Treatment of acute pancreatitis.

Acute pancreatitis (AP) is a potentially life-threatening inflammatory condition characterized by localized pancreatic damage and the activation of the inflammatory cascade, leading to systemic inflammatory response syndrome (SIRS). This complex disease often presents with a variable and unpredictable course. The primary causes of AP include the migration of gallstones and alcohol consumption. The Revised Atlanta Classification 2012 (RAC) is the most widely utilized classification system for AP, distinguishing between interstitial edematous pancreatitis and necrotizing pancreatitis, three seve

Mixed venous oxygen saturation monitoring after stage 1 palliation for hypoplastic left heart syndrome.

BACKGROUND: Staged palliation for hypoplastic left heart syndrome has been marked by high early mortality due to the limited cardiac output of the postischemic single right ventricle combined with the inefficiency and volatility of parallel circulation. METHODS: Since July 1996, we have performed stage 1 palliation (S1P) in 178 patients. Within this group is a consecutive cohort of 116 patients with true hypoplastic left heart syndrome that underwent S1P with a modified Blalock-Taussig shunt. A prospective database containing postoperative hemodynamic data was maintained on all patients. Stud

Commonly used severity scores are not good predictors of mortality in sepsis from severe leptospirosis: a series of ten patients.

Introduction. Severe leptospirosis, also known as Weil's disease, can cause multiorgan failure with high mortality. Scoring systems for disease severity have not been validated for leptospirosis, and there is no documented method to predict mortality. Methods. This is a case series on 10 patients admitted to ICU for multiorgan failure from severe leptospirosis. Data were collected retrospectively, with approval from the Institution Ethics Committee. Results. Ten patients with severe leptospirosis were admitted in the Patras University Hospital ICU in a four-year period. Although, based on SOFA

Aquatic Therapy as a Programmable Multisensory Environment for Arousal and Postural Control After Severe Acquired Brain Injury: A Perspective.

Background/Objectives: Severe acquired brain injury (sABI) disrupts early rehabilitation because arousal fluctuates, trunk control is fragile, and agitation limits therapy tolerance; land-based practice is frequently constrained by fall risk and staffing. We aim to reframe aquatic therapy as a programmable multisensory environment to stabilize arousal and support axial alignment before conventional impairment targets are feasible. Here, programmable denotes the deliberate titration and reporting of water depth, turbulence or perturbation, temperature, body orientation, and flotation and manual

Intravenous fluid therapy in sepsis.

Sepsis is the dysregulated immune response to severe infection that is common and lethal among critically ill patients. Fluid administration is a common treatment for hypotension and shock in early sepsis. Fluid therapy can also cause edema and organ dysfunction. Research on the best treatment strategies for sepsis has provided insights on the optimal timing, dose, and type of fluid to treat patients with sepsis. Initial research on early goal-directed therapy for sepsis included an initial bolus of 30 ml/kg of fluid, but more recent research has supported use of smaller volumes. After initial…

Treatment of early-stage laryngeal cancer: A comparison of treatment options.

Over the course of the last several decades, the treatment options for early laryngeal cancers (T1 and T2) have evolved; however, simultaneously the mortality rate has increased. As larynx preservation approaches have become the standard of care, the selection of the proper treatment modality has become paramount. Radiation therapy or transoral laser microsurgery are the most common options for treatment of these early lesions. Oncologic and functional outcomes are considered equivalent between the two modalities for early glottic cancers; however, no direct comparisons exist for robust analys

Best evidence for fluid resuscitation nursing in hypovolemic shock patients in emergency care based on GRADE system.

BACKGROUND: Hypovolemic shock is a life-threatening condition frequently encountered in emergency care, resulting from significant blood or fluid loss that impairs oxygen delivery to vital organs. Common causes include trauma, surgery, gastrointestinal bleeding, and ruptured ectopic pregnancy. Prompt fluid resuscitation is critical, with nurses playing a central role in early recognition, timely intervention, and continuous monitoring. OBJECTIVE: To synthesise the best evidence for fluid resuscitation practices in hypovolemic shock and assess nursing knowledge and practice in emergency settin

A proposed scoring system for predicting mortality in melioidosis.

Melioidosis, due to infection with the environmental organism Burkholderia pseudomallei, continues to be associated with high mortality despite improvements in antibiotic therapy. Using simple clinical findings and baseline laboratory tests available at the time of admission, we attempted to define those patients with acute melioidosis who were at higher risk of death. Using data, collected prospectively from the period October 1989 to June 2002, from patients with acute culture-confirmed melioidosis presenting at the Royal Darwin Hospital, Darwin, Australia, a number of variables were selecte

Beyond Survival: Outpatient Treatment for Functional Recovery After Brain Injury.

Acquired brain injuries (ABIs), including hypoxic-ischemic and traumatic injuries, are leading causes of long-term disability in children. While acute treatment has advanced, many survivors face persistent cognitive, emotional, and physical challenges that require coordinated outpatient care. This review aims to equip pediatric providers with the knowledge and tools necessary to provide longitudinal, comprehensive, and family-centered outpatient care for children recovering from ABIs. This review begins with the definitions, etiologies, and prognostic factors associated with pediatric brain in

DIAGNOSIS AND HABILITATION OF CONGENITAL MUSCULAR TORTICOLLIS: A NARRATIVE REVIEW.

