PURPOSE: Continence is a difficult goal in exstrophy-epispadias complex repair. A case we believe to be the longest with successful anatomic closure of exstrophy and epispadias witholt reflux or incontinence is reported. The bladder exstrophy-epispadias complex (BEEC) is an abdominal midline malformation comprising a spectrum of congenital genitourinary abnormalities of the abdominal wall, pelvis, urinary tract, genitalia, anus, and spine. The vast majority of BEEC cases are classified as non-syndromic and the etiology of this malformation is still unknown. This review presents the current knowledge on this multifactorial disorder, including phenotypic and anatomical characterization, epidemiology, proposed developmental mechanisms, existing animal models, and implicated genetic and environmental components.

INTRODUCTION: Complete female epispadia (CFE) is a rare congenital anomaly occurring in 1 of 500,000 live births. The goals of CFE management include achieving continence, while protecting the kidneys, and creating functional, cosmetic external genitalia.
PATIENT AND METHODS: We demonstrate in this video the steps of the surgery and present midterm follow-up of a 6-year-old child with CFE.
RESULTS AND DISCUSSION: The patient had an excellent cosmetic result, and reported continence of 3 h period with mild leakage. The perineal infrapubic approach offers the possibility of restoring cosmesis

Isolated female epispadias without exstrophy is an extremely rare syndrome. The symptoms of female epispadias are primary urinary incontinence and abnormal anatomical features. A 12-year-old girl presented with primary urinary incontinence. On physical examination, bifid clitoris and labia minora were seen. The vagina and hymen were normal. Voiding cystourethrogram showed no reflux. With the diagnosis of isolated female epispadias, one-stage reconstruction of the urethra, bladder neck and labia minora and clitoris was performed.

This review of the complications of all methods of modern treatment of bladder exstrophy emphasizes the complexity of reconstruction of the bladder exstrophy spectrum. The main complications of any method of primary bladder exstrophy closure are complete wound dehiscence, bladder prolapse and urethral outlet obstruction; others include bladder and renal calculi. These complications as well as methods to avoid them are discussed here. Evidence supporting the management strategy is presented where possible.

PURPOSE: We present a multicenter experience using the Mitchell epispadias technique to determine if satisfactory results could be obtained by various pediatric urologists at multiple centers using the same technique to repair epispadias.
