Neck stiffness in a post-irradiated nasopharyngeal carcinoma adult: An unusual diagnosis.

Nasopharyngeal carcinoma is a neoplasm commonly found in population of South East Asia. The mainstay of treatment is high dose irradiation. Complications from radiotherapy are not uncommon especially to those nearby structures such as vertebrae and spinal cord. A 57 year-old gentleman with nasopharyngeal carcinoma (NPC) who was treated with chemo-radiation (total of 35 fractions,70Gy) presented to us 6 months post therapy with bilateral nasal discharge and progressive neck stiffness. Nasoendoscopy showed inflamed nasophayngeal mucosa and Computed Tomography (CT) brain and cervical spine showed

Symptomatic hematoma of cervical ligamentum flavum: case report.

STUDY DESIGN: A case of symptomatic hematoma of cervical ligamentum flavum. OBJECTIVE: To report the first ligamentum flavum hematoma in the cervical spine and review the reported cases. SUMMARY OF BACKGROUND DATA: A herniated nucleus pulposis, spondylosis, epidural hematoma or abscess, neoplasm, or some pathology of the ligamentum flavum, such as hypertrophy, ossification, or calcification, are the most common causes of spinal cord and nerve root compression. A ligamentum flavum hematoma has also been reported as a cause of compression of the cauda equina and lumbar nerve roots but has neve

Thoracic Epidural Analgesia in Major Cancer Surgery: An Update.

Background and Objectives: Postoperative pain after major cancer surgery contributes to pulmonary, cardiovascular, metabolic, and functional complications. Thoracic epidural analgesia [TEA] has traditionally been considered the standard analgesic technique for open thoracic and upper abdominal oncologic surgery, although minimally invasive techniques and ERAS pathways have prompted a re-evaluation of its role. Methods: This narrative review summarizes evidence from PubMed and MEDLINE published between 1995 and June 2026, emphasizing randomized trials, systematic reviews, meta-analyses, large o

Hodgkin Lymphoma revealed by epidural spinal cord compression.

CONTEXT: Hodgkin Lymphoma is rarely diagnosed as spinal cord compression syndrome. Caused by an epidural mass, this complication is often encountered in a late stage of the disease. We report the case of a 40-year-old man presenting with symptoms of low thoracic spinal cord compression due to an epidural tumor on the MRI. FINDINGS: Emergent surgery was undertaken on this patient, consisting in laminectomy and tumor resection. After surgery, pain relief and mild neurological improvement were noticed. The histological study revealed a Hodgkin Lymphoma and the patient was referred to chemotherap

[A case of peripheral T cell lymphomas-unspecified in vertebra canal].

Peripheral T cell lymphomas-unspecified (PTCL-U) is an uncommon malignant tumor, accounting for 5%-7% of non-Hodgkin's lymphoma. Clinical feature of a case of PTCL-U was investigated and the optimal treatment protocol was proposed. The clinical presentation of this case was progressing limbs weakness with sudden paralysis. Multiple superficial lymph nodes swelling and decreased sensation in all modalities below T10 level were found in physical examination. Bilateral leg paralysis (Grade 0/5) with high muscle tension, overactive knee reflex, bilateral Babinski sign (+) were present. Magnetic re

[Supportive measures in palliation: pain therapy and nutrition].

Pain relief and nutritional support represent two main efforts of palliative medicine. A considerable proportion of surgical patients might not be treated with adequate analgetic medication. Those patients are often treated too late, too short or with an insufficient amount of drug. Particularly if the treatment goal is palliation problems of drug abuse are of less importance. However, randomized trials aiming at best pain relief have rarely been carried out in oncological patients. Psychological factors (suffering, affective aspects) have to be borne in mind when deciding upon pain treatment.

[Intrathecal analgesia. Apropos of 50 cases].

Fifty patients with intractable pain, mainly of neoplastic origin, were treated by morphine through unidose drug delivery system. Criteria of selection of the patients and technical procedures are reported. Most common side effects are nausea and dysuria but can be effectively prevented. The most severe complications are leakage of cerebro spinal fluid with or without meningitis. The success rate at 20 days is 80% as well in pelvic pain as in subdiaphragmatic extra cephalic pain.

Diagnosis and treatment of epidural metastases.

Epidural metastases occur in 5% to 10% of cancer patients and represent a neurological emergency. Patients most commonly present with an acute onset of motor weakness, and restoration of neurological function is critically dependent on prompt diagnosis and treatment. This review discusses the clinical, epidemiological, and radiological features associated with epidural metastases and resulting spinal cord compression. Moreover, current treatment paradigms are reviewed. The timely initiation of radiation as well as surgery in select cases is critical for preserving neurological function and ach

History of Evolution of Posterolateral Approaches to the Thoracic Spine: From Cure of Pott's Disease to Epidural Tumor Resection.

Since the end of the nineteenth century, the wide dissemination of Pott's disease has ignited debates about which should be the ideal route to perform ventrolateral decompression of the dorsal spine in case of paraplegia due to spinal cord compression in tuberculosis spondylitis. It was immediately clear that the optimal approach should be the one minimizing the surgical manipulation on both neural and extraneural structures while optimizing the exposure and surgical maneuverability on the target area. The first attempt was reported by Victor Auguste Menard in 1894, who described, for the firs

Cephalad migration of morphine in CSF following lumbar epidural administration in patients with cancer pain.

This study examines the cephalad migration of morphine in CSF following lumbar epidural administration in cancer patients with pain. Fourteen cancer patients were administered 10 mg of morphine in 10 ml of normal saline via an epidural catheter inserted in the lumbar region (usually L2.3) and attached to a subcutaneously implanted portal for drug administration. There was a rapid vascular uptake of morphine from the epidural space with a mean (+/- S.D.) peak blood concentration of 110 +/- 32 ng/ml (range 76-182 ng/ml and the mean time associated with this peak blood concentration was 5.1 +/- 2

Spinal cord compression: with special reference to metastatic epidural tumors.

Compression of the spinal cord or cauda equina is an increasingly important problem in cancer patients. The most common primary tumors are carcinoma of the breast in women and carcinoma of the lung in men. Metastases invade the epidural space by extension from involved vertebrae, by growth through intevertebral foramina, and by hematogenous dissemination. Pain is the usual initial symptom, followed by progressive sensory, motor, and sphincteric dysfunction. Appropriate x-ray films of the spine are positive in most patients, and complete myelographic examination to delineate the exact location

Tumor-to-tumor metastasis of lung adenocarcinoma into a spinal intradural solitary fibrous tumor: a case report.

Solitary fibrous tumors (SFTs) are rare mesenchymal neoplasms commonly involving visceral or parietal pleura. We present the first report of tumor-to-tumor metastasis involving a pulmonary adenocarcinoma donor and an intradural SFT recipient. The patient presented with a 1 year history of diffuse back pain. A spinal intradural contrast-enhancing mass at the T9/10 level and a tumor of the lung were diagnosed radiologically. Bronchoscopic biopsy confirmed pulmonary adenocarcinoma in the right upper lung lobe. Due to deteriorating neurological status with conus medullaris syndrome, we performed a
