PURPOSE: Although olecranon osteotomies are helpful for distal humerus visualization, traditional methods of fixation are commonly irritating for patients and require hardware removal. Recent studies have shown lower hardware removal rates for medullary screw constructs and 3.5-mm plates, but no studies have investigated the use of 2.7-mm plates for olecranon osteotomy fixation. The purpose of this study is to report on the outcomes of single 2.7-mm mini-fragment plate fixation of olecranon osteotomies for distal humerus intra-articular fractures.
METHODS: Patients who sustained an intra-arti…

BACKGROUND: Controversy persists as to whether Lisfranc injuries are best treated with open reduction internal fixation (ORIF) versus primary arthrodesis (PA). Reoperation rates certainly influence this debate, but prior studies are often confounded by inclusion of hardware removal as a complication rather than as a planned, staged procedure inherent to ORIF. The primary aim of this study was to evaluate whether reoperation rates, excluding planned hardware removal, differ between ORIF and PA. A secondary aim was to evaluate patient risk factors associated with reoperation after operative trea…

OBJECTIVE: Medial epicondyle fractures are a common pediatric injury. When operative, cannulated partially threaded screws, with or without … ting for age and ulnar nerve status, when no washer was used, the hazard ratio (HR) for symptomatic hardware removal for 4.5 mm screws was 2.92 times the HR for 4.0 mm screws (95% CI: 1.35-6.29). When a 4.0 mm screw was used, the HR for symptomatic hardware removal for a washer was 3.24 times the HR without a washer (95% CI: 1.53-6.84).
CONCLUSION: Implant remova …

We present the case of a delayed pseudoarthrosis resulting from a fracture at the site of a radiographically confirmed anterior cervical fusion following plate removal. In this case, an anterior cervical plate was removed to allow for further surgery at a supra-adjacent level. A modicum of literature exists describing delayed fractures following hardware removal in thoracolumbar fusion constructs. The development of a fracture/pseudoarthrosis following hardware removal at the site of a radiographically confirmed anterior cervical fusion has not been previously reported. We describe the clinica…

Osteoporotic patients being treated with bisphosphonates present an interesting dilemma when removing hardware such as dynamic hip screws "DHS". In this paper, we describe the case of a 66-year-old osteoporotic patient who was placed on long term bisphosphonate therapy after sustaining an intertrochanteric hip fracture which was stabilized with a DHS. She presented with a subtrochantric fracture on the ipsilateral side. She was planned for DHS removal and intramedullary nailing. Removal of the dynamic hip screw proved to be difficult, likely due to possible cold welding of the DHS to the barre

Fractured instrument retrieval remains one of the most challenging aspects of endodontics, with reported incidence rates ranging from 0.3% to 12%. This complication is particularly common with nickel‒titanium (NiTi) rotary files used in curved canals. Instrument separation hinders effective canal disinfection, compromises the quality of obturation, and may necessitate additional interventions such as bypassing, retrieval, or surgical management. Conventional retrieval techniques often lead to dentin loss, compromising root structure. The use of dental operating microscopes and their integratio

CASE: A 28-year-old man with a distal radius fracture and small volar marginal fragment (VMF) underwent fixation using a "through-plate K-wire" technique. Two 1.2 mm K-wires were drilled through distal plate holes into the VMF and then bent over the plate to create a fixed-angle construct. Radiographic union was achieved by 6 weeks. Following elective hardware removal at 6 months, the patient demonstrated excellent clinical outcomes (DASH 4.2) and full range of motion at 1-year follow-up without complications.
CONCLUSION: Through-plate K-wire fixation is a simple, cost-effective bailout optio…

CASE: Hardware complications in the perioperative period of a pilon fracture open reduction internal fixation or fusion procedure present a unique challenge. Revision of hardware can lead to additional complications. In this case report, we share a technique of in situ plate bending to manage damaged hardware in a patient who underwent a primary tibiotalar arthrodesis for a pilon fracture.
CONCLUSION: Preoperative planning, protection of the soft tissue envelope, and use of the "F" tool are keys to the success of this treatment method. Proper technique allows for a noninvasive method for corr

This is a review of 183 patients with 218 displaced intraarticular fractures of the calcaneus treated by open reduction and internal fixation. One hundred and ninety four (89%) wounds underwent primary uneventful healing. Twenty-four wounds (11%) required local wound care. One deep infection occurred in a neuropathic foot that required below-knee amputation. No free-tissue transfers, local tissue flaps or skin grafts were needed in patients who presented initially with a closed fracture. Ninety-five fractures (43.5%) required subsequent surgical procedures (hardware removal-88/95 [93% of secon…

The purpose of this study was to determine patient-reported outcomes, hardware removal rates, as well as conversion to arthroplasty at mid-term follow-up following high tibial osteotomy (HTO) with a modern polyetheretherketone (PEEK) system. All PEEK system-based HTOs performed at a single institution between January 2010 and January 2016 with a minimum of 2 years follow-up were reviewed. Rates of postoperative complications, hardware removal and conversion to arthroplasty were calculated. Patient-reported outcomes including visual analog pain scale (VAS), Tegner activity scale, Lysholm, Knee …

Wound dehiscence and hardware exposure are significant complications following open reduction and internal fixation (ORIF) of low energy distal tibia and ankle fractures. This study aims to evaluate the long-term outcomes after removal of the exposed implants, application of an Ilizarov framework when needed and flap reconstruction. A prospective case series study was conducted, including eight patients with wound breakdown and exposed hardware following ORIF for closed distal tibia and ankle fractures since January 2022. Data collection consisted of patients' demographics, characteristics of

The use of a washer to supplement screw fixation can prevent fragmentation and penetration during the surgical treatment of pediatric medial epicondyle fractures. However, concerns may arise regarding screw prominence and the need for subsequent implant removal. The purpose of this study is to evaluate the impact of washer utilization on the need for hardware removal and elbow range of motion (ROM). All pediatric medial epicondyle fractures treated with a single screw over a 7-year period were queried for this retrospective case-control study. Hardware removal was performed only if the patient…

PURPOSE: The primary purpose of this study was to describe the rate of volar locking plate (VLP) removal after distal radius fracture and how long it takes for the risk of VLP removal to stabilize. The secondary purpose was to describe the reasons for VLP removal and analyze the relationship between it and the Soong index.
METHODS: This was a single-center retrospective cohort study. Patients aged >18 years with distal radius fracture who underwent VLP fixation were included. Hardware removal, time until VLP removal, and the primary reason for removal were recorded. The implant prominence was…

Background: The purpose of this study was to determine whether prominent radial shaft screws in volar locked plating of distal radius fractures increase the risk of tendon irritation, tendon rupture, or hardware removal. Methods: Patients who underwent volar locked plating of distal radius fractures from April 2002 to March 2016 with at least 6 months of follow-up were evaluated. Variables examined included demographics, extensor tendon irritation, extensor tendon rupture, and hardware removal. The most prominent cortical screws on postoperative lateral radiographs were identified and measured…
