Net Endogenous Acid Excretion and Kidney Allograft Outcomes.

BACKGROUND AND OBJECTIVES: High dietary acid load may accelerate a decline in kidney function. We prospectively investigated whether dietary acid load is associated with graft outcomes in kidney transplant recipients, and whether venous bicarbonate mediates this association. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: We used data from 642 kidney transplant recipients with a functioning graft ≥1 year after transplantation. Net endogenous acid production was estimated using food frequency questionnaires and, alternatively, 24-hour urinary urea and potassium excretion to estimate net endogen

Food-chemical intolerance in the breastfed infant.

Adverse food reactions can be caused by a wide range of foods and the reactions vary widely too. Lactation consultants and other health professionals should look for causes of infant symptoms associated with less than optimal breastfeeding management and rule out medical issues before assuming adverse food reactions are involved. The investigation of adverse food reactions can be complex and ideally should be guided and monitored by a dietitian experienced in the field. Maternal elimination diets require high motivation and must include all important nutrients.

Paediatric Patients with Coeliac Disease on a Gluten-Free Diet: Nutritional Adequacy and Macro- and Micronutrient Imbalances.

PURPOSE OF REVIEW: A strict, lifelong gluten-free diet is the cornerstone for management of coeliac disease. Elimination of gluten from the diet may be associated with nutritional imbalance; however, the completeness of this diet in energy and macro- and micronutrients in children is not well described. Understanding the nutritional adequacy of the gluten-free diet in children during this critical period of growth and development when dietary intake is strongly influential is important. RECENT FINDINGS: Children, regardless of whether they have eliminated gluten from their diet, have a tenden

Influence of Eating Behavior and Dietary Patterns on Gut Microbiota Formation in Children with Autism Spectrum Disorder.

Background/Objectives: Autism spectrum disorder (ASD) is often associated with gastrointestinal dysfunction and gut microbiota alterations. This study aimed to characterize the gut microbiota in children with ASD in relation to nutritional factors and to evaluate the effects of dietary interventions combined with probiotics. Methods: The study included 96 children with ASD and 39 neurotypical controls. Follow-up data after intervention were available for 60 children with ASD. Gut microbiota composition was assessed by 16S rRNA sequencing, and fecal calprotectin and zonulin were measured before

Changes in gut microbial metagenomic pathways associated with clinical outcomes after the elimination of malabsorbed sugars in an IBS cohort.

Specific diets to manage sugar malabsorption are reported to reduce clinical symptoms of irritable bowel syndrome (IBS). However, the effects of diets for malabsorbed sugars on gut microbiota signatures have not been studied, and associations with clinical outcomes in IBS have not been characterized. 22 IBS patients positively tested for either lactose-, fructose-, sorbitol- or combined malabsorptions were subjected to 2-weeks sugar elimination and subsequent 4-weeks re-introduction. 7 IBS patients tested negative for sugar malabsorption were used as controls. Nutrition and clinical symptoms w

Utility of the immunoglobulin E crosslinking-induced luciferase expression assay in buckwheat allergy diagnosis.

BACKGROUND: Buckwheat allergy carries a high risk of anaphylaxis. Although the oral food challenge (OFC) is the diagnostic gold standard, it can provoke severe reactions, leading many patients to adopt elimination diets. Conventional markers, including buckwheat-specific IgE (Bw-sIgE) and skin prick testing, have limited accuracy. The IgE crosslinking-induced luciferase expression (EXiLE) assay is a serum-based in vitro cell-activation test that has shown high accuracy for other food allergies. We evaluated its utility for buckwheat allergy. METHODS: This multicenter observational study in Ja…

Efficacy of a Dairy-Free Diet in Adult Patients With Eosinophilic Esophagitis: A Pilot Prospective Multicenter Study.

BACKGROUND: Eosinophilic esophagitis (EoE) is a chronic, immune-mediated disease in which elimination diets represent an effective therapeutic option. Although six-food elimination diets (6-FED) achieve high remission rates, their complexity often compromises adherence. Dairy-free diet (DFD) has emerged as a simpler alternative, yet evidence in adult populations remains limited. AIMS: To evaluate the clinical, endoscopic, and histological outcomes of a 12-week DFD in adult patients with EoE and to explore potential predictors of histological remission and dietary adherence. METHODS: We condu…

An oat fiber intervention for reducing PFAS body burden: A pilot study in male C57Bl/6 J mice.

