[The acrodynic form of the Riley-Day syndrome].

The authors report the case of a 10-year-old girl who died after suffering from severe erythromelalgia combined with digestive and general disorders associated with multiple vegetative disturbances (high blood pressure, hypothermia, colic and urinary disorders, raised catecholamine levels) for 2 years. Although the appearance of the tongue was normal and in spite of the absence of genetic criteria the diagnosis of Riley-Day dysautonomia was made and its association with neural crest disorders was indicated.

[Hemichorea caused by polycythemia vera].

Polycythaemia vera (PV) is a chronic myeloproliferative disorder. PV should be suspected with hemoglobin >10.2/9.9 mmol/L (m/f) and/or PV-related features (aquagenic pruritus, erythromelalgia, atypical thrombosis, splenomegaly, thrombocytosis or leukocytosis). This case report highlights the connection between PV and neurological manifestations, including rare symptoms like chorea. An 82-year-old woman presented with sudden choreiform movements, dysarthria, and behavioral changes. Investigations ruled out primary neurological disease and confirmed PV. With treatment, the neurological symptoms

[Diabetic neuropathy: do not only consider distal symmetrical neuropathy].

BACKGROUND: Diabetic neuropathy is a common complication of diabetes mellitus. The length-dependent symmetrical sensorimotor type of neuropathy is the most prevalent form of diabetic neuropathy but other forms of diabetic neuropathy also need to be kept in mind. Their differential diagnosis is often more challenging but implicates specific forms of treatment other than improvement of metabolic control. AIM OF THE STUDY: This article gives an overview of the less frequent forms of diabetic neuropathy and discusses their impact, diagnostic and therapeutic implications. RESULTS: Autonomic diabe

Small-fibre neuropathies and skin: news and perspectives for dermatologists.

Small-fibre neuropathies (SFNs) can be defined as diseases of small nerve fibres. Because their symptoms are mainly located in the skin in the initial stages, dermatologists may frequently be confronted with these diseases. Moreover, skin biopsies and the subsequent measurement of intraepidermal nerve fibre density have become a widely accepted technique to investigate the structural integrity of small nerve fibres. The pathogenesis of injury to small nerve fibres is poorly understood. It probably depends on the cause. SCN9A-gene variants have been reported. Diabetes mellitus is one of the mai

Routine use of punch biopsy to diagnose small fiber neuropathy in fibromyalgia patients.

Fibromyalgia is a clinical syndrome that currently does not have any specific pathological finding to aid in diagnosis. Therefore, fibromyalgia is most likely a heterogeneous group of diseases with similar symptoms. Identifying and understanding the pathological basis of fibromyalgia will allow physicians to better categorize patients, increasing prospective treatment options, and improving potential therapeutic endeavors. Recent work has demonstrated that approximately 50% of patients diagnosed with fibromyalgia have damage to their small unmyelinated nerve fibers. A skin punch biopsy is a se

Small Fiber Neuropathy in Children: Two Case Reports Illustrating the Importance of Recognition.

Small fiber neuropathy (SFN) is a debilitating condition that often leads to pain and autonomic dysfunction. In the last few decades, SFN has been gaining more attention, particularly in adults. However, literature about SFN in children remains limited. The present article reports the cases of 2 adolescent girls diagnosed with SFN. The first patient (14 years of age) complained about painful itch and tingling in her legs, as well as dysautonomia symptoms for years. She also reported a red/purple-type discoloration of her legs aggravated by warmth and standing, compatible with erythromelalgia.

If your ears are burning we must be talking about red ear syndrome: A brief report.

Red Ear Syndrome is an uncommon disorder that can affect all age groups. It is frequently referred to Dermatology as it can present similarly to erythromelalgia. Although the exact pathophysiology is unknown, a common hypothesis suggests a shared pathophysiological background with migraine due to their well-known association. Currently, there are no established treatment guidelines. Delays in accurate diagnosis and commencing optimal treatment can significantly negatively impact on a patients quality of life. We discuss the clinical presentation and response to treatment of a case of Red Ear S

[Differential diagnosis of CRPS from a dermatologist's viewpoint].

Because of clinical similarities many dermatological diseases must be differentiated from both the acute and chronic phases of the complex regional pain syndrome (CRPS). The differential diagnosis of CRPS in the acute phase includes erythema migrans, early inflammatory lesions of acrodermatitis chronica atrophicans, hypodermitis in chronic venous insufficiency, eosinophilic fasciitis, erythromelalgia and an acute contact dermatitis. The chronic phase of CRPS with trophic disturbances of the skin, joints, and bones needs to be distinguished from the atrophic stage of acrodermatitis chronica atr

Botulinum Toxin Type A in Red Ear Syndrome and a Novel Multi-nerve Injection Paradigm: A Case Report.

