Glenohumeral arthrodesis improves elbow flexion in patients with adult brachial plexus injuries.

Restoring elbow flexion is a focus in treating brachial plexus injuries (BPI), aiming for anti-gravity strength (mBMRC grade ≥3). However, patients with flail or subluxated glenohumeral (GH) joints may struggle to achieve adequate strength due to energy lost stabilizing the GH joint. This study evaluates whether glenohumeral arthrodesis (GHA) can improve elbow flexion in BPI patients with prior failed elbow reconstruction and identifies preoperative factors affecting outcomes. A retrospective review of adult BPI patients with prior elbow flexion reconstruction who later underwent GHA was condu

Elbow injuries in overhead throwing athletes: clinical evaluation, treatment, and osteopathic considerations.

Injury to the elbow is very common in the throwing athlete and can potentially lead to long absences from play and, in the most severe scenarios, medical retirement. The throwing motion is a highly complex series of movements through the entire kinetic chain that results in very high angular velocities and valgus forces at the elbow joint. The repetitive nature of overhead throwing in combination with the high levels of accumulated force at the elbow puts both pediatric and adult athletes at risk of both acute and chronic overuse injuries of the elbow. This review provides an update on common

Anterolateral Thigh Flap-the Optimal Flap in Coverage of Severe Elbow Injuries.

Introduction Traditionally, the latissimus dorsi muscle with or without skin paddle has been the flap of choice for coverage of elbow defects. The ALT flap has found application in elective upper limb defects on account of it's ability to be tailor made for individual defects. Our series of 10 cases shows the advantages of using this flap for acute trauma defects. Materials and Methods Consecutive 10 cases of severe elbow injuries, involving varying amounts of the lower arm and proximal forearm underwent debridement followed by coverage using the free anterolateral thigh flap. Nine of 10 arter…

Outcomes of coronoid-first repair in terrible triad injuries of the elbow.

BACKGROUND: Clinical outcomes of terrible triad injuries (TTIs) of the elbow are historically poor. To date, it is still debatable whether the coronoid needs to be fixed and if so, how and in which sequence. METHODOLOGY: Between 2010 and 2013, 13 patients were treated surgically for acute TTIs of the elbow at a Tertiary Level 1 Trauma Centre by a single surgeon, using a standardized protocol, which included coronoid-brachialis complex fixation via pull-through trans-osseous sutures, radial head fixation or prosthetic replacement and a repair of the lateral ulnar collateral ligament. Repair of

Imaging Injuries in Throwing Sports Beyond the Typical Shoulder and Elbow Pathologies.

This review article describes injuries that occur in the upper extremities of athletes less commonly than those typically discussed with shoulders and elbows. A survey of osseous, musculotendinous, ligamentous, and neurovascular injuries is presented along with associated imaging findings and standard treatment options. This article does not focus on the classic throwing injuries of the shoulder or elbow; the goal is to survey injuries in throwing sports that involve structures away from the glenohumeral, acromioclavicular, or elbow joints. The goal of this article is to introduce readers to t

Optimizing the rehabilitation of elbow lateral collateral ligament injuries: a biomechanical study.

BACKGROUND: Elbow lateral collateral ligament (LCL) injury may arise after trauma or lateral surgical approaches. The optimal method of rehabilitating the LCL-insufficient elbow is unclear. Therapists often prescribe active motion exercises with the forearm pronated. Recently, overhead exercises have become popular as they may enable gravity to compress the elbow joint, improving stability, although this has not been proved biomechanically. This investigation aimed to quantify the effects of several variables used in LCL injury rehabilitation on elbow stability. METHODS: Seven cadaveric speci

Soft tissue constraint injuries in complex elbow instability: prevalence, pathoanatomy, and classification.

The types and prevalence of soft tissue constraint injuries associated with complex elbow instability have been rarely investigated. The purpose of this study was to analyze the intraoperative findings of soft tissue constraint injuries in complex elbow instability and provide a comprehensive classification of these lesions. Forty-seven patients undergoing surgery for complex elbow instability were prospectively analyzed. Ligament injuries were classified as simple or complex lesions, depending on whether the ligament was damaged at a single zone or 2 to 3 zones, including its proximal, middle

Nonsurgically treated terrible triad injuries of the elbow: report of four cases.

This case series describes 4 patients with terrible triad injury of the elbow (dislocation with fractures of the radial head and coronoid) who were treated nonsurgically. Following nonsurgical treatment in this select group of patients, the elbow was well aligned, and the patients regained good elbow function. Three of these 4 patients had good results. One patient had surgery for residual stiffness, ulnar neuropathy, and a radial head deformity. We conclude that, in selected terrible triad cases, when the elbow is well aligned and the radial head and coronoid fractures are relatively small an

[Complex osteoligamentary injuries of the elbow. Treatment with a hinged external fixator].

