Treating hyperuricemia of gout: safety and efficacy of febuxostat and allopurinol in older versus younger subjects.

Despite an increasing incidence of gout in older age patients with multiple metabolic and cardiovascular comorbidities, there are limited data addressing whether currently available urate-lowering therapy is comparably effective and safe in older (≥65 years of age) versus younger (<65 years of age) patients. In this secondary analysis of data from the CONFIRMS trial, we found that among 374 older subjects, urate-lowering therapy with approved doses of febuxostat or commonly prescribed doses of allopurinol was at least comparable to that in 1894 younger subjects and was well tolerated despite h…

Febuxostat: a non-purine, selective inhibitor of xanthine oxidase for the management of hyperuricaemia in patients with gout.

Febuxostat is a non-purine, selective inhibitor of xanthine oxidase being developed for the management of hyperuricaemia in patients with gout. With febuxostat 10-120 mg, the pharmacokinetics are linear. No dose adjustment appears to be necessary in those with renal insufficiency or mild-to-moderate hepatic impairment. Febuxostat 10-120 mg/day rapidly and sustainably reduces serum uric acid by 25-70% in uric acid underexcretors and overproducers. Prophylaxis with colchicine or a non-steroidal anti-inflammatory drug can mitigate the gout-flare risk from the rapid urate lowering after febuxostat…

Cost-effectiveness analysis of allopurinol versus febuxostat in chronic gout patients: a U.S. payer perspective.

BACKGROUND: Gout is a chronic inflammatory condition associated with poor urate metabolism. Xanthine oxidase inhibitors such as allopurinol and febuxostat are recommended to reduce uric acid levels and to prevent gout attacks in adult patients. Under budget-driven constraints, health care payers are faced with the broader challenge of assessing the economic value of these agents for formulary placement. However, the economic value of allopurinol versus febuxostat has not be assessed in patients with gout over a 5-year time period in the United States. OBJECTIVE: To evaluate the cost-effective…

Integrating crystal engineering with PEG6000 conjugation to formulate advanced solid dispersions of febuxostat for gout management.

OBJECTIVES: Febuxostat (FEB) is a poorly soluble xanthine oxidase inhibitor, which limits its in vitro dissolution performance. This study aimed to enhance the aqueous solubility and dissolution behavior of FEB through crystal engineering (salt and co-crystal formation) and the development of PEG6000-based amorphous solid dispersions. METHODS: Three FEB salts with p-toluenesulfonic acid (Tos1), tromethamine, and N-methyl-D-glucamine, as well as one FEB co-crystal with acridine (Acd1), were prepared together with their amorphous solid dispersions using PEG6000. Solubility, dissolution, and phy…

Cardiovascular Safety of Febuxostat vs. Allopurinol in Gout Patients with Cardiovascular Diseases: A Target Trial Emulation.

Guidelines recommend febuxostat and allopurinol as first-line urate-lowering therapies (ULTs) for gout, yet the cardiovascular safety of febuxostat remains debated, with conflicting randomized trials (CARES, FAST) results and limited real-world evidence in high-risk patients. This target trial emulation study evaluated the long-term cardiovascular safety of febuxostat vs. allopurinol in gout patients with preexisting cardiovascular disease (CVD) using US collaborative electronic health records. We included 7555 propensity-score matched (42 covariates) pairs of adults aged ≥ 50 years initiating…

Efficacy and Safety of Ultra-Low Starting Dose Febuxostat Titration in Male Patients With Primary Gout.

BACKGROUND: Since gout is a common metabolic arthritis caused by urate crystal deposition, urate-lowering therapy (ULT) is clearly indicated, but the initiation of ULT frequently causes paradoxical acute flares that in turn impair patient adherence. OBJECTIVE: This study sought to assess the effectiveness and safety of an ultra-low-dose initiation strategy for febuxostat (10 mg/day) compared to the standard starting dose (20 mg/day) in reducing initiation-related flares while maintaining long-term urate control. METHODS: 120 male patients with primary gout presenting with acute arthritis wer…

Comparative assessment of the effects of dotinurad and febuxostat on the renal function in chronic kidney disease patients with hyperuricemia.

Although hyperuricemia is associated with chronic kidney disease (CKD), the impact of uric acid (UA)-lowering drugs on CKD has been controversial. Previous investigations have primarily included xanthine oxidase inhibitors; therefore, research of dotinurad, a recently developed selective urate reabsorption inhibitor, is necessary. This retrospective study included 58 patients with CKD; of these, 29 newly initiated dotinurad and 29 initiated febuxostat. The effects of dotinurad and febuxostat on the serum UA, urinary UA-to-creatinine ratio (UUCR), and estimated glomerular filtration rate (eGFR)…

Rhabdomyolysis Associated With Febuxostat in a Non-Chronic Kidney Disease (CKD) Patient: Diagnostic and Management Challenges.

Rhabdomyolysis is a critical medical condition characterized by the rapid breakdown of muscle tissue, releasing substances such as myoglobin and creatine kinase into the bloodstream, potentially leading to acute kidney injury. The etiology includes trauma, exertion, genetic factors, infections, and adverse drug reactions. Febuxostat, a non-purine selective xanthine oxidase inhibitor used to manage hyperuricemia in gout patients, is typically well-tolerated but has been associated with rare instances of severe adverse reactions like rhabdomyolysis. This case report describes a case of a 73-year…