Perfluoroalkyl substances (PFAS) are a major public health concern, in part because several PFAS have elimination half-lives on the order of years and are associated with adverse health outcomes. While PFAS can be transported into bile, their efficient reuptake by intestinal transporter proteins results in minimal fecal elimination. Here, we tested the hypothesis that consumption of oat β-glucan, a dietary supplement known to disrupt the enterohepatic recirculation of bile acids, will reduce PFAS body burdens. Male C57Bl/6 J mice were fed diets based on the "What we eat in America" analysis th

Chronic nutritional restriction of omega-3 fatty acids induces a pro-inflammatory profile during the development of the rat visual system.

Modern western diets have been associated with a reduced proportion of dietary omega-3 fatty acids leading to decreased levels of DHA (docosahexaenoic acid) in the brain. Low DHA content has been associated with altered development of visual acuity in infants and also with an altered time course of synapse elimination and plasticity in subcortical visual nuclei in rodents. Microglia has an active role in normal developmental processes such as circuitry refinement and plasticity, and its activation status can be modulated by omega-3 (ω3) and omega-6 (ω6) essential fatty acids. Eosinophilic esophagitis (EoE) is an antigen-triggered chronic disease of increasing prevalence in children and adults. Untreated or therapy-resistant EoE leads to complications of esophageal food impactions and luminal narrowing. The past decade has seen a number of new clinical trials for EoE therapies including elimination diets, proton pump inhibitors, swallowed topical corticosteroids, and biologics. In this review, we comment on the current state of the art for dietary and pharmacologic management of EoE and the need for future clinical trials to help tailor therapies to individual patie…

Heredopathia atactica polyneuritiformis phytanic-acid storage disease, Refsum's disease:" a biochemically well-defined disease with a specific dietary treatment.

Heredopathia atactica polyneuritiformis (phytanic-acid storage disease, Refsum's disease) is an inborn error of metabolism, in which the body accumulates exogenous phytanic acid. The severe manifestations of this disease, which include pigmentary retinal degeneration, chronic polyneuropathy, ataxia, impaired hearing, and cardiopathy, can be either kept from worsening or reversed by elimination of foods rich in phytanic acid from patients' diets. This is borne out by a 20-year experience with two patients, whose conditions improved markedly once they stopped eating butter, animal fat, and other

Dietary Management of Eosinophilic Esophagitis.

Eosinophilic esophagitis (EoE) is a chronic immune-mediated food antigen-driven disease characterized by tissue eosinophilia and clinical symptoms of esophageal dysfunction. Medical and dietary therapies can be offered as treatment options in both pediatric and adult populations. Advances in nutritional research in EoE have produced different levels of dietary restriction, ranging from elimination of a single food group to more extensive restriction such as the two-food elimination diet, four-food elimination diet, or six-food elimination diet. Efficacy and outcomes vary for each level of rest

Children with eczema and sensitization to foods are recommended skin care and, if food allergy is proven by challenge, an elimination diet. For most children the diet period is transient, but the process behind tolerance development and the influence of decreased allergen exposure is not fully known. The aim of the study was to investigate the effect of elimination diet on serum and salivary antibodies and to identify immunological parameters related to the ability to tolerate foods. Eighty-nine children, below 2 yr of age, with eczema and suspected food allergy were included. Recommended trea

Milk-free diet followed by breastfeeding women.

BACKGROUND: Breastfeeding is the most common way of feeding infants. Human milk contains nutrients which are necessary to provide proper growth of a child. Nowadays there aren't any recommendations to follow elimination diet in order to decrease the risk of allergy or baby colic. Only the occurrence of health problems such as lactose intolerance or cow's milk protein allergy in breastfeeding mother or infant should be a reason for eliminating dairy products from diet. It seems to be important to explore the reasons and the frequency of following milk-free diet by breastfeeding women. OBJECTIV
