Cranial ultrasonographic findings in healthy full-term neonates: a retrospective review.

BACKGROUND: Ultrasonography is a non-invasive diagnostic technique, and it has been used to detect intracranial lesions in neonates for a long time. Correspondingly, screening tests using cranial ultrasonography have been applied for early detection of intracranial lesions in full-term neonates during the past decade. METHODS: We retrospectively reviewed the findings of cranial ultrasonographic screening tests in healthy full-term neonates between September 2004 and August 2009. The ultrasonographic findings were divided into the following categories: (a) nonsignificant (NS) group, including

Myth: cerebral palsy cannot be predicted by neonatal brain imaging.

There is controversy in the literature about the value of brain imaging in neonates regarding the prediction of cerebral palsy (CP). The aim of this review was to unravel the myth that CP cannot be predicted by neuroimaging in neonates. Major intracranial lesions in the preterm infant should be recognized with sequential cranial ultrasound and will predict those with non-ambulatory CP. Magnetic resonance imaging (MRI) at term-equivalent age will refine the prediction by assessment of myelination of the posterior limb of the internal capsule. Prediction of motor outcome in preterm infants with

Use of fetal magnetic resonance imaging in patients electing termination of pregnancy by dilation and evacuation.

OBJECTIVE: The purpose of this study was to determine whether magnetic resonance imaging of the fetal brain before dilation and evacuation enhances diagnosis when equivocal ultrasound findings and disrupted autopsy specimens exist. STUDY DESIGN: Patients with equivocal fetal brain abnormalities on ultrasound examination who were considering termination of pregnancy were evaluated retrospectively. Abdominal and pelvic magnetic resonance imaging was performed for further evaluation, and orthogonal fetal brain images were obtained. A multidisciplinary team reviewed all cases and discussed the fi

Subarachnoid fluid collections: a cause of macrocrania in preterm infants.

We report the outcome of 12 very low birth weight infants with macrocrania caused by subarachnoid fluid collections. By the age of 15 to 18 months, head growth had stabilized along a curve above and parallel to the 95th percentile. No infant required neurosurgical intervention, nor was cerebral palsy or mental retardation diagnosed in any of the infants.

Periventricular leukomalacia associated with hypocarbia.

We here report a case of periventricular leukomalacia (PVL) associated with hypocarbia which remained even after extubation. The patient had no risk factors affecting PVL development other than hypocarbia. We consider that the irregular tachypnea, which remained after extubation might be attributable to overdriving of ventilation of central neurogenic origin. Our patient's clinical course suggests that sodium bicarbonate drip infusion is a very effective way to alter the set point of respiratory neuronal drive of the patient with central neurogenic hyperventilation.

[Diagnosis and prognosis of periventricular leukomalacia].

Periventricular leukomalacia (PVL) is an important pathogenetic factor in cerebral palsy in premature infants. We studied 708 premature infants with gestational ages < 36 weeks, who had been admitted to the NICU of Matsudo City Hospital, examined by cranial ultrasonographic method repeatedly, and followed up for more than 1 year with cranial ultrasonography, magnetic resonance imaging (MRI), and CT scanning. We made a diagnostic protocol for PVL. All cystic lesions and periventricular echo densities (PVE) 3, and 14.9% of PVE 2 on ultrasonography were clinically diagnosed in PVL. Cranial MRI we

Multicystic encephalomalacia in term infants.

The terms "multicystic encephalomalacia" and "subcortical leucomalacia" (SCL) are used to describe the presence of areas of necrosis that develop into cystic lesions inside the brain. These lesions are generally due to severe asphyxia and/or hypotension. The designation SCL can also be used to describe the extent and the seat of the lesion in the brain or to distinguish subcortical lesions from periventricular and/or combined ones. In this study we give an account of our experience with eight newborns admitted to our neonatal intensive care unit, who presented clinical-neurological alterations

Cranial ultrasound and magnetic resonance imaging in hypoxic-ischaemic encephalopathy: a comparison with outcome.

