Endolymphatic hydrops induced by immune response of the endolymphatic sac: relation to perilymph antibody levels.

This study investigated the effect of a secondary immune response in the endolymphatic sac on the development of endolymphatic hydrops in guinea pigs. Following secondary keyhole limpet hemocyanin (KLH) challenge directly to the sac in systemically presensitized animals, hydrops rapidly developed within 1 week, then gradually reduced by 4 weeks. After 5 weeks, hydrops gradually recurred and developed again. Mean perilymph anti-KLH antibody levels rose from day 1 to 4 weeks and then decreased. The elevation of perilymph anti-KLH antibody levels on day 2 was due to an increased vascular permeabi

Profound sensorineural hearing loss in ears with endolymphatic hydrops.

BACKGROUND: Recently, Hydrops MRI has unexpectedly identified endolymphatic hydrops (EH) in patients presenting with profound sensorineural hearing loss (SNHL), highlighting a discrepancy that prompted further investigation. OBJECTIVE: This study aimed to elucidate the mechanism of profound SNHL in ears with EH. METHODS: Six (3%) of 184 patients with positive EH on Hydrops MRI presented with profound SNHL. Three of 6 patients had been previously diagnosed as Meniere's disease, and 4 patients were compromised hosts. All patients underwent an inner ear test battery followed by Hydrops MRI. RE

Endolymphatic hydrops presumption tests for patients with vestibular migraine.

BACKGROUND: The aetiology of vestibular migraine (VM) has not yet been defined; endolymphatic hydrops (EH) has been suggested as a candidate. OBJECTIVES: This study aimed to clarify the relationship between VM and EH using neuro-otological tests, including the EH presumption test. MATERIALS AND METHODS: Fourteen patients with VM underwent caloric testing, video head impulse test (vHIT), cervical and ocular vestibular evoked myogenic potential (cVEMP and oVEMP), and EH presumption tests such as the Futaki's test and furosemide loading VEMP. RESULTS: Caloric testing was abnormal in two of the

Endolymphatic hydrops and ionic transporters: genetic and biohumoral aspects.

Ménière's disease (MD) is an inner ear disorder, characterized by a burden of symptoms, probably arising from the interplay of genetic and environmental factors. In this brief review, we consider the role of ion channels and transporters in the pathophysiology of MD, focusing on genetic and biohumoral aspects. Pathophysiological mechanisms related to altered concentrations of ions in the endolymph include altered osmotic pressure leading to hydrops and/or immunomodulatory effects of K+ and Endogenous Ouabain (EO) concentrations in the inner ear. Aquaporins 1-5 (AQPs) have been found in the inn

Unilateral Enlarged Vestibular Aqueduct Syndrome and Bilateral Endolymphatic Hydrops.

Enlarged vestibular aqueduct (EVA) syndrome is a common congenital inner ear malformation characterized by a vestibular aqueduct with a diameter larger than 1.5 mm, mixed or sensorineural hearing loss that ranges from mild to profound, and vestibular disorders that may be present with a range from mild imbalance to episodic objective vertigo. In our study, we present the case of a patient with unilateral enlarged vestibular aqueduct and bilateral endolymphatic hydrops (EH). EH was confirmed through anamnestic history and audiological exams; EVA was diagnosed using high-resolution CT scans and

Vestibular Schwannoma and Ipsilateral Endolymphatic Hydrops: An Unusual Association.

Vestibular schwannoma and endolymphatic hydrops are two conditions that may present with similar audio-vestibular symptoms. The association of the two in the same patient represents an unusual finding that may lead clinicians to errors and delays in diagnosis and clinical management of affected subjects. We discuss the case of a patient with an intrameatal vestibular schwannoma reporting symptoms suggestive for ipsilateral endolymphatic hydrops. The patient presented with fluctuating hearing loss, tinnitus, and acute rotatory vertigo episodes, and underwent a full audiological evaluation and i

Detection of endolymphatic hydrops using traditional MR imaging sequences.

PURPOSE: The purpose of this study was to determine whether Meniere's disease (MD) produces endolymphatic cavity size changes that are detectable using unenhanced high-resolution T2-weighted MRI. MATERIALS & METHODS: This retrospective case-control study included patients with documented MD who had a high-resolution T2-weighted or steady-state free procession MRI of the temporal bones within one month of diagnosis, between 2002 and 2015. Patients were compared to age- and sex- matched controls. Cross sectional area, length, and width of the vestibule and utricle were measured in both ears alo

Does the intravenous administration of frusemide reduce endolymphatic hydrops?