BACKGROUND: Hinged external fixation is a recognized method to treat instabilities after complex dislocations and fracture-dislocations of the elbow. The hinged external fixator allows stabilization of the elbow while preserving flexion and extension. METHODS: Eighteen patients with an average age of 47 years (range 35-67) were treated with a hinged external fixator between April 2001 and March 2006 for 6 weeks. In 11 patients an internal fixation had to be done; six were treated initially with an AO fixator. After 8 days (3-14), we changed to a hinged external fixator. RESULTS: At 39 months

Hyperbaric oxygen therapy facilitates surgery on complex open elbow injuries: preliminary results.

Complex open elbow injuries present a significant challenge to orthopaedic surgeons because of the poor potential for achieving a good functional level, even given good anatomic realignment. Associated massive soft-tissue damage impedes surgical fixation, delays rehabilitation, and therefore, further deteriorates the functional outcome. We studied a prospective, consecutive series of 16 patients with complex open elbow injuries who were treated with a combination of treatment modalities including early bony stabilization, debridement of soft tissue, and early coverage. The treatment protocol a…

MR imaging of sports injuries in the adult elbow: a tailored approach.

With an understanding of the normal anatomy of the elbow and a pertinent clinical history, MR imaging of the elbow need not be a source of fear and confusion for the MR radiologist. Because positioning and scan planning are not as straightforward as they are with other joints such as the knee or shoulder, these processes can be time-consuming. The position in which the patient is optimally scanned is inherently uncomfortable. In a busy MR center with rigorous time constraints, these factors and patient fatigue can limit the time available to acquire images. Thus, a tailored approach to designi

Complex Elbow Dislocations and the "Terrible Triad" Injury.

BACKGROUND: The elbow is the second most commonly dislocated joint in adults and up to 20% of dislocations are associated with a fracture. These injuries can be categorised into groups according to their mechanism and the structures injured. METHODS: This review includes a literature search of the current evidence and personal experiences of the authors in managing these injuries. RESULTS: All injuries are initially managed with closed reduction of the ulno-humeral joint and splinting before clinical examination and radiological evaluation. Dislocations with radial head fractures should be t

Analysis of complications after a floating elbow injury.

BACKGROUND: The aim of the present study is to analyse complications after a floating elbow injury, attempting to establish which of them act as a poor prognosis factor regarding clinical and functional results. MATERIALS AND METHODS: Twenty-three patients who suffered a floating elbow injury, treated at our institution from 2004 to 2013, were retrospectively reviewed. Patients were divided into four groups depending on the type of injury. An analysis of demographic data, associated injuries, treatment options and complications was carried out. Clinical evaluation was made by a conventional g

Terrible triad of the elbow: is it still a troublesome injury?

BACKGROUND: Terrible triad injury (TTI), one of the main patterns of complex elbow instability, is difficult to treat and yields conflicting surgical results. We analyzed prospectively a series of patient affected by TTI and treated according to the current diagnostic and surgical protocols to investigate whether their application allow to obtain more predictable outcomes. MATERIAL AND METHODS: We analyzed 26 patients with a mean age of 52 years. Preoperative X-rays and CT were performed; all patients were operated by the same elbow surgeon and underwent the same surgical and rehabilitation t

Terrible triad injury of the elbow: current concepts.

Fracture-dislocations of the elbow remain among the most difficult injuries to manage. Historically, the combination of an elbow dislocation, a radial head fracture, and a coronoid process fracture has had a consistently poor outcome; for this reason, it is called the terrible triad. An elbow dislocation associated with a displaced fracture of the radial head and coronoid process almost always renders the elbow unstable, making surgical fixation necessary. The primary goal of surgical fixation is to stabilize the elbow to permit early motion. Recent literature has improved our understanding of

Terrible triad injury of the elbow: a historical perspective.

PURPOSE: The aims of this study are to summarize (1) the history of terrible triad injury (TTi) treatment and (2) the clinical and biomechanical data that engendered its evolution. METHODS: A literature search was performed using five electronic databases. Results were discussed as a chronologic review of the relevant literature between 1920 and 2022. RESULTS: In 1962, Osborne was the first to describe a link between elbow dislocation, radial head fracture, and lateral collateral ligament complex injury via a novel mechanism: posterolateral rotatory instability (PLRI). Given that untreated e

Irreducible posterolateral elbow dislocation: a rare injury.

Posterolateral dislocation of the elbow is an injury commonly treated in the emergency department by closed reduction. Very rarely it can be irreducible and require open reduction. Only four cases of irreducible posterolateral elbow dislocation have been described in the literature over the past 50 years. We report the case of a 20-year-old man who sustained such an injury. Open reduction was performed and revealed the radial head protruding or 'buttonholing' through the lateral collateral ligament complex. This case highlights that continued closed reduction of the elbow should not be attempt
