Globus Pallidus Necrosis Unrelated to Carbon Monoxide Poisoning: Retrospective Analysis of 27 Cases of Basal Ganglia Necrosis.

Bilateral globus pallidus necrosis is said to be characteristic of carbon monoxide (CO) poisoning. However, there has been no scientific test of this hypothesis. To examine the assertion that globus pallidus necrosis is typical of CO poisoning, this study examined autopsy cases from the King County Medical Examiner's Office (KCMEO) between 1994 and 2013. Twenty-seven cases with bilateral basal ganglia lesions were identified and examined for associated or causative disease or injury with the following results: 10 cases of drug overdose, seven heart disease, three asphyxia, two chronic ethanoli

Diagnosis of aluminum phosphide poisoning using a new analytical approach: forensic application to a lethal intoxication.

Aluminum phosphide (AlP) is an effective and cheap pesticide that is commonly used worldwide, but it is also a common cause of human poisoning and carries a high mortality rate. AlP reacts with moisture in air, water, and hydrochloric acid in the stomach to produce phosphine (PH3) gas. Two routes of exposure are ingestion of AlP and inhalation of phosphine generated by the action of moisture on AlP. Absorbed phosphine is rapidly metabolized into phosphite and hypophosphite. A method is described for the analysis of the phosphine metabolites in various biological matrices. The method involves r

Case Report: A case of occupational methyl iodide-induced encephalopathy.

Iodomethane is a commonly used methylation reagent in organic synthesis. However, reports of acute iodomethane poisoning are rare. Herein, we report the successful treatment of a patient with acute iodomethane poisoning. During processing and production, owing to inadequate personal protection, the patient's eyes were directly exposed to iodomethane gas, leading to blurred vision and magnetic resonance imaging findings consistent with toxic encephalopathy. Following a comprehensive diagnosis and treatment at our hospital, outpatient follow-ups and clinical assessments confirmed the patient's r

A systematic review of aluminium phosphide poisoning.

Every year, about 300,000 people die because of pesticide poisoning worldwide. The most common pesticide agents are organophosphates and phosphides, aluminium phosphide (AlP) in particular. AlP is known as a suicide poison that can easily be bought and has no effective antidote. Its toxicity results from the release of phosphine gas as the tablet gets into contact with moisture. Phosphine gas primarily affects the heart, lungs, gastrointestinal tract, and kidneys. Poisoning signs and symptoms include nausea, vomiting, restlessness, abdominal pain, palpitation, refractory shock, cardiac arrhyth

Development of sodium tetrathionate as a cyanide and methanethiol antidote.

CONTEXT: Hydrogen cyanide and methanethiol are two toxic gases that inhibit mitochondrial cytochrome c oxidase. Cyanide is generated in structural fires and methanethiol is released by decaying organic matter. Current treatments for cyanide exposure do not lend themselves to treatment in the field and no treatment exists for methanethiol poisoning. Sodium tetrathionate (tetrathionate), a product of thiosulfate oxidation, could potentially serve as a cyanide antidote, and, based on its chemical structure, we hypothesized it could react with methanethiol. RESULTS: We show that tetrathionate, un

Treatment of critical aluminum phosphide (rice tablet) poisoning with high-dose insulin: a case report.

BACKGROUND: Aluminum phosphide (rice tablet) is a highly efficient agent for preserving grains against rodents and insects. It accounts for a large number of poisoning cases. Aluminum phosphide poisoning has a high mortality rate of about 90%, and to date, no antidote is available. It releases phosphine gas after exposure to moisture, and this reaction is catalyzed by the acidity of the stomach. Phosphine is then absorbed throughout the respiratory or gastrointestinal tracts and causes toxicity through inhibition of cytochrome c oxidase and formation of highly reactive free radicals. Treatment

[Case report of butane intoxication with fatal outcome].

Forensic medical examiners know numerous cases of non-occupational intoxication (e.g. suicide, drug intoxication, household poisoning) but rarely encounter cases of occupational poisoning with household gases. Collection of forensic chemical facts testifying to the presence of saturated hydrocarbons in blood and other tissues is a challenging task. It hampers evaluation of their toxic effect on human brain for the purpose of forensic medical examination. A method is proposed for the analysis of biological materials allowing for the detection of natural alkanes, such as butane, along with recom

The therapeutic effect of a novel parenteral formulation of dihydroxyacetone in aluminum phosphide-intoxicated patients.

BACKGROUND AND OBJECTIVES: Aluminum phosphide (AlP), known as "rice tablet," is widely used as an effective pesticide. However, AlP poisoning is a common cause of mortality in many countries, such as Iran. Unfortunately, there is no specific antidote for AlP toxicity to date. AlP releases phosphine gas when it is exposed to moisture or acid. Phosphine is a potent mitochondrial toxin that could significantly inhibit cellular energy metabolism. AlP poisoning is an emergency condition that needs instant and effective intervention. Dihydroxyacetone (DHA) is a simple saccharide used for several pha

Accidental Aluminum Phosphide Poisoning of A Family and Role of Impella® Support: A Case Report.

