Potocki-Lupski syndrome: a microduplication syndrome associated with oropharyngeal dysphagia and failure to thrive.

OBJECTIVE: Failure to thrive (FTT) is a feature of children with Potocki-Lupski syndrome (PTLS) [duplication 17p11.2]. This study was designed to describe the growth characteristics of 24 subjects with PTLS from birth through age 5 years in conjunction with relevant physical features and swallow function studies. STUDY DESIGN: We evaluated 24 individuals with PTLS who were ascertained by chromosome analysis and/or array comparative genome hybridization. Clinical assessments included review of medical records, physical examination, otolaryngological examination, and swallow function studies. M…

Childhood autism, feeding problems and failure to thrive in early infancy. Seven case studies.

Despite longstanding clinical experience of unusual feeding difficulties in children with autism, there is no published literature describing their association with early onset FTT. This paper examines literature that may link feeding problems and abnormal growth with developmental and psychiatric conditions and describes seven cases of children with autism, who showed growth failure caused by severe feeding problems starting in the first year of life. Inadequacies in existing classifications systems are highlighted. The presence of severe or atypical feeding problems and FTT in infancy should

Non-organic failure to thrive complicated by benign intracranial hypertension during catch-up growth.

Severe non-organic failure to thrive associated with physical and emotional abuse including food deprivation was diagnosed in a 9-y-old boy. Rapid catch-up growth (weight and height) followed change of carer. Recovery of poor growth hormone response to clonidine stimulation was associated with benign intracranial hypertension accompanied by headaches and vomiting. Possible mechanisms are discussed.

Occult pulmonary artery associated with failure to thrive and recurrent pneumonia--a case report.

Occult pulmonary artery is an uncommon cardiovascular defect. Associated symptoms include recurrent pulmonary infections and congestive heart failure. The authors describe a one-year-old boy initially diagnosed as having broncho-pulmonary dysplasia who developed severe failure to thrive, recurrent pneumonias, and pulmonary hypertension. The presence of an occult right pulmonary artery was suspected by lung perfusion scan and diagnosed by cardiac catheterization and angiography. After surgical repair, his clinical course improved and his growth and development were normal. This case demonstrate…

Cricopharyngeal Achalasia Leading to Recurrent Pneumonia and Failure to Thrive in an Infant: A Case Report.

Cricopharyngeal achalasia (CPA) is a rare cause of feeding difficulties in children, potentially leading to failure to thrive (FTT) and recurrent pneumonia. It involves the partial or complete inability of the upper oesophageal sphincter to relax. We present the case of a 12-month-old male infant with FTT, recurrent pneumonia, chronic cough, watery diarrhoea, laryngopharyngeal reflux and vomiting. Following a series of clinical examinations and diagnostic procedures, CPA was diagnosed. Early and accurate diagnosis of CPA is critical to reduce the risk of progressive or irreversible lung damage…

Crowned Dens Syndrome Presenting as Failure to Thrive in a Patient With Moderate Cognitive Impairment.

Crowned dens syndrome (CDS) is a rare condition characterized by the deposition of calcium pyrophosphate (CPP) or hydroxyapatite crystals around the odontoid process of the C2 vertebra, often presenting with acute neck pain, reduced cervical mobility, and systemic inflammatory response. This case describes a 68-year-old male patient with a history of type 2 diabetes, hypertension, and benign prostatic hyperplasia who presented with generalized weakness and failure to thrive. This atypical presentation initially masked symptoms of CDS, delaying the diagnosis until the patient developed acute ne…

Distal Renal Tubular Acidosis: A Conundrum of Short Stature, Failure to Thrive, Rickets, and Nephrocalcinosis.

Distal renal tubular acidosis (dRTA) is an uncommon disorder marked by defective hydrogen ion excretion in the distal nephron, leading to hyperchloremic metabolic acidosis, persistently alkaline urine, and hypokalemia. Chronic, untreated dRTA in children often presents with growth failure, refractory rickets, and nephrocalcinosis, yet is frequently misdiagnosed. We report a 13-year-old girl with severe stunting, bone pain, proximal muscle weakness, and polyuria for several years. Examination revealed significant short stature, malnutrition, widened wrists, a protuberant abdomen, and genu valgu

Euglycemic diabetic ketoacidosis caused by empagliflozin complicated by failure to thrive in a geriatric patient.

Euglycemic diabetic ketoacidosis (euDKA) is a rare but deadly complication of sodium-glucose cotransport-2 (SGLT-2) inhibitors. Primarily indicated for the treatment of Type 2 Diabetes Mellitus, the incidence of euDKA is expected to rise as SGLT-2 inhibitors become a mainstay therapy for diabetics with heart failure. Diagnosis of euDKA can be difficult given the presence of normoglycemia and is especially challenging among geriatric patients that are complicated by additional comorbidities. We present a case of an elderly male with multiple comorbidities who presented for dehydration and alter

Oropharyngeal Teratoma: Five-Month-Old Presenting With Failure to Thrive and Severe Obstructive Sleep Apnea.

Oropharyngeal teratomas are an extremely rare congenital tumor. They are often diagnosed prenatally and can cause significant airway obstruction and feeding difficulties at birth. We present a five-month-old female that was diagnosed with a palatal teratoma that presented with failure to thrive, difficulty feeding, and eventually with severe obstructive sleep apnea. We present a five-month-old term, otherwise healthy female who became stridulous after an episode of the respiratory syncytial virus at one month old. At three months old, an otolaryngologist diagnosed mild laryngomalacia with no m…

Sanjad-Sakati syndrome with macrocytic anemia and failure to thrive: a case from South Jordan.

UNLABELLED: Backgorund: Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disease caused by a deletion mutation (155-166del) in exon 3 of the TBCE gene on chromosome 1q42-43. The syndrome is characterized by primary hypoparathyroidism, typical dysmorphic features and severe growth retardation. CASE PRESENTATION: We encountered a 2-year-old boy with hypocalcemia, failure to thrive and macrocytic anemia. The patient had the characteristic features of SSS and genetic testing confirmed that he was homozygous for the TBCE mutation. Although malabsorption was initially considered the cause…

Hidden in Plain Sight: Neglected Congenital Ranula Presenting as Failure to Thrive in an Infant.

Congenital ranula is a rare mucous extravasation cyst arising from the sublingual gland, presenting early in life and potentially causing feeding difficulties and growth failure. We report a two-month-old infant with a large intraoral swelling present since birth, leading to poor weight gain and tongue displacement. Ultrasound confirmed a plunging ranula, and marsupialization was successfully performed. Post-surgery, the infant showed significant improvement in feeding and growth. This case highlights the importance of early oral cavity examination in neonates, as timely identification and int

Vallecular Cyst: Reminder of a Rare Cause of Stridor and Failure to Thrive in Infants.

The vallecular cyst is a rare cause of stridor, respiratory distress, and failure to thrive in infants. Large vallecular cysts may present with serious complications such as life-threatening airway obstruction. This report is of an infant who presented with stridor and failure to thrive. The patient's condition was diagnosed as the presence of a vallecular cyst using flexible laryngoscopy. The vallecular cyst was successfully managed using endoscopic marsupialization. After the procedure, the patient was asymptomatic and dramatically gained weight within a few months. This case report serves a…
