Ethanol is most commonly consumed orally, although rare cases of intentional rectal administration have been described. We present the case of a 37-year-old man who presented to the emergency department following intentional rectal administration of ethanol and a concomitant acetaminophen overdose with extensive review of literature. This unusual presentation highlights the diagnostic and management challenges associated with concurrent toxic exposures and emphasizes the importance of early recognition and multidisciplinary care.

Urologists often care for adult patients with neurogenic bladder. Adult patients with neurogenic bladder may have concommitant neurogenic bowel. Neurogenic bowel may be refractory to conservative management. The Malone antegrade continence enema (ACE) has been successful in pediatric neurogenic bowel. Little experience has been described among adult neurogenic bowel patients. We describe the technique of the ACE and review our results for adult neurogenic bowel.

Intussusception is a common pediatric condition in China. Hydrostatic reduction with barium sulfate or pneumatic reduction using air and fluoroscopy has become the standard method in most centers throughout the world. To avoid radiation exposure, reduction without fluoroscopic confirmation was studied. Between 1981 and 1985 a total of 224 children were diagnosed and treated for intussusception. OBJECTIVE: To report a suicide attempt with an aspirin enema.
CASE SUMMARY: A patient presented to the emergency room after self-administering, in enema form, approximately 700 aspirin tablets dissolved in water. Over the next 12 hours the patient became progressively acidemic with eventual cardiac arrest and subsequent chronic hypoxic encephalopathy.
DISCUSSION: This patient's poor outcome was the result of retained aspirin products in the rectal vault combined with the failure to recognize the delayed absorption properties of rectally administered aspirin.
