The spectrum of disease was assessed in 43 patients with Felty's syndrome. Twenty-three underwent splenectomy. Although complete remission of neutropenia occurred in only two nonoperated patients, most of the patients had no serious infections during the subsequent period of observation. Operative morbidity was minimal in the splenectomized patients. A patient with splenomegaly, severe granulocytopenia and a strongly positive rheumatoid factor test initially had no clinical evidence of rheumatoid arthritis. Leukopenia responded to splenectomy and did not recur during one year of follow up. Symmetrical metacarpophalangeal joint swelling developed after nine months. This case emphasizes that arthritis may occasionally be a late and minor manifestation of Felty's syndrome.

Eight patients with Felty's syndrome who had associated fevers, cutaneous ulcers, or infections were treated with parenteral gold. A satisfactory response was noted in 7 of the 8 patients. Improvement in leukopenia (7/8), decrease in infections (5/8) and fevers (7/8), and healing of ulcers (4/5) were noted. We conclude that gold has a role in the treatment of this complication of rheumatoid arthritis.

A reaction of the RES with its organs (lymph nodes, spleen, bone marrow and Kupffer's cells) is not uncommon in rheumatoid arthritis. Although these manifestations are pronounced in Felty's syndrome, reviewing articles about Felty's do not mention liver manifestations. A case of Felty's syndrome with nodular regenerative hyperplasia of the liver, presinusoidal portal hypertension, and bleeding esophageal varices is reported. Increased splenic blood flow may be a contributing factor to the development of the regenerative nodules. The portal hypertension is postulated to be a result of intrahepatic vascular compression.
