Electronic Health Record-Linked Biobank Expansion Reveals Global Health Inequities.

Electronic health record (EHR)-linked biobanks are transforming biomedical research, enabling population-scale studies that integrate genomic, clinical, and phenotypic data. Yet as these resources proliferate, it remains unclear how their research outputs reflect global health priorities. This article presents a comprehensive review of five globally established EHR-linked biobanks: UK Biobank, the Million Veteran Program, FinnGen, the All of Us Research Program, and the Estonian Biobank. Drawing on 14,142 peer-reviewed publications from 2000 to 2024, we show how each biobank displays a distinc

Recommendations for a national electronic health record in Spain: a Delphi study.

Despite decades of health-care digitalisation efforts worldwide, health information systems remain highly fragmented, with multiple vendor-specific silos that communicate through incomplete solutions. This fragmentation prevents the creation of real-time, lifelong patient health records and becomes increasingly problematic as demand grows for person-centred care, data-driven clinical practice, and greater patient involvement in health-care decisions. To address these challenges and establish a foundation for a nationwide electronic health record (EHR), the Spanish Ministry of Health commission

Impact of an electronic health record transition on chemotherapy error reporting.

BACKGROUND: Incident reporting systems allow for frontline employees to report errors and are a critical component of healthcare patient safety programs. Although incident reporting systems cannot quantify total errors, organizations can utilize incident reporting systems to help identify risks and trends to act upon. The objective of this article is to utilize incident reporting systems to evaluate trends in medication error reporting before and after implementation of a new electronic health record system. METHODS: A five-month pre- and post-analysis was completed in a cancer hospital follo

Use of instrumental variables in electronic health record-driven models.

Precision medicine presents various methodological challenges whose assessment requires the consideration of multiple factors. In particular, the data multitude in the Electronic Health Records poses interoperability issues and requires novel inference strategies. A problem, though apparently a paradox, is that highly specific treatments and a variety of outcomes may hardly match with consistent observations (i.e., large samples). Why is it the case? Owing to the heterogeneity of Electronic Health Records, models for the evaluation of treatment effects need to be selected, and in some cases, t…

Does an Electronic Health Record Improve Completeness of Prenatal Studies?

OBJECTIVE: To determine whether implementation of an electronic health record (EHR) would increase the rate of prenatal Human Immunodeficiency Virus (HIV) and purified protein derivative (PPD) testing. METHODS: Eligible participants received prenatal care and delivered at term at a single academic institution in March-April 2011, March-April 2012, and March-April 2013. As part of routine prenatal care, all women were tested for HIV and tuberculosis (via a PPD test) during each pregnancy. The 2011 cohort was charted on paper. The 2012 and 2013 cohorts were charted via EHR. To appear in the pre

Silence in the EHR: infrequent documentation of aphonia in the electronic health record.

BACKGROUND: To begin to deliver patient-centered care, providers need to be aware of when a patient has a communication disability and what communication methods to use with the patient. The aim of the study was to describe if and how patients' communication disabilities are documented within electronic health records (EHR). METHODS: A retrospective manual chart review of all inpatient and outpatient clinical encounter notes within the EHR for patients who had undergone a laryngectomy at Northwestern Memorial Hospital (Chicago, IL) between 2000-2013. We selected patients who had undergone a l…

Use of electronic health records to characterize patients with uncontrolled hypertension in two large health system networks.

BACKGROUND: Improving hypertension control is a public health priority. However, consistent identification of uncontrolled hypertension using computable definitions in electronic health records (EHR) across health systems remains uncertain. METHODS: In this retrospective cohort study, we applied two computable definitions to the EHR data to identify patients with controlled and uncontrolled hypertension and to evaluate differences in characteristics, treatment, and clinical outcomes between these patient populations. We included adult patients (≥ 18 years) with hypertension (based on either I…

Substance use disorders and COVID-19: An analysis of nation-wide Veterans Health Administration electronic health records.

BACKGROUND: Substance use disorders (SUD) elevate the risk for COVID-19 hospitalization, but studies are inconsistent on the relationship of SUD to COVID-19 mortality. METHODS: Veterans Health Administration (VHA) patients treated in 2019 and evaluated in 2020 for COVID-19 (n=5,556,315), of whom 62,303 (1.1%) tested positive for COVID-19 (COVID-19+). Outcomes were COVID-19+ by 11/01/20, hospitalization, ICU admission, or death within 60 days of a positive test. Main predictors were any ICD-10-CM SUDs, with substance-specific SUDs (cannabis, cocaine, opioid, stimulant, sedative) explored indiv

Leveraging the Electronic Health Record to Identify and Manage Resistant Hypertension.

PURPOSE OF REVIEW: Resistant hypertension (RH) affects more than a tenth of adults and yet remains relatively underrecognized and undertreated. The electronic health record (EHR) now stands as a critical tool for overcoming barriers to the diagnosis and treatment of RH, specifically with respect to aiding in detection, excluding alternate diagnoses, and guiding clinical decision making. We review how the EHR has clarified the population level prevalence of RH and its impact on both quality of life and longevity. RECENT FINDINGS: We examine how the EHR can be leveraged to identify relevant spe

Identifying virtual care modality in electronic health record data.

BACKGROUND: Virtual care increased dramatically during the COVID-19 pandemic. The specific modality of virtual care (video, audio, eVisits, eConsults, and remote patient monitoring) has important implications for the accessibility and quality of care, but rates of use are relatively unknown. Methods for identifying virtual care modalities, especially in electronic health records (EHR) are inconsistent. This study (a) developed a method to identify virtual care modalities using EHR data and (b) described the distribution of these modalities over a 3-year study period. METHODS: EHR data from 31…

Perspective: leveraging the electronic health record to improve sleep in the hospital.

UNLABELLED: Inpatient sleep loss can worsen health outcomes, including delirium and falls. Sleep disruptions in the hospital often originate from provider-patient interactions ordered electronically through computerized provider order entry. These orders contain clinical decision support systems with default schedules. These defaults are often around-the-clock, may not align with patients' needs, and cause iatrogenic sleep loss. Optimizing clinical decision support in the electronic health record can decrease unnecessary sleep disruptions and influence sleep-friendly decision-making. CITATION

Optimal sampling for positive only electronic health record data.

Identifying a patient's disease/health status from electronic medical records is a frequently encountered task in electronic health records (EHR) related research, and estimation of a classification model often requires a benchmark training data with patients' known phenotype statuses. However, assessing a patient's phenotype is costly and labor intensive, hence a proper selection of EHR records as a training set is desired. We propose a procedure to tailor the best training subsample with limited sample size for a classification model, minimizing its mean-squared phenotyping/classification er…
