Diagnosis and treatment difficulties in 18-year-old male patient with hereditary hemochromatosis, chronic hepatitis B, Gilbert syndrome and ulcerative colitis.

Among possible causes of chronic hepatitis in adolescents most common are infections, autoimmune disorders and metabolic diseases. Thus, diagnostic procedures should be multidirectional. This study reports diagnosis and treatment difficulties in an 18-year-old male patient with hereditary hemochromatosis (HH), ulcerative colitis (UC), chronic hepatitis B (CHB) and Gilbert syndrome. The presented case illustrates problems in diagnostics related to the presence of numerous disease conditions in one patient. It should be taken into consideration that these diseases coexisting in one patient can m

A puzzling jaundice.

In an adult beta-thalassaemic patient admitted on account of the sudden onset of nausea, vomiting and biliary pain, the concomitant jaundice was considered "obstructive" on the basis of ultrasonographic evidence of cholecysto-choledocal stones. However, when the patient was re-evaluated it was found that concomitant Gilbert's disease, hepatitis C virus related cirrhosis and dilated cardiomyopathy had a detrimental effect on clinical and laboratory findings.

[Investigation of redox homeostasis of liver in experimental and human studies].

Great importance has been attributed to antioxidants in the treatment of conditions associated with oxidative stress for many years. At the same time the antioxidants can exert prooxidant activity. Combined antioxidant treatment is more favourable compared with monotherapy, because antioxidants have scavenger-, compartment- and tissue-specificity and they regenerate each other directly, too. Drugs of chronic liver diseases should be considered because of the role of liver in biotransformation. Our aim was to study the redox status of liver and investigate the effects of natural antioxidants (v

[Microcirculation disturbances in liver diseases (author's transl)].

Microcirculation was studied in liver biopsies of patients with viral hepatitis, Gilbert's syndrome, liver steatosis, chronic hepatitis, liver cirrhosis, obstructive jaundice and venous congestion. All these diseases were shown to be usually accompanied by morphologic changes of sinusoid, Disse's and intercellular spaces, cellular membranes of hepatocytes. Those changes causing microcirculation disturbances result in the functional insufficiency of the liver.

[Juvenile hyperbilirubinaemia and its early manifestation in adolescence].

Mild isolated unconjugated hyperbilirubinaemia is incidentally or purposefully attained finding in 4-8% of population. Adolescence is a typical age for detection of hypebilirubinaemia. In these patients a mutation in gene-promoter for uridin-diphospho-glucuronyl-transpherase A 1, which determines development of the benign Gilbert's syndrome, is present in most of the cases. Although homozygote formation of this mutation was described in 11-16% of general population, only in a part of them hyperbilirubinaemia is manifested. Beginning or continuation of hyperbilirubinaemia in adolescent age is l

Molecular pathology of Crigler-Najjar type I and II and Gilbert's syndromes.

BACKGROUND AND OBJECTIVE: Crigler-Najjar syndromes type I and II and Gilbert's syndrome are familial unconjugated hyperbilirubinemias caused by genetic lesions involving a single complex locus encoding for bilirubin-UDP-glucuronosyltransferase which is involved in the detoxification of bilirubin by conjugation with glucuronic acid. Over the last few years a number of different mutations affecting this gene have been characterized. The aim of this work is to review the molecular pathology of Crigler-Najjar and Gilbert's syndromes, to discuss its impact on the clinical and genetic classification

Diagnostic criteria and contributors to Gilbert's syndrome.

Hyperbilirubinemia is a well-known condition in the clinical setting; however, the causes of elevated serum bilirubin are diverse, as are the clinical ramifications of this condition. For example, diagnoses of individuals vary depending on whether they exhibit an unconjugated or conjugated hyperbilirubinemia. Diagnoses can include conditions of disordered bilirubin metabolism (Gilbert's, Crigler-Najjar, Rotor, or Dubin-Johnson syndromes) or an acquired disease, including alcoholic/non-alcoholic fatty liver disease, hepatotropic hepatitis, cirrhosis, or hepato-biliary malignancy. Assessment of

Subfractionation of serum bilirubins by alkaline methanolysis and thin-layer chromatography. An aid in the differential diagnosis of icteric diseases.

