Freud and the diagnosis of Gilles de la Tourette's illness.

A number of observers recently have taken Freud to task for failing to have diagnosed both Frau Emmy von N.'s (1888-9) involuntary ticcing and vocalizations and Daniel Paul Schreber's (1911) coprolalia and convulsive tics as Tourette's syndrome. None of Freud's critics, however, has placed Freud's understanding of motor and vocal tics in historical context. None seems aware of the contests over the classification of tic symptoms in the 1880's and 1890's, nor do they appreciate the extent to which Charcot and Gilles de la Tourette had conceded that motor and vocal tics, as well as coprolalia, c

[Psychopathological study on Gilles de la Tourette's Syndrome].

Ten cases of Gilles de la Tourette's Syndrome (GTS) are reported. Average age of onset of the syndrome was 6.3 years old. Many of them were eldest children. 40% of them were caused by hereditary factor, and 40% of them had some abnormalities at their birth, for verbal tics including coprolalia. The GTS were classified into two types, Type I and Type II, from psychopathological point of view. Type I is "sadistic self-conversion type" and Type II is "masochistic personality type." Type I have an experience of attacks from others first, and, then, the desire of sadism aroused by the others conver

[Neuroimaging techniques in the Gilles de la Tourette syndrome].

Tourette's syndrome (GdT) is a chronic neuropsychiatric disorder of unclear etiology characterized clinically by the presence of motor and phonic tics as well as a variety of behavioral and cognitive abnormalities such as Obsessive-Compulsive Disorder (TOC) and Attention Deficit Hyperactivity Disorder (ADHD). Dopaminergic pathway and movement control related structures may have some implications for pathogenesis. Neuroimaging techniques may yield relevant data for a better understanding of the symptoms and physiopathology of this disease. Among them, functional isotopic techniques such as PET

Gilles de la Tourette's syndrome: a case study.

Tourette's Syndrome was first described by Gilles de la Tourette in 1885. Previously thought to be a rare neuropsychiatric disorder, its prevalence is now estimated at 0.05%. In Canada, this would mean approximately 12,000 people are affected. The rapid growth of knowledge in the neurosciences has led to better understanding and treatment of this disorder. Tourette's Syndrome is characterized by motor tics, usually beginning in the face and progressing to other areas, and by vocal tics, such as hissing, snorting, and barking. Fifty percent also develop copralalia. The age of onset is between 2

[Differential therapy of Gilles de la Tourette's syndrome].

The previous works of the author demonstrated the clinical heterogeneity of Gilles de la Tourette's syndrome (GTS). Based on statistical treatment of the efficacy of 138 courses of the treatment with benzodiazepine tranquilizers, neuroleptics, antidepressants, and anticonvulsants, methods of the individualized treatment of GTS were elaborated. It is recommended that low doses of neuroleptics (orap, haloperidol) be applied in patients with typical GTS. Schizophrenia associated with Tourette-like disorders requires the use of higher doses of neuroleptics and, in some cases, of the combined thera

[Multiple tics and Gilles de la Tourette syndrome].

Tic disorders are frequent in childhood, varying from transient motor tics over multiple chronic tic disorders to full-blown Gilles de la Tourette syndrome (GTS). GTS is genetically determined. In affected families, relatives of the patient(s) with GTS may show minor symptoms only. The clinical picture is extremely variable, extending from the well known motor tics over complex compulsive obsessive behavior to purely sensory tics. The optimal treatment today is the use of dopamine antagonists.

The Gilles de la Tourette syndrome and obsessional disorder.

Georges Gilles de la Tourette first drew attention to the psychopathology of the Gilles de la Tourette Syndrome (GTS) in 1889 when he commented on the anxieties and phobias of his patients. In this paper he acknowledged the contribution of Guinon, who in 1886 had noted that "tiquers" nearly always had associated psychiatric disorders characterised by multiple phobias, arithmomania and agoraphobia. Since that time many types of psychopathology have been documented in association with GTS, including depression, anxiety, phobic disorder, hostility and aggression. However, the exact association be

[Family coping of Gilles de la Tourette syndrome].