Forty term infants with hypoxic-ischaemic encephalopathy were assessed during the neonatal period with cranial ultrasound and MRI, and the findings were compared with outcome at one year of age. 38 had abnormalities on ultrasound and all had changes on MRI. The incidence of changes in the basal banglia/thalami and periventricular white matter was much greater with MRI than with ultrasound. Changes in the basal ganglia and thalami on MRI were associated with a poor outcome if they had also been detected with ultrasound. However, MRI identified four small infarcts which were not detected by ultr

[Evaluation of cerebral hemodynamics by transcranial Doppler ultrasonography in carotid endarterectomy].

The authors studied the linear blood flow rate (LBFR) in the middle cerebral artery (MCA) which was measured by means of transcranial dopplerography during carotid endarterectomy in 25 patients. Analysis of the results showed that the changes of LBFR in the MCA correlate with alterations in blood pressure: LBFR in the MCA reduced most during compression of the common carotid artery in patients with considerable affection of the contralateral carotid artery. The maximal increase of the blood flow during compression of the common carotid artery occurred when the initial LBFR in the MCA was lowes

Transvaginal Doppler assessment of the fetal cerebral circulation.

Blood flow velocity waveforms were recorded by pulsed Doppler examination of the fetal internal carotid and middle cerebral arteries using the established transabdominal route as well as a new transvaginal approach. Fourteen normal controls and 11 pregnancies complicated by intrauterine growth retardation were studied. Satisfactory recordings were obtained in all 25 cases. There was a good correlation (R2 = 0.98) between pulsatility index (PI) values obtained transabdominally and transvaginally. The PI in the middle cerebral artery was significantly higher than in the internal carotid artery i

Transcranial Doppler ultrasonographic assessment of intermittent light stimulation at different frequencies.

Seven normal adult volunteers underwent intermittent photic stimulation at frequencies of 5-60 Hz while their posterior cerebral arteries were monitored using transcranial Doppler ultrasound. Baseline measurements were obtained under conditions of total darkness, and sampling was also done during continuous illumination. Overall variation in mean flow velocity between complete darkness and continuous illumination was 9.8%, but the maximal change (expressed as percentage deviation from baseline) occurred consistently when stimulation was undertaken at frequencies of 10 (21%) and 20 (19%) Hz (p

Tangential sulcal echoes. Potential pitfall in the diagnosis of parenchymal lesions on cranial sonography.

The sonographic appearance of normal neonatal cerebral sulci may mimic that of significant intracranial pathology. An echogenic pseudolesion can be seen when a sulcus is imaged through its long axis. Criteria are discussed for the differentiation of this normal variant from true parenchymal lesions with their attendant serious clinical implications.

Sonographic recognition of agenesis of the corpus callosum.

Agenesis of the corpus callosum may be diagnosed successfully in vivo when sonograms demonstrate absence of corpus callosum; absence of pericallosal and cingulate sulci; "sunburst" pattern of sulci along the medial surface of the hemisphere; wide interhemispheric fissure; elevation of the third ventricle; small, laterally positioned frontal horns with concave medial borders; large, laterally positioned and inverted cingulate gyri; and Probst lateral callosal bundles. Comparison of necropsy specimens with sonograms from eight patients with callosal agenesis illustrates the anatomic basis for th

[Two-dimensional ultrasonic diagnosis of brain tumors and subdural hematomas by electronic sector scanning (author's transl)].

Electronic sector scanning (ESS) is a new two-dimensional ultrasonic technique which provides continuous and instantaneous cross-sections of intracranial structures through the intact skull, including the ventricles and large cerebral arteries, by means of a stationary probe. 2. The efficiency of the ESS in the diagnosis of brain tumors and extracerebral hematomas in 295 neuroradiologically controlled cases out of an unselected number of patients during 1972-1974 is described. 3. Supratentorial brain tumors can be detected directly with the ESS by abnoramal reflections of the tumors as well as