Red ear syndrome (RES) is a rare, poorly understood disorder characterized by episodic ear erythema and pain. Its pathophysiology remains unclear, with proposed associations to migraine, erythromelalgia, and rosacea. Existing treatments are often ineffective, emphasizing the need for novel approaches. We describe a 33-year-old male with bilateral, non-headache RES refractory to standard therapies. A novel botulinum toxin type A (BoNT-A) injection paradigm, targeting both sensory and autonomic innervation, was implemented. Although episode frequency remained unchanged, the patient experienced s

Red scrotum syndrome: An update on clinicopathologic features, pathogenesis, diagnosis, and management.

The genital skin may be affected by a variety of dermatoses, be it inflammatory, infectious, malignant, idiopathic, or others. The red scrotum syndrome is characterized by persistent erythema of the scrotum associated with a burning sensation, hyperalgesia, and itching. Its cause is unknown, but proposed mechanisms include rebound vasodilation after prolonged topical corticosteroid use and localized erythromelalgia. The condition is chronic, and treatment is often difficult. Here we review the etiology, the physical and histopathologic findings, and the management of this condition. We also de

Invasive Spinal Neuromodulation for Chronic Pain in Pediatric Patients: A Scoping Review.

OBJECTIVE: Chronic pain affects up to 21% of pediatric patients and significantly impairs function and quality of life. Although spinal neuromodulation, including spinal cord stimulation (SCS) and dorsal root ganglion stimulation (DRG-S), has demonstrated efficacy in adults, evidence in children remains limited. This scoping review aims to systematically evaluate current evidence on spinal neuromodulation for pediatric chronic pain patients, focusing on clinical indications, outcomes, and safety. METHODS: A structured scoping review was conducted using PubMed/MEDLINE and Scopus databases on J

Extreme thrombocytosis and cardiovascular surgery: risks and management.

Extreme thrombocytosis is a major risk factor for excessive bleeding and for thrombosis, either of which can complicate cardiovascular surgical and interventional procedures. Extreme thrombocytosis can also cause an unusual syndrome, erythromelalgia, that results in a type of chronic microvascular occlusive arterial disease. We present the differential diagnosis of conditions that may lead to extreme thrombocytosis, 3 cases (each of which illustrates a different potential complication), and a review of the pertinent medical literature. Correcting excessive thrombocytosis is typically not diffi

[GABApentin--new therapeutic possibilities].

The paper is a review of current experience with use of gabapentin--a new antiepileptic drug--in neurologic conditions others than epilepsy. Mechanism of action of the drug is not fully elucidated yet. However it proved to be effective in therapy of chronic pain, especially in neuropathic pain, neuralgia, low back pain, reflex sympathetic dystrophy and erythromelalgia. Gabapentin is also effective in pain and spasticity in multiple sclerosis. Clinical studies of gabapentin in movement disorders, such as Parkinson disease, essential tremor and atrophic lateral sclerosis are discussed in the pap

Breast cancer presenting as paraneoplastic erythroderma: an extremely rare case.

The skin may exhibit the first clinical evidence of a systemic disease and may provide the first clues to a diagnosis in malignancies. Erythroderma is defined as generalized redness and scaling and it is a clinical manifestation of a variety of underlying diseases including, rarely, solid tumors. Breast cancer is associated with a variety of skin paraneoplastic manifestations like acanthosis nigricans, erythromelalgia, thrombotic thrombocytopenic purpura, acrokeratosis paraneoplastica, dermatomyositis, systemic sclerosis, and scleroderma. However, in the literature, the correlation of erythrod

Erythermalgia Involving the Face Alone: Two Case Reports.

Erythermalgia is a rare cutaneous disease characterized by episodic attacks of burning pain, erythema, and increased temperature. It primarily involves the extremities, with possible extensions to the ears, face, neck, and scrotum; in rare instances, it may afflict the ears, face, or the scrotum alone. Although various medications alone or in different combinations have been tried with significant variations in response, no recommended therapeutics have been established until very recently. This report presents two case histories of a 20-year-old and a 46-year-old woman, respectively, who disp

A patient with adult erythermalgia: evidence suggesting an autoimmune etiology.

We report the case of a woman with a combination of erythermalgia, idiopathic thrombocytopenic purpura, and vitamin B-12 deficiency with positive parietal cell antibodies. The patient was treated with intravenous administration of immunoglobulins together with small doses of prednisone, which resulted in an improvement in her platelet counts, rise in her vitamin B12 levels, and resolution of her painful discolored digits. These findings suggest an underlying autoimmune component to the development of erythermalgia.
