The incidence of bacteremia associated with endoscopy varies widely for different endoscopic procedures. This study prospectively evaluates the incidence of bacteremia in patients undergoing elective endoscopic laser therapy for two diseases of the upper gastrointestinal tract. In five patients (eight treatment sessions) with arteriovenous malformations, no bacteremia developed. In 15 patients (26 treatment sessions) with esophagogastric carcinoma, bacteremia developed at some time during the procedure in six (40%). However, bacteremia appeared to be associated with the endoscopic insertion th

Malignant melanoma is the most common malignancy to metastasize to the gastrointestinal tract. In a retrospective computer-assisted data search of over 2500 patients with melanoma registered over the past 10 years, 110 patients have been identified to have premortem gastrointestinal metastatic disease (metastatic disease identified at least 6 months before death). The small intestine (35%), colon (14.5%), and stomach (7%) are the most common sites for metastases. Polypoid or ulcerating masses and intramucosal nodules are typical radiologic presentations for gastric and colonic lesions, while o

Esophagogastroduodenoscopy has become the most accurate diagnostic method for identifying lesions of the upper gastrointestinal (GI) tract. It permits thorough inspection and a direct biopsy yielding a tissue diagnosis of surface lesions from the upper part of the esophagus to the second portion of the duodenum. The surgeon responsible for patients with upper GI tract disease should perform the endoscopic examination to gain first-hand information on the nature, extent, and location of the patient's problem. The surgeon-endoscopist gains an extra advantage, since the size and proximity of a le

A new technique for the endoscopic control of upper gastrointestinal hemorrhage has been developed. It consists of a spraying device which delivers atomized clotting factors (thrombin and fibrinogen) directly to a focal bleeding site. In vivo studies in a standardized dog model of acute gastric ulcer bleeding, demonstrated a significant reduction in bleeding time by spraying thrombin and cryoprecipitate (mean +/- SE bleeding time = 223 +/- 14 sec) as compared to a saline spray (mean 705 +/- 60 sec) or to the untreated controls (mean 498 +/- 43 sec), P less than 0.001. Similar results were obta

Endoscopic resection (ER) has become the preferred curative-intent treatment for early upper gastrointestinal cancer, given its superior safety profile compared to surgery. Over the past decade, technological and procedural innovation has substantially expanded the scope, safety, and precision of endoscopic submucosal dissection (ESD) and related techniques. This review synthesises current evidence on key advances relevant to upper gastrointestinal ESD practice. Enhanced imaging modalities have improved lesion detection and characterisation, as well as recognition of intraoperative anatomical

Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal neoplasms that commonly originate from the kidney, uterus, or lung, with gastrointestinal involvement being exceptionally uncommon. This case highlights the first known use of endoscopic ultrasound and endoscopic tunnel biopsy technique in diagnosing rectal PEComa. A 66-year-old woman presented with acute rectal bleeding. Imaging revealed a distal rectal mass 7.4 cm in diameter that was initially suspected to be a gastrointestinal stromal tumor. Endoscopic ultrasound with fine-needle aspiration and subsequent endoscopic tunnel

Miniprobe endoscopic ultrasonography (mEUS) combines high-resolution imaging of the gastrointestinal (GI) wall and bile ducts with ease of applicability during routine endoscopy. This narrative review aims to provide an overview of known and emerging fields of application for mEUS in gastrointestinal endoscopy. After its initial development in pancreatobiliary scenarios in the early 1990s, mEUS has been recently reconsidered a third-space endoscopic technique that is progressively developing and spreading for the treatment of early gastrointestinal neoplastic lesions. The high spatial resoluti

This multicenter retrospective study included patients undergoing EUS-guided GI anastomoses from 2016 to 2023. Indications for EUS-guided anastomosis were GOO, ALS or patients with altered anatomy needing endoscopic interventions. The primary outcome was technical success, while secondary outcomes included clinical success, safety, lumen-apposing metal stent (LAMS) patency, and the need for reinterventions. A total of 216 patients (mean age 64.5 [±13.94] years; 49.1% males) were included. In total, 149 cases (69%) were GOO, 44 (20.4%) cases were bilioenteric anastomotic strictures or lithiasis

In this article we comment on the paper by Xu et al describing retrospective data on endoscopic treatment outcome of esophageal gastrointestinal stromal tumors (GISTs). Esophageal GIST is a rare type of mesenchymal tumor. GISTs originate from the interstitial cells of Cajal, which are pacemaker cells involved in gut motility. GISTs are most commonly found in the stomach and small intestine, but esophageal involvement is rare. Esophageal GISTs account for < 1% of all GISTs. Endoscopic resection remains the mainstay for small, localized tumors with excellent outcomes. However, larger tumors may

Upper gastrointestinal bleeding (UGIB) is a major cause of morbidity and mortality. Clinical symptoms that patients may present with include: hematemesis, coffee-ground emesis, melena, and hematochezia. Clinical signs can range from tachycardia to shock. The anatomical landmark that differentiates upper gastrointestinal (GI) bleeds from lower bleeds is the ligament of Treitz. The first steps of treating a patient who presents with signs of UGIB are resuscitation with appropriate fluids and blood products as necessary. The consideration of endoscopy and the urgency at which it should be perform

BACKGROUND AND STUDY AIMS: Systemic autoinflammatory diseases are characterized by recurrent or chronic inflammation, and monogenic forms are increasingly defined. However, a group of patients without genetic diagnosis is called the syndrome of undifferentiated recurrent fever (SURF). This study analyzed the clinical and endoscopic features of patients with SURF presenting with gastrointestinal (GI) symptoms.
PATIENTS AND METHODS: Between 2019 and 2022, GI endoscopy were performed in patients with SURF who presented with GI symptoms. Clinical, genetic, laboratory, and endoscopy findings were

OBJECTIVE: Lymphagioma, which in most cases as benign tumors, occurs in head, neck, axilla, and mediastinum. Lymphangioma is exceedingly rare in the upper gastrointestinal tract including esophagus, stomach, and duodenum. However, the clinical characteristics, natural history, and recurrence rate after endoscopic resection remain unclear. This study aims to evaluate the characteristic findings and assess the efficacy of endoscopic techniques in the management of this disease.
METHODS: In this systematic retrospective analysis, we evaluated all 24 cases of upper gastrointestinal lymphangioma r

Endoscopic cryotherapy is a technique utilized for the ablation of target tissue within the gastrointestinal tract. A cryotherapy system utilizes the endoscopic application of cryogen such as liquid nitrogen, carbon dioxide or liquid nitrous oxide. This leads to disruption of cell membranes, apoptosis, and thrombosis of local blood vessels within the target tissue. Several trials utilizing cryotherapy for Barrett's esophagus (BE) with variable dysplasia, gastric antral vascular ectasia (GAVE), esophageal carcinoma, radiation proctitis, and metastatic esophageal carcinomas have shown safety and
