A case of fracture-dislocations of the hip, knee and tarsometatarsal joints in the same limb is presented. This combination of injuries has not previously been reported. There were no associated neurovascular complications.

A patient with a fracture of an ankylosed thoracic spine was treated initially along conservative lines. Subsequently the fracture displaced, causing paraplegia. Internal fixation proved difficult but stability was obtained by wiring two Sherman plates to the posterior elements. This succeeded in holding the fracture until the death of the patient 13 days postoperatively. The difficulties in the management of these fractures are discussed.

Rockwood type VI acromioclavicular (AC) joint dislocations are extremely rare high-energy injuries characterized by inferior displacement of the distal clavicle and are often associated with complex shoulder girdle injury. We report the case of a 36-year-old male who sustained a Gustilo-Anderson type IIIA open Rockwood type VIb AC joint dislocation associated with a Neer two-part proximal humerus fracture-dislocation and an ipsilateral scapular spine fracture following a road traffic accident. The patient underwent early surgical debridement and single-stage operative management, including ope

Monteggia fractures represent complex injuries requiring careful assessment and surgical intervention. This case report presents a rare variation of a Bado type I Monteggia fracture-dislocation that resembles features from the Jupiter subclassification type IID. A 39-year-old male sustained a high-energy injury while riding an all-terrain vehicle, resulting in a proximal segmental ulnar shaft fracture with anterior radial head dislocation. Open reduction and internal fixation (ORIF) of the ulna using a pre-contoured proximal ulna low-contact dynamic compression plate (LC-DCP) successfully rest

Perilunate dislocations, dislocation fractures and lunate dislocations are rare injuries predominantly resulting from hyperextension of the wrist during high-energy trauma. Early recognition and treatment of these injuries usually results in good functional outcome despite degenerative changes on radiographs. Immediate reduction and adequate restoration of normal alignment are the key to successful healing. Surgical intervention with open reduction and ligament repair aims at stable reconstruction of the carpus. It requires broad surgical experience and a profound knowledge of normal anatomy.

Most fractures of the phalanges or metacarpals are amenable to closed treatment, with favorable outcomes. However, two groups of complex fractures are difficult to diagnose and treat. The first group includes unicondylar and bicondylar fractures, fracture-dislocations, and fracture-related instability of the proximal interphalangeal joint. Fracture-dislocations can be treated with splinting or surgical intervention. Microscrews and condylar plates have added considerably to the ability to securely fix small articular fractures and fracture-dislocations about the proximal interphalangeal joint.

A total of 717 fractures and fracture-dislocations of the cervical spine occurred in 657 patients in a 13-year period. Injury was commonest in the third decade of life. The levels most frequently injured were the second, fifth, and sixth. Injuries of the first and second cervical vertebrae commonly occurred together, and injuries involving the upper and lower cervical spine in the same individual occurred in 9 per cent of patients with fractures of C1 and C2. Odontoid fractures were the most frequent fractures in patients over 70 years of age, and formed the majority of fractures in patients o

Ankle fracture-dislocations may require delayed internal fixation. Our aim was to compare acute open reduction and internal fixation (ORIF) with delayed ORIF, using external fixation or cast splint in ankle fracture-dislocations. Factors that affect the rates of re-operation and Surgical site infection (SSI) were identified. In this retrospective cohort study, patients were included with open and closed ankle fracture-dislocations treated with ORIF from two large peripheral hospitals and one academic center in the Netherlands. This study included 447 patients with an ankle fracture-dislocation

The posterior luxation of the shoulder joint is a rarely reported and often not recognized lesion in the clinical workday. The authors present the first case of a posterior luxation of the shoulder joint in combination with an contralateral anterior shoulder joint fracture dislocation. In that case the authors stress the importance of an exact clinical and radiology diagnostic and the right way to reduce a posterior luxation of the shoulder joint. Finally they draw the readers attention to operative steps to prevent a reluxation of the shoulder joint.

In order to prevent permanent disability, a proper diagnosis of the fixed posterior fracture dislocation of the fibula with fracture dislocation of the ankle joint must be made. Full-length (knee to ankle) radiographs are necessary to make this diagnosis. Although a closed reduction has been described as successful, we feel that an open reduction is usually necessary. Freeing of the trapped fibula from behind the tibia results in a satisfactory reduction of this fracture dislocation. The reduction and fixation of the malleolar fragment is then simple, and an excellent result can be expected.

A combined volar-dorsal approach was used to treat 11 perilunate dislocations and fracture dislocations between 1989 and 1994. The mean average age of the patients was 38 years, and the mean average time between injury and surgery was 13 hours. Outcome was assessed after an average of 30 months. Results were based on measurements of grip strength, range of motion, radiographs, and patient satisfaction. Patient satisfaction was high in 9 of 11 patients. Seven had satisfactory pain relief, and 5 had returned to their previous occupation without limitation. The wrist flexion-extension arc and gri…

CASE: A 7-year-old girl sustained a radiographic appearance seemed harmless (TRASH) elbow lesion after falling from play equipment. Initial radiographs demonstrated a transolecranon fracture-dislocation. Additional injury was suspected, given valgus instability after reduction. Subsequent magnetic resonance imaging (MRI) revealed a large osteochondral coronoid fragment only partially visible on initial radiographs and computed tomography (CT). Successful outcomes were achieved by ulnar nerve decompression, open reduction, and fixation.
CONCLUSION: TRASH lesions, including this osteochondral c

RATIONALE: Lunate fractures are very rare, accounting for only 0.5% to 6.5% of all carpal fractures. They are mostly caused by high-energy trauma and commonly occur with fractures and dislocations of other carpal bones. It is rarely combined with Colles' fracture and the underlying pathogenesis and prognostic significance still remain controversial.
PATIENT CONCERNS: Here we report a case of a fresh lunate fracture combined with Colles' fracture, without dislocations of other carpal bones caused by a low-energy fall injury.
DIAGNOSES: Lunate fracture, Colles' fracture.
INTERVENTIONS: This p

Isolated sacral fractures are very rare in children. Two children, aged eight and 12 years, presented with an isolated sacral type III fracture and a fracture dislocation, respectively, both of which were not associated with any neurologic problems. Without any attempts for reduction, the fractures healed completely with conservative treatment. These cases demonstrated a high remodeling potential in children for spontaneous healing.

BACKGROUND: Multiple carpometacarpal fractures and dislocations are rare. This case report describes a novel multiple carpometacarpal injury, namely, 'diagonal' carpometacarpal joint fracture and dislocation.
CASE PRESENTATION: A 39-year-old male general worker sustained a compression injury to his right hand in the dorsiflexion position. Radiography indicated a Bennett fracture, hamate fracture, and fracture at the base of the second metacarpal. Subsequent computed tomography and intraoperative examination confirmed an injury to the first to fourth carpometacarpal joint along a diagonal line

PURPOSE OF REVIEW: Proximal humerus fracture dislocations typically result from high-energy mechanisms and carry specific risks, technical challenges, and management considerations. It is vital for treating surgeons to understand the various indications, procedures, and complications involved with their treatment.
RECENT FINDINGS: While these injuries are relatively rare in comparison with other categories of proximal humerus fractures, fracture dislocations of the proximal humerus require treating surgeons to consider patient age, activity level, injury pattern, and occasionally intra-operat…