BACKGROUND: Aluminum phosphides (AlPs) are highly effective fumigant pesticides that emit phosphine gas when exposed to moisture. Phosphine gas is highly toxic and exposure can lead to widespread multiorgan failure. Cardiovascular collapse can be rapid and many AlP poisonings are fatal despite aggressive resuscitation measures. CASE REPORT: We describe a series of ten cases of phosphine gas poisoning due to a single exposure. Five patients had minor symptoms, four died presumably due to rapid cardiovascular collapse, and one survived to hospital discharge after undergoing Impella® support and…

Management of aluminium phosphide poisoning.

This manuscript presents a clinical case of a patient poisoned by aluminium phosphide in its hydrolysed form, phosphine gas, following a suicide attempt, and its therapeutic management in the emergency room. The cardiovascular, metabolic and gasometric changes described in the pathophysiology of this poisoning were documented. There is no specific antidote, and treatment involves careful supportive care and reduction of the bioavailability of the phosphine in the poisoned patient. In this case, the patient was treated with vegetable oil, mannitol, bicarbonate and activated charcoal during the

[Poison content in the blood and tissues of mammals after the inhalational uptake of carbon monoxide and benzene and cyanide vapors].

The time of retention in the body of albino mice, hamsters, albino rats, guinea pigs, rabbits, cats and dogs of carbon oxide benzene and methylcyanide was analyzed. The amount of poison was determined by spectrophotometry and gas chromatography every 4 and 24 hours following inhalation. Conclusions are drawn about constancy of the poison half life for each animal species. Two phases of the poison elimination were determined: rapid (up to 5 hours) and slow (up to several days). The poison elimination rate directly correlated with the pulse rate, respiration and oxygen consumption per unit body

Smoke inhalation injury during enclosed-space fires: an update.

In view of the tragic fire at a nightclub in the city of Santa Maria, Brazil, which culminated in the sudden death of 232 young people, we decided to review the literature regarding smoke inhalation injury caused by enclosed-space fires, which can be divided into direct thermal damage, carbon monoxide poisoning, and cyanide poisoning. Such injuries often call for immediate orotracheal intubation, either due to acute airway obstruction or due to a reduced level of consciousness. The diagnosis and the severity of the thermal injury can be determined by fiberoptic bronchoscopy. The levels of gase

Development of detection methods for the diagnosis and analysis of highly toxic metal phosphides: A comprehensive and critical review.

Metal phosphides, especially aluminum phosphide, and phosphine (PH3 ) are widely used as insecticides and rodenticides for protection of grains during process of storage and transportation. The main reason of poisoning with this compound is related to the conscious ingestion of salts or accidental inhalation of PH3 . So the early and accurate diagnosis of poisoning can significantly help to the effective clinical treatment or recognition of death cause. PH3 is somewhat unstable due to reaction with oxygen or hemoglobin leading to formation of oxy-acids phosphorous. Here, we critically reviewed

Charge Confinement in an Ordered High-Entropy Intermetallic Breaks the Activity-Poisoning Trade‑Off for Glycolic Acid Electrosynthesis From Polyethylene Terephthalate.

Selective electrooxidation of ethylene glycol (EG) from polyethylene terephthalate (PET) hydrolysate to glycolic acid (GA) offers a sustainable route for plastic upcycling but is hindered by insufficient activity at industrially relevant current densities and severe catalyst deactivation caused by poisoning intermediates. Here, we design a two-dimensional ordered high-entropy intermetallic, HEI (PdPtRh)(InBi)ene, based on a Pd1In1-type body-centered-cubic framework. The ordered lattice induces pronounced p--d hybridization and charge redistribution, giving rise to a charge-confinement effect a

Color-reversible fluorescence tracking for the dynamic interaction of SO2 with Hg2+ in living cells.

Mercury ion (Hg2+) is one of the most threatening substances to human health, and the mercury poisoning can damage physiological homeostasis severely in human, even cause death. Intriguingly, Sulfur dioxide (SO2), a gas signal molecule in human, can specifically interact with Hg2+ for relieving mercury poisoning. However, the dynamic interaction of Hg2+ with SO2 at the tempospatial level and the correlation between Hg2+ and SO2 in the pathological process of mercury poisoning are still elusive. Herein, we rationally designed a reversible and dual color fluorescent probe (CCS) for dynamically v

Successful treatment of aluminium phosphide poisoning by dihydroxyacetone: A two-case report study.

WHAT IS KNOWN AND OBJECTIVE: Aluminium phosphide (AlP) is an agricultural fumigant which produces phosphine gas in the presence of moisture. Phosphine inhibits oxidative phosphorylation and causes cell death by inhibiting cytochrome C oxidase. Clinical manifestations of AlP poisoning are refractory hypotension, tachycardia, low oxygen saturation and severe metabolic acidosis. CASE SUMMARY: Two cases received dihydroxyacetone (DHA) in addition to routine management of AlP poisoning. Administration of DHA (7 gr in 50 mL sodium bicarbonate, gavage) 2 times at a 1-hour interval improved the clini