A survey over the disorder of chronic multiple tics (Tourette-Syndrom) is given. Two case reports reflect the effects of the syndrome on the family and other relational systems. Specific relations between the unusual and irritating symptomatology and the reaction of the family members are emphasized. In conclusion we offer some proposal for psychotherapeutic interventions, to help the relatives to cope better with the burden of the disorder.

Stress management and Gilles de la Tourette's syndrome.

Tourette's syndrome is a lifelong disorder characterized by multiple motor and verbal tics. The present study examined relaxation training and desensitization training as a method of reducing the frequency and intensity of tics and the distress they caused in a young adult diagnosed with Tourette's syndrome. After a period of symptom monitoring the subject underwent 3 weeks of intensive training in relaxation skills and 5 weeks of desensitization training with situational cues previously identified as eliciting Tourette's symptoms. According to self-report monitoring, the experience of symptom

Biochemical pharmacology of Gilles de la Tourette's syndrome.

A comprehensive brief review of Tourette's syndrome has been presented. This included description of various symptoms display, neuropathological findings, biochemical and pharmacological studies. An outline of pharmacotherapy and pharmacological properties of agents used in management of Tourette's syndrome has been made. They were classified according to their effect in modulating biogenic amine systems. Drugs producing exacerbation of Tourette's syndrome were also surveyed. Biochemical and pharmacological hypotheses relevant to the underlying pathology of the disease have been addressed and

Psychological interventions in Gilles de la Tourette's syndrome.

Even with the advent of successful pharmacotherapy of TS, emotional sequelae of chronicity, the public and intrusive nature of the disorder, and the potential side effects of the medications challenge the family's and the patient's capacity to successfully adjust to the disorder. Psychological intervention provides the physician with a comprehensive assessment of related problems that can affect family dynamics and social and academic adjustment, thus enhancing the ability to treat the patient successfully. Behavioral intervention may improve the patient's capacity to alter or inhibit a tic, w

Oculomotor control in Gilles de la Tourette syndrome.

Saccadic eye movements, fixation and smooth pursuit were studied in 28 children with Gilles de la Tourette syndrome and found to be normal. A link has been postulated between Gilles de la Tourette syndrome and other movement disorders. The results obtained in the present series do not support this hypothesis.

[The treatment of children with Gilles de la Tourette's syndrome].

Tourette syndrome is a neuropsychiatric disorder of motor and vocal tics, with associated symptoms of obsessive-compulsive behaviors and attention deficit disorder with hyperactivity. Genetic factors seem to play a major role, but the precise mode of inheritance is still not known. Intervention includes reassurance and support of child and family, medication if needed and guidance and advocacy in relation to school achievements. The major goal of treatment is to help the child succeed in moving along the various lines of development. Clonidine, a centrally active alpha2-noradrenergic agonist i

Obsessions and compulsions in Gilles de la Tourette's syndrome.

The incidence of obsessive-compulsive disorder (OCD) in patients with Gilles de la Tourette's syndrome (GTS) was assessed with a specially designed questionnaire. The Inventory was administered to patients with OCD, patients with GTS, and normal controls. Fifty-one percent of the patients with GTS had significantly elevated Inventory scores. The frequent occurrence of OCD in GTS suggests that the two disorders may share common neurobiologic mechanisms.

Psychological aspects of Gilles de la Tourette syndrome.

The unmistakable symptoms of Gilles de la Tourette Syndrome are classically of muscular spasms or tics, often accompanied by uncontrolled verbal outbursts or shouting of obscenities. Anecdotal as well as clinical reports suggest that these patients also suffer some psychological distress. This study used traditional MMPI scales to evaluate the psychopathological features that may underlie or accompany this disorder. In addition, we analyzed individual items of the MMPI to learn more of the phenomenology of this disorder. Data were collected from 29 Tourette patients and 29 normal controls matc

Remarks on the etiology of Gilles de la Tourette's syndrome.

Utilizing a case of Tourette's syndrome as an illustration, this paper seeks to refine the problem of causality in medical illness. In contrast to numerous examples reported in the literature, the patient presented in this communication, a 26-year-old man afflicted with tics, convulsive movements, intractable vomiting, and coprolalia, failed to respond favorably to medication, although he showed marked improvement in the course of psychotherapy. Despite rather transparent psychodynamic connections between his life history on the one hand and the nature and vicissitudes of his symptomatology on