The determination of direct and indirect-reacting bilirubin fractions by diazo procedures does not allow a definite diagnosis of icteric diseases. Therefore, the clinical relevance of serum bilirubin subfractionation by alkaline methanolysis and subsequent thin-layer chromatography (AM-TLC) was evaluated. Esterified bilirubins could be detected and quantitated in all serum samples investigated. The ratio of serum esterified to total bilirubin was 10-28% in 60 healthy adults (mean 17 +/- 5% S.D.), 1-11% in 77 patients with Gilbert's syndrome (mean 6 +/- 2%), and 2 and 3%, respectively, in two p

New insights into the classification and mechanisms of hereditary, chronic, non-haemolytic hyperbilirubinaemias.

Gilbert's syndrome is typically associated with a deficiency in hepatic bilirubin UDP-glucuronosyltransferase activity (B-GTA). The overproduction of bilirubin that is often found in this condition could be a fortuitous coincidence that leads to the unmasking of the disease, which otherwise often remains latent. Some cases of chronic unconjugated hyperbilirubinaemia could, however, be related to a defect in hepatic uptake, as reflected by alterations in BSP kinetics. Severe deficiencies of hepatic B-GTA exist in all types of Crigler-Najjar disease. An increased proportion of bilirubin monogluc

Partitioning reference intervals by use of genetic information.

BACKGROUND: Reference intervals that incorporate genetic information could reduce the misidentification of unusual test results caused by non-disease-associated genetic variation and increase the detection of results indicating underlying pathology. Subdividing reference groups by genetic effects, however, may lead to increased uncertainty around reference interval endpoints (because of the smaller subgroup sample sizes), thus offsetting any benefits. METHODS: We evaluated CLSI guidelines to develop a method appropriate for partitioning reference intervals on the basis of genetic variants wit

Other genetic liver diseases in children.

Wilson disease is rare but proteiform, and should be suspected in any child with liver disease and older than 3 years of age. The treatment … ic fibrosis, with risks of portal hypertension and cholangitis, or a more variable biliary disease. Gilbert disease is frequent and benign. Crigler-Najjar syndrome is rare, severe, and may be an indication for liver or liver-cell transplantation.

[Study of liver glucuronidase function in indirect hyperbilirubinemia and liver cirrhosis].

A discussion is made of the basic peculiarties and differences in the clinico-laboratory profile in various forms of pre- and postmicrosome jaundice. The importance of direct to total bilirubin ratio, and of quantitative bilirubin and urobilinogen bodies' determination in the urine is stressed. Bile pigments conjugation by the hepatic cells and free bilirubin conversion to glucuronose may be also assayed by resorting to some additional tests. The test with N-acetyl-paraaminophenol (NAPA) provides for an indirect assessment of the liver's glucuronidase function and has a good informative value.

"Gilbert's-like" syndrome as part of a spectrum of persistent unconjugated hyperbilirubinemia in post-chronic hepatitis patients.

Gilbert's syndrome (GS) patients present with remittent unconjugated hyperbilirubinemia. In this study, we investigated the correlation between polymorphisms in the gene encoding UDP-glucuronosyltransferase, UGT1A1, and the development of unconjugated hyperbilirubinemia in clinical GS and post-hepatitis hyperbilirubinemia. Blood samples were collected from 285 patients, including 85 patients who were clinically diagnosed with GS, 70 patients who had indirect hyperbilirubinemia during the recovery period of chronic liver diseases, 109 patients with normal hepatic function and 21 chronic active

Tolvaptan-induced isolated elevation of bilirubin in a patient with Gilbert syndrome.

Tolvaptan is the current standard of treatment for autosomal dominant polycystic kidney disease. It operates by acting on V2 receptors and blocks vasopressin interactions, causing a reduction in the rate of renal cyst growth and preserving kidney function. The current known risks of tolvaptan involve a serious liver injury characterized by an elevation in total bilirubin and alanine transaminase and aspartate transaminase levels. In this report, we document a unique liver injury characterized by an elevated bilirubin with normal alanine transaminase and aspartate transaminase levels in a patie

Coincidence of schizophrenia and hyperbilirubinemia.

In a retrospective study, the bilirubin plasma concentrations of every patient admitted to the Psychiatric Hospital were collected. Patients suffering from liver disease, substance abuse, overt hemolysis, or increased liver enzymes were excluded. Schizophrenics showed a significantly higher incidence of hyperbilirubinemia than patients suffering from other psychiatric disorders. This phenomenon seems to be independent of drug usage and therefore points to a relationship between hyperbilirubinemia and schizophrenic psychosis. Hypothetic explanations, such as a possible genetic disposition for G
