Genetic Influence on LDL-Cholesterol Levels: Role of Polygenic Risk Scores and Lp(a) Beyond Monogenic Hypercholesterolemia.

High levels of low-density lipoprotein cholesterol (LDL-c) have been recognized as the main causal factor of atherosclerotic cardiovascular disease (ASCVD) and are influenced by both genetic and environmental factors. Among genetic determinants, Familial Hypercholesterolemia (FH) is the most common monogenic disorder, caused by rare high-impact variants in genes involved in LDL uptake. Other monogenic causes of hypercholesterolemia include sitosterolemia, cerebrotendinous xanthomatosis and lysosomal acid lipase deficiency (LALD). However, monogenic disorders only account for a small proportion

The predictive effect of family genetic risk scores as an indirect measure of causal effects of one disorder on another.

BACKGROUND: One potential cause of comorbidity is the direct causal effect of one disorder - A - on risk for subsequent onset of disorder B. Could genetic risk scores be utilized to test for such an effect? If disorder A causally impacts on risk for disorder B, then genetic risk for disorder A should be lower in cases of disorder A with v. without a prior onset of B. METHODS: In all individuals (n = 905 736) born in Sweden from 1980 to 1990, from six psychiatric and drug use disorders (major depression, anxiety disorders, alcohol use disorder, drug use disorder, bipolar disorder, and schizoph…

Genetics of cannabis use in opioid use disorder: A genome-wide association and polygenic risk score study.

BACKGROUND: Individuals with an Opioid Use Disorder (OUD) have increased rates of cannabis use in comparison to the general population. Research on the short- and long-term impacts of cannabis use in OUD patients has been inconclusive. A genetic component may contribute to cannabis cravings. AIMS: Identify genetic variants associated with cannabis use through Genome-wide Association Study (GWAS) methods and investigate a Polygenic Risk Score (PRS). In addition, we aim to identify any sex differences in effect size for genetic variants reaching or nearing genome-wide significance in the GWAS.

Genetic Landscape of Kidney Failure in a Korean Transplant Cohort: Genome-Wide Association and Multi-Polygenic Risk Score Analyses.

KEY POINTS: Genome-wide association study identified multiple genetic loci associated with kidney failure and its subtypes in Korean cohorts. Multiple polygenic risk scores improved the prediction of kidney failure beyond basic available clinical factors. Genetic architecture of kidney failure differed from that of CKD driven mainly by kidney function traits. BACKGROUND: Kidney failure represents the final, irreversible stage of CKD, yet its genetic architecture remains incompletely defined compared with CKD. Although previous studies largely focused on kidney function traits, the genetic det

Preimplantation genetic testing for type 2 diabetes based on family-specific polygenic risk score: A proof-of-concept study.

AIMS: This study aims to evaluate the feasibility of family-specific polygenic risk prediction in reducing the risk of type 2 diabetes (T2D) in the offspring from an infertile couple with a family history of early-onset T2D. METHODS: We innovatively established a family-specific polygenic risk prediction model for this T2D family and the embryo with the lowest risk of T2D were selected for implantation. RESULTS: Initially, whole exome sequencing analysis in the family failed to identify monogenic-level pathogenic or likely pathogenic variants responsible for T2D. Thus, preimplantation geneti

Single nucleotide polymorphism SULT2A1 rs4149448 and genetic risk score as biomarkers of frailty and progression of chronic kidney disease.

BACKGROUND: Frailty is a multifactorial condition highly prevalent in patients with chronic kidney disease (CKD), increasing with disease progression. Identifying genetic determinants associated with frailty may aid in risk stratification. We aimed to identify whether single nucleotide polymorphisms (SNPs), individually or combined into a genetic risk score (GRS), are associated with the frailty phenotype and adverse clinical outcomes in CKD patients. METHODS: This is a longitudinal study that evaluated patients with CKD 3b-5 non-dialysis dependent [69 (57-72) years; 59,1 % men; body mass ind…

Impact of Lipid Genetic Risk Score and Saturated Fatty Acid Intake on Central Obesity in an Asian Indian Population.

Abnormalities in lipid metabolism have been linked to the development of obesity. We used a nutrigenetic approach to establish a link between lipids and obesity in Asian Indians, who are known to have a high prevalence of central obesity and dyslipidaemia. A sample of 497 Asian Indian individuals (260 with type 2 diabetes and 237 with normal glucose tolerance) (mean age: 44 ± 10 years) were randomly chosen from the Chennai Urban Rural Epidemiological Study (CURES). Dietary intake was assessed using a previously validated questionnaire. A genetic risk score (GRS) was constructed based on choles…

A single nucleotide polymorphism genetic risk score to aid diagnosis of coeliac disease: a pilot study in clinical care.

BACKGROUND: Single nucleotide polymorphism-based genetic risk scores (GRS) model genetic risk as a continuum and can discriminate coeliac disease but have not been validated in clinic. Human leukocyte antigen (HLA) DQ gene testing is available in clinic but does not include non-HLA attributed risk and is limited by discrete risk stratification. AIMS: To accurately characterise both HLA and non-HLA coeliac disease genetic risk as a single nucleotide polymorphism-based GRS and evaluate diagnostic utility. METHODS: We developed a 42 single nucleotide polymorphism coeliac disease GRS from a Euro…

Machine learning methodology using a masked neural network for robust genetic risk score calculation from noisy and missing data.