Congenital muscular torticollis (CMT) is one of the most common congenital musculoskeletal deformities, and is defined as tilted head position with hypertonic sternocleidomastoid muscle and limited cervical spine mobility. Incidences up to 16% have been recorded, more often in male children. Clinical features are characterized by an inclined position of the head on the diseased side and a gaze directed towards the healthy side, deformities such as plagiocephaly, and various complications. In order to prevent this, it is recommended to screen all infants up to four months of age. Diagnosis is u

Severe scombroid fish poisoning syndrome requiring aggressive fluid resuscitation in the emergency department: two case reports.

Scombroid fish poisoning (scombrotoxism, scombroid ichthyotoxicosis) is a food-related illness typically associated with the consumption of dark and white meat fish. Two patients presented to the emergency department. Metilprednisone 1000 mg and ranitidine 150 mg were administered initially. A large amount of crystalloids and colloids in in combination with vasoactive drugs were required to maintain normopressure. Levels of histamine and N-methylhistamine were far above the normal mean. Carboxyhemoglobin levels were also tested to exclude a superimposition of carbon monoxide intoxication. In b

Perioperative Management of Patients Receiving Renal Replacement Therapy Undergoing Major Cardiac Surgery: A Narrative Review.

The perioperative management of patients with end-stage renal disease on renal replacement therapy undergoing cardiac surgery presents significant clinical challenges and is associated with high morbidity and mortality. These patients frequently exhibit multiple comorbidities, including electrolyte disturbances, anemia, bleeding tendency, and cardiovascular instability, all of which complicate surgical management and postoperative recovery. Renal dysfunction markedly increases the risk of adverse postoperative events, even at mild stages, and is incorporated into established risk stratificatio

Oliguria, volume overload, and loop diuretics.

Acute kidney injury (AKI) is commonly and increasingly encountered in patients with critical illness. In the past, epidemiologic studies have consistently found that oliguria further increases the risk of death from AKI. Compared with patients outside the intensive care unit (ICU), critically ill patients are more likely to have volume overload as a result of impaired solute and water excretion. Recently, broad changes have occurred in ICU practice, such as early goal-directed therapy in sepsis, which may further compound volume overload in the ICU patient with oliguric AKI. Evidence has also …

Secondary Nasolabial Aesthetic Revision After Cleft Lip Surgery: A Narrative Review of Scar-Related Deformity and Subunit Reconstruction.

Secondary nasolabial aesthetic revision after cleft lip surgery requires coordinated assessment of scar-related contracture, lip subunit distortion, and cleft nasal deformity. Traditionally, these 2 aspects have been managed separately. This narrative review summarizes a focused, revision-oriented framework in which scar management is considered as an adjunct to staged nasolabial subunit reconstruction rather than as an isolated therapeutic goal. Early scar-directed measures, including tension reduction, silicone-based therapy, pulsed-dye laser, and selected pharmacologic interventions, may im

Standardizing the Clinical Approach to Cancer Therapy-Related Cardiac Dysfunction: Applying Cardio-Oncology Guidelines as a Practical Tool for Hematology and Oncology Providers.

INTRODUCTION AND METHODS: Cancer therapy-related cardiac dysfunction (CTRCD) is a well established and potentially life-threatening complication of contemporary oncologic treatment. Although comprehensive cardio-oncology guidelines have been developed, their integration into routine hematology and oncology practice remains inconsistent. This consensus statement, developed by a multidisciplinary panel of cardio-oncology experts, aims to provide practical, case-based guidance to help oncology providers recognize, assess, and manage CTRCD across a spectrum of malignancies and cardiovascular prese

Lupus Nephritis: Unmet Needs and Evolving Solutions.

Lupus nephritis (LN) is seeing more and more enriching immunotherapies, but important unmet needs remain. Here, we discuss how to focus on histologic signs of immunologic activity triggering immunotherapy versus signs of irreversible kidney injury requiring care for CKD. In addition, the correct interpretation of residual proteinuria requires dissecting immunologic activity from glomerular hyperfiltration, e.g ., by repeat biopsy. Despite modern triple immunotherapy, per-protocol biopsies still document irreversible injury to occur in the first year. Immediate inhibition of the complement syst

Phases of fluid management and the roles of human albumin solution in perioperative and critically ill patients.

OBJECTIVE: Positive fluid balance is common among critically ill patients and leads to worse outcomes, particularly in sepsis, acute respiratory distress syndrome, and acute kidney injury. Restrictive fluid infusion and active removal of accumulated fluid are being studied as approaches to prevent and treat fluid overload. Use of human albumin solutions has been investigated in different phases of restrictive fluid resuscitation, and this narrative literature review was undertaken to evaluate hypoalbuminemia and the roles of human serum albumin with respect to hypovolemia and its management.

Anticoagulants for atrial fibrillation: from warfarin and DOACs to the promise of factor XI inhibitors.

Anticoagulation is the mainstay of stroke prevention in appropriate patients with atrial fibrillation. Due to advances in pharmacotherapy the anticoagulants used for this purpose have evolved significantly over the past decades with the aim of optimizing effectiveness while minimizing bleeding risks. Though significant improvements have been made toward this goal, bleeding risk remains the major concern with these therapies. An investigational class of agents which inhibit Factor XI have shown promise in pre-clinical and early clinical trials to significantly minimize bleeding while maintainin