OBJECTIVE: To verify the hypothesis that intravenous frusemide reduces endolymphatic hydrops, as evaluated by three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging following intratympanic gadolinium administration. METHODS: The study comprised 12 patients (7 females and 5 males, aged 19-74 years) with Ménière's disease. Disease duration ranged from 0.5 to 8 years, with a frequency of 0.5 to 6 vertigo spells per month, as calculated in the last 6 months. Three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging was performed 24 hours after intr

Endolymphatic hydrops and Ménière's disease: a lesion meta-analysis.

OBJECTIVE: To determine whether the distribution of membrane lesions associated with Ménière's disease is random, as might be expected from a pervasive process such as hydrops, or orderly, as might be expected if membrane resistance is graded. METHOD: A meta-analysis of temporal bone reports on 184 specimens demonstrating endolymphatic hydrops was undertaken to determine membrane lesion evolution and distribution. RESULTS: Lesion distribution was found to be orderly and cochleocentric. No random scattershot lesions were reported in any study. Disease always started in the cochlear apex, even

Relationship between endolymphatic hydrops and vestibular-evoked myogenic potential.

CONCLUSION: Vestibular-evoked myogenic potential (VEMP) can be used to examine endolymphatic hydrops, especially in the vestibule. OBJECTIVE: To investigate the relationship between the degree of endolymphatic hydrops revealed by magnetic resonance imaging (MRI) and VEMP. METHODS: Gadolinium diluted with saline was injected intratympanically in 49 ears (40 patients). One day after the injection, the endolymphatic space in the vestibule and the cochlea was visualized by 3 Tesla MRI. A VEMP test was done, and VEMP was judged as absent when the VEMP was within the noise level. RESULTS: VEMP wa

Endolymphatic hydrops in otologic syphilis: a temporal bone study.

HYPOTHESIS: Endolymphatic hydrops in temporal bones with otologic syphilis directly relates to osteitis, new bone formation, and/or resorption along the course of the endolymphatic system. BACKGROUND: Ménière's disease and otosyphilis both cause progressive endolymphatic hydrops, and the mechanism of hydrops formation in each disease process remains unclear. Traditionally, osteitic changes of the labyrinthine capsule were thought to lead to endolymphatic hydrops in syphilitic temporal bones. More recently, authors have suggested that microgummata and inflammatory change obliterating the endol

Allopurinol attenuates endolymphatic hydrops in the guinea pig cochlea.

Based on the hypothesis that Ca(2+) overload in the scala media may produce endolymphatic hydrops and generate free oxygen radicals (FOR), allopurinol, a xanthine oxidase inhibitor and free radical scavenger, was administered to guinea pigs after the surgical obliteration of the endolymphatic duct. Allopurinol was given intraperitoneally (50 mg/kg/day) for 15 days starting 1 day prior to the surgical blockage procedure. Measurements from histological serial sections of these temporal bones showed that the total volume of the scala media was significantly reduced (p = 0.007) compared with contr

Immunoglobulin E and circulating immune complexes in endolymphatic hydrops.

Levels of circulating immune complexes (CICs) in 59 patients with Meniere's disease and total serum immunoglobulin E (IgE) levels in a subgroup of 42 of the 59 were determined quantitatively for possible abnormalities of humoral immunity. Significant differences in average IgE levels between the 42 patients with Meniere's disease and 18 normal control subjects were not determined; however, five (11.9%) of the 42 patients were found to have obviously raised IgE levels. Elevated CIC levels were found in 19 (32.2%) of the 59 patients with Meniere's disease and in one (2.3%) of the 43 control subj

Delayed endolymphatic hydrops and its relationship to Meniére's disease.

Delayed endolymphatic hydrops (EH) can be characterized as having ipsilateral and contralateral types. They are similar in that both have early and late phases of otologic symptoms and that the early phase is a profound hearing loss in one ear. The late phases differ, however, in that the ipsilateral type develops the symptoms of EH (episodic vertigo) in the deaf ear and the contralateral type develops the symptoms of EH (fluctuating hearing loss and/or episodic vertigo) in the hearing ear. In more than half the cases of both types of delayed EH, the profound hearing losses in the early phase

Measurement of action potential thresholds in experimental endolymphatic hydrops.

Unilateral experimental endolymphatic hydrops was created by endolymphatic sac and duct obstruction in guinea pigs. Studies of action potential (AP) threshold were then performed and compared with histologic findings. Action potential thresholds were significantly increased in the hydrops ears. The close relationship between AP changes and hydrops was sufficient to allow the detection of hydrops by AP threshold measurements alone. The AP threshold changes were positively related to the degree of hydrops and postoperative interval in a manner consistent with hearing changes seen in Meniere's di