PURPOSE: Genetic risk scores (GRSs) are summaries of genetic data that can improve prediction of disease risk and progression. GRSs are increasing available but rely on high-quality input data to produce good output results; with noisy or missing inputs the GRS may be inaccurate. We aimed to develop a method to produce a robust estimate of the GRS when input data are missing, noisy or both. APPROACH: We developed a neural network approach, named masked-multi-layer perceptron (MLP), for robust GRS calculation trained on a set of GRS scores calculated on clean data. The masked-MLP includes addi…

Glucose-Related Genetic Risk Score and Its Association With Cerebrovascular Abnormality, White-Matter Hyperintensity Volume, and Brain Microstructure Markers.

AIM: Cerebral white-matter lesions serve as biomarkers for stroke, cardiovascular disease, and mortality. However, no genetic association studies on white-matter hyperintensities (WMH) have focused specifically on the Han Chinese population. Despite previous studies on candidate genes, none have explored association with glucose regulation. This study aimed to investigate the association between a genetic risk score (GRS) derived from glucose-related loci and cerebrovascular abnormality, cerebral WMH, and brain microstructure markers. METHODS: This study is a community-based family research c…

Genetic risk score for Alzheimer's disease predicts brain volume differences in mid and late life in UK biobank participants.

INTRODUCTION: We estimated the ages when associations between Alzheimer's disease (AD) genes and brain volumes begin among middle-aged and older adults. METHODS: Among 45,616 dementia-free participants aged 45-80, linear regressions tested whether genetic risk score for AD (AD-GRS) had age-dependent associations with 38 regional brain magnetic resonance imaging volumes. Models were adjusted for sex, assessment center, genetic ancestry, and intracranial volume. RESULTS: AD-GRS modified the estimated effect of age (per decade) on the amygdala (-0.41 mm3 [-0.42, -0.40]); hippocampus (-0.45 mm3 …

Digital Clock Drawing as an Alzheimer's Disease Susceptibility Biomarker: Associations with Genetic Risk Score and APOE in Older Adults.

BACKGROUND: Alzheimer's disease (AD) is the leading cause of dementia in older adults, but most people are not diagnosed until significant neuronal loss has likely occurred along with a decline in cognition. Non-invasive and cost-effective digital biomarkers for AD have the potential to improve early detection. OBJECTIVE: We examined the validity of DCTclockTM (a digitized clock drawing task) as an AD susceptibility biomarker. DESIGN: We used two primary independent variables, Apolipoprotein E (APOE) ε4 allele carrier status and polygenic risk score (PRS). We examined APOE and PRS associatio

Polygenic Risk Score-Based Association Analysis Identifies Genetic Comorbidities Associated with Age-Related Hearing Difficulty in Two Independent Samples.

PURPOSE: Age-related hearing loss is the most common form of permanent hearing loss that is associated with various health traits, including Alzheimer's disease, cognitive decline, and depression. The present study aims to identify genetic comorbidities of age-related hearing loss. Past genome-wide association studies identified multiple genomic loci involved in common adult-onset health traits. Polygenic risk scores (PRS) could summarize the polygenic inheritance and quantify the genetic susceptibility of complex traits independent of trait expression. The present study conducted a PRS-based

Evaluating Individual Level Performance of Polygenic Risk Scores Using Early Onset High Genetic Risk Coronary Artery Disease as a Benchmark.

Polygenic risk scores (PRSs) are typically validated using population-level metrics, masking variability in individual-level risk prediction and hindering clinical translation. To address this, we introduced a novel framework using a "benchmark" cohort (N=1184) of "unexpected coronary artery disease (CAD)": early-onset patients (<55 years) with a clinical profile-low 10-year risk, no diabetes or severe hypercholesterolemia-that excludes therapy indications. The occurrence of early CAD in these clinically low-risk individuals establishes a "ground truth" for high genetic risk. We evaluated 58 p

Genotype Combinations and Genetic Risk Score Analyses of MTHFR, MTRR, and MTR Polymorphisms in Hypothyroidism Susceptibility: A Case-Control Study.

Hypothyroidism is a multifactorial endocrine disorder where genetic predisposition plays a significant role. The MTHFR, MTRR, and MTR genes influence thyroid hormone regulation via homocysteine remethylation and DNA methylation. This study examined associations between hypothyroidism and polymorphisms in MTHFR (C677T-rs1801133, A1298C-rs1801131), MTRR (A66G-rs1801394), and MTR (A2756G-rs1805087) genes. Eighty-six patients with hypothyroidism and 87 healthy controls were included. Genotyping was performed using PCR-RFLP. Post hoc analysis confirmed adequate statistical power (95% for MTRR A66G,

A Module-Level Polygenic Risk Score-Based NetWAS Framework for Identifying AD Genetic Modules Mediated by Amygdala: An ADNI Study.

Network-based GWAS (NetWAS) has advanced brain imaging research by identifying genetic modules associated with brain alterations. However, how imaging risk genes exert functions in brain diseases, particularly their mediation through imaging quantitative traits (iQTs), remains underexplored. We propose a module-level polygenic risk score (MPRS)-based NetWAS framework to uncover genetic modules associated with Alzheimer's disease (AD) through the mediation of an iQT, using amygdala density as a case study. Our framework integrates genotype data, brain imaging phenotypes, clinical diagnosis of A

Constructing a multi-ancestry polygenic risk score for uterine fibroids using publicly available data highlights need for inclusive genetic research.

Uterine leiomyomata, or fibroids, are common gynecological tumors causing pelvic and menstrual symptoms that can negatively affect quality of life and child-bearing desires. As fibroids grow, symptoms can intensify and lead to invasive treatments that are less likely to preserve fertility. Identifying individuals at highest risk for fibroids can aid in access to earlier diagnoses. Polygenic risk scores (PRS) quantify genetic risk to identify those at highest risk for disease. Utilizing the PRS software PRS-CSx and publicly available genome-wide association study (GWAS) summary statistics from
