following case represented a diagnostic challenge, because of the uncommon nature of the lesion. CASE REPORT: A 26-year-old woman, who had previously undergone middle-ear surgery for cholesteatoma, presented with a painful swelling involving the post-auricular area and the conchal bowl. Initially, it was believed to be an infective process related to the surgery or an unusual cholesteatoma recurrence. Following investigations, which involved imaging and histology, the swelling was diagnosed a…

In Vivo Measurements of Middle Ear Temperature During Transcanal Endoscopic Ear Surgery.

OBJECTIVE: The light emitted from the endoscope during transcanal endoscopic ear surgery (TEES) heats the intratympanic space. This heat may potentially be dangerous to nearby important structures, as documented by in vitro and by animal and cadaveric studies. The aim of our work was to monitor middle ear temperatures during TEES in vivo. STUDY DESIGN: Cohort study. SETTING: Tertiary referral hospital. PATIENTS: Four patients (15-69 yrs old) underwent transcanal endoscopic tympanoplasties for chronic perforation or retraction. INTERVENTION: After elevating the tympanomeatal flap, a thermoc…

Hybrid Ear Cubes for local controlled dexamethasone delivery to the inner ear.

A new type of miniaturized implants for local controlled drug delivery to the inner ear is proposed: Hybrid Ear Cubes. They are composed of two main parts: (i) a cylinder, which is placed into a tiny hole (<0.4 mm) drilled into (or close to) the oval (or round) window, and (ii) a cuboid, which is placed into the middle ear. The drug is released at a pre-programmed rate into the perilymph: (i) via the cylindrical part of the implant, which is in direct contact with this liquid, and (ii) via diffusion from the cuboid through the oval/round window. Importantly, the cylindrical part assures a reli…

Prolonged Radiant Exposure of the Middle Ear during Transcanal Endoscopic Ear Surgery.

Transcanal endoscopic ear surgery (EES) provides a high-resolution, wide-field view of the middle ear compared with the conventional operating microscope, reducing the need for a postauricular incision or mastoidectomy. Our group has shown in cadaveric human temporal bone studies that radiant energy from the endoscope tip can quickly elevate temperatures of the tympanic cavity. Elevated temperatures of the middle ear are associated with acute auditory brainstem response shifts in animal models. In EES, proposed methods to decrease middle ear temperature include frequent removal of the endoscop…

Ear asymmetries in middle-ear, cochlear, and brainstem responses in human infants.

In 2004, Sininger and Cone-Wesson examined asymmetries in the signal-to-noise ratio (SNR) of otoacoustic emissions (OAE) in infants, reporting that distortion-product (DP)OAE SNR was larger in the left ear, whereas transient-evoked (TE)OAE SNR was larger in the right. They proposed that cochlear and brainstem asymmetries facilitate development of brain-hemispheric specialization for sound processing. Similarly, in 2006 Sininger and Cone-Wesson described ear asymmetries mainly favoring the right ear in infant auditory brainstem responses (ABRs). The present study analyzed 2640 infant responses …

Effect of increased middle ear pressure on blood flow to the middle ear, inner ear and facial nerve in guinea pigs.

We investigated the effect of increased middle ear pressure on blood flow to the inner ear, middle ear, and facial nerve in guinea pigs using a nonradioactive microsphere technique. The elevation of middle ear pressure significantly reduced blood flow to the middle ear. Blood flow to the facial nerve also decreased due to the elevation of the middle ear pressure but the reduction was not significant. Blood flow to the inner ear did not change even after the middle ear pressure was increased. Findings are discussed in relationship to aural barotrauma, alternobaric vertigo and facial baroparesis…

Transplantation of Ears Provides Insights into Inner Ear Afferent Pathfinding Properties.

Numerous tissue transplantations have demonstrated that otocysts can develop into normal ears in any location in all vertebrates tested thus far, though the pattern of innervation of these transplanted ears has largely been understudied. Here, expanding on previous findings that transplanted ears demonstrate capability of local brainstem innervation and can also be innervated themselves by efferents, we show that inner ear afferents grow toward the spinal cord mostly along existing afferent and efferent fibers and preferentially enter the dorsal spinal cord. Once in the dorsal funiculus of the…

Does ear keloid formation depend on the type of earrings or piercing jewellery?

The ear is one of the most common sites of keloid formation, usually associated with the wearing of earrings. However, although this is a small perforating lesion that is equal on both sides, keloids are bigger and more prevalent on the posterior surface. In this study, 141 keloids were evaluated, most of which were located on the posterior surface of the earlobe and caused by the piercing of the ear. Most of the earrings and piercing jewellery used had metallic backs. The more frequent occurrence of earlobe keloids on the posterior surface of the ear therefore may be associated with the exace…

[Correlation of ear and extra-ear effects in workers exposed to industrial noise].

A morbidity of 600 female twister operators was exposed to a long-standing (over 20 years) study and manifold statistic analysis, data of which are represented. The vascular disorders appeared to correlate with the occupational deafness formation. Cerebral resistance recording (CRR) and EEG in 120 workers with various deafness degrees demonstrated the different grades of impaired cerebral and peripheral blood flow. The manifold analysis and CRR produced basic data so as to classify the vasculogenic lesions caused by the exposure to noise and to evaluate the occupational disablement.
