Results of decompression of four medial ankle tunnels in the treatment of tarsal tunnels syndrome.

Controversy surrounds the surgical approach and efficacy for tibial nerve compression at the ankle. The hypotheses tested are that the poor published results are due to failure to recognize that the tarsal tunnel is analogous to the forearm, not the carpal tunnel, and that postoperative ankle immobilization contributes to poor results by permitting fibrosis of the tibial nerve branches. From January of 1987 through December of 1994, a consecutive series of 77 patients with tarsal tunnel syndrome was accrued, 10 of whom had the condition bilaterally. The surgical approach included a neurolysis

[Prevention of venous thromboembolism in surgery].

Incidence of postoperative deep venous thrombosis extends from 7% (transurethral prostatectomy) to more than 50% (hip surgery) depending on the magnitude of trauma and due to the corresponding surgical procedure. Although more common with higher age, younger patients suffer from postoperative thromboembolic complications as well. This insight leads to the conclusion that all immobilized adult patients or those needing an operation are to receive thromboembolic prophylaxis should there be no contraindication. As 50% of all postoperative thromboses develop sometime during the operative procedure

Nonoperative treatment of acute knee ligament injuries. A review with special reference to indications and methods.

Nonoperative treatment has received little attention in the numerous scientific reports on knee ligament injuries. Great controversy still exists concerning the proper treatment of a knee with a ruptured ligament, especially the anterior cruciate ligament. However, according to the studies of the authors and an extensive review of the literature the indications for conservative management can be established to be all grade I and II sprains (partial tears) of knee ligaments as well as an isolated grade III sprain (complete tear) of the posterior cruciate ligament. In addition, an isolated compl

Failure of magnesium sulphate to prevent suxamethonium induced muscle pains.

In fit unpremedicated patients undergoing minor operations and who were ambulant on the afternoon of the operations, pretreatment with magnesium sulphate given intravenously did not reduce the incidence of suxamethonium induced myalgia below that in a similar series who received no prophylactic therapy. The injection of magnesium in conscious patients is followed by unpleasant side effects.

Impact of canine-assisted ambulation on hospitalized chronic heart failure patients' ambulation outcomes and satisfaction: a pilot study.

BACKGROUND AND RESEARCH OBJECTIVE: Chronic heart failure (HF) is a prevalent and costly disease process. Early ambulation has been shown to have a positive impact on patient outcomes and length of stay. Animal-assisted therapy is a novel modality that has shown to be a safe and effective adjunct to a number of traditional treatment plans. This study sought to synergistically combine ambulation and animal-assisted therapy by using canine-assisted ambulation (CAA) to improve the ambulation outcomes of HF patients. SUBJECTS AND METHODS: Sixty-nine hospitalized patients with a primary diagnosis o…

Implementing a thoracic enhanced recovery with ambulation after surgery program: key aspects and challenges.

Enhanced recovery after surgery (ERAS) protocols aim to improve operative outcomes by focusing on perioperative care, including early mobilization, limitation of narcotics, and maintenance of fluid balance. We implemented a T-ERAAS (Thoracic-Early Recovery with Ambulation After Surgery) protocol which focused on early ambulation, with the rationale that a patient's mobility may be a reproducible and measurable metric for their overall status-pain control, respiratory function, cardiac function, and patient satisfaction. We set a benchmark distance of 250 feet for our early ambulation goal and …

Consensus statement on the use of standardized early mobilization in pediatric patients admitted to pediatric intensive care units in Italy.

UNLABELLED: Prolonged immobility contributes to pediatric intensive care unit (PICU)-acquired weakness and postintensive care syndrome (PICS). Early mobilization (EM) shows feasibility and potential benefits in adults, and pediatric experiences suggest safety, though data are limited and practice varies. There is a wide variability in Italy regarding EM practices. Thus, we aimed to develop consensus statements to standardize EM across Italian PICUs today while respecting local variability. This project was conducted by a 20-member multidisciplinary panel of Italian PICU experts, including nurs

Immediate mobilization after repair of Achilles tendon rupture may increase the incidence of re-rupture: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Achilles tendon rupture (ATR) is a significant injury that can require surgery and can have the risk of re-rupture even after successful treatment. Consequently, to minimize this risk, it is important to have a thorough understanding of the rehabilitation protocol and the impact of different rehabilitation approaches on preventing re-rupture. MATERIALS AND METHODS: Two independent team members searched several databases (PubMed, EMBASE, Web of Science, Cochrane Library, and CINAHL) to identify randomized controlled trials (RCTs) on operative treatment of ATR. We included articles

[The use of robotic and technical systems for early mobilization of intensive care patients: A scoping review].

The use of robotic and technical systems for early mobilization of intensive care patients: A scoping review Abstract: Background: Intensive care patients are often subjected to immobility for too long. However, when they are mobilized early, positive effects on patient outcomes, such as improvement in physical function, can be demonstrated. One of the reasons for rare mobilization is that too less therapeutic equipment is available. Aims: This paper provides an overview of previous research on early robot- or technology-assisted mobilization of intensive care patients. Which robotic and techn

Impact of Early Mobilization in the Intensive Care Unit on Psychological Issues.

Early mobilization is an intervention protocol that can be employed in the critically ill population to effectively reduce the risks and consequences normally associated with immobility in high-acuity patients. In turn, rate and quality of recovery are improved as well as overall patient outcomes. Although there are many challenges inherent to the implementation of an early mobilization program, success of such a program is achievable through structured education of staff and diligence in its application. As a result, staff and patient experience the benefits of medical treatment that addresse

[Principles and challenges of mobilisation in intensive care].

The harmful consequences of bed rest and inactivity in patients in intensive care have been widely described. The point at which these patients should be mobilised and the methods used however still remain unclear. It is nevertheless important that the mobilisation is implemented early and often, adapted to the condition of the patient and overseen by a cross-disciplinary team.

Early versus delayed mobilisation for aneurysmal subarachnoid haemorrhage.

BACKGROUND: Rebleeding is an important cause of death and disability in patients with aneurysmal subarachnoid haemorrhage (SAH). In order to prevent rebleeding, the preferred strategy is aneurysm ablation (removal) as early as possible. However, in clinical practice some patients are not suitable for surgical treatment, or prefer conservative treatments. In some countries, therefore, total bedrest for four to six weeks has been considered one of the basic interventions to avoid rebleeding. However, the influence of bedrest on outcome in patients with SAH is not well known. OBJECTIVES: To esta

[Early rehabilitation of vascular amputees. Value of the multipurpose prosthesis].

After a recall of surgical indications and of technical points of the lower limbs amputations in arteriosclerosis disease, the authors report their experience of early rehabilitation. They define the aims of the rehabilitation and the means to reach it. Among these, a multipurpose prosthesis made by them, authorized their patients to an early walk training. The results are analysed with 86 patients rehabilitated in 2 years by this method. They have 49 good results/51 with amputation below the knee, 31 good results/35 with femoral amputation. The mean age of the patients is 72.
