Results of the bone-anchored hearing aid in unilateral hearing loss.

OBJECTIVES: The advantages of binaural hearing are well established and universally accepted. However, a tendency remains to withhold the benefits of binaural hearing to adults and children with one normal ear. The purpose of this study is to demonstrate the benefit of the bone-anchored hearing aid (BAHA) in a group of patients with unilateral conductive or mixed hearing loss. STUDY DESIGN: This is a prospective study of nine patients (five males and four female patients) with conductive or mixed hearing loss who met the criteria for BAHA except for having normal hearing in the other ear. The…

Investigation of binaural interference in normal-hearing and hearing-impaired adults.

When speech recognition testing is performed under diotic conditions, some elderly persons with asymmetric hearing loss exhibit a phenomenon in which the performance of the poorer ear interferes with that of the better ear. This binaural interference phenomenon has been estimated to occur in 8 to 10 percent of elderly hearing aid users. The purpose of the present study was to investigate the prevalence of this phenomenon in groups (n = 12) of young and elderly listeners with normal hearing, plus groups of elderly listeners with hearing loss who were aided or unaided. Of 48 subjects tested, onl…

Azathioprine with cortisone in treatment of hearing loss in only hearing ear.

A sudden or progressive hearing loss in the only hearing ear is an otologic emergency, in which therapeutic possibilities are limited. We describe results of immunosuppressive therapy in 10 patients with one deafened ear either due to cochlear hydrops or Meniere's disease, who developed progressive or fluctuant hearing loss in the only hearing ear. The perilymphatic fistula was excluded with tympanoscopy. Azathioprine (25 mg t.i.d.) with prednisone (initial dose 20-40 mg with daily dose of 5-15 mg/day) was used. The hearing of 6 subjects responded to treatment. The initial pure tone audiogram …

In-the-ear hearing aids. The use and benefit in the elderly hearing-impaired.

Preliminary clinical experiences with in-the-ear hearing aids (ITE-HAs) have been promising, and a beneficial effect has been demonstrated in younger hearing-impaired subjects. However, the subjects applying for audiological examination and rehabilitation are predominantly elderly people (i.e. greater than or equal to 70 years). The present questionnaire examination was performed in order to evaluate the use and benefit of ITE-HAs also in the elderly hearing-impaired, and compare with the effect in younger subjects. An extensive questionnaire was sent 6 months after hearing aid fitting to all …

[Effect of noise and other factors damaging to hearing on hearing loss].

The goal of the present paper was to investigate the relationship between hearing loss and noise exposure. Moreover, the author also examined the question whether damaging factors detected in the case history (hereditary susceptibility, toxic and infectious-toxic factors, cerebral and acoustic trauma, war, tinnitus) and working conditions (ototoxic industrial substances, shift work, hearing protection) affect hearing loss in any way. A tone and speech audiogram of 1020 persons exposed to industrial noise in a power station and non-exposed persons was evaluated for this purpose. In addition to …

Epidemiology of hearing loss and aetiological diagnosis of hearing impairment in childhood.

A total of 117 children, 55 girls and 62 boys with a median age of 8 years, range 2-12 years, was retrospectively evaluated in consideration of: (1) the prevalence rate and characteristics of permanent hearing loss (defined as the average of 500, 1000 and 2000 Hz greater than or equal to 35 dB HL on the better hearing ear); (2) the aetiology of the hearing impairment; (3) the basis for the aetiological diagnosis in the individual child, and (4) the value of non-audiological investigations. The overall prevalence of permanent hearing loss was 1.4% in this geographical area (children with recurr…

Auditory handicap of hearing impairment and the limited benefit of hearing aids.

The aim of this article is to promote a better understanding of hearing impairment as a communicative handicap, primarily in noisy environments, and to explain by means of a quantitative model the essentially limited applicability of hearing aids. After data on the prevalence of hearing impairment and of auditory handicap have been reviewed, it is explained that every hearing loss for speech can be interpreted as the sum of a loss class A (attenuation), characterized by a reduction of the levels of both speech signal and noise, and a loss D (distortion), comparable with a decrease in speech-to…

Sensorineural hearing loss in early childhood after passing newborn hearing screening.

INTRODUCTION: The newborn hearing screening (NHS) program in the Netherlands consist of two rounds of otoacoustic emissions (OAE) and a third round of automated auditory brainstem responses (AABR). The program identifies about 200 newborns with sensorineural hearing loss (SNHL) annually, enabling early intervention. Nevertheless, a subgroup of patients passes the NHS but present with SNHL in early childhood. PURPOSE: To determine the number of patients diagnosed with SNHL (< 5 years) who initially passed their NHS, and characterize this group in terms of patient demographics, NHS results, hea…

Hearing Recovery in Patients Treated for Idiopathic Sudden Sensorineural Hearing Loss.

PURPOSE: The purpose of this study was to investigate factors related to hearing recovery in patients treated for idiopathic sudden sensorineural hearing loss (SSNHL). METHOD: This study included 36 participants (23 men, 13 women) diagnosed with unilateral SSNHL. The inclusion criteria required a confirmed diagnosis of unilateral SSNHL using standard pure-tone audiometry. Associated symptoms, such as new-onset dizziness and tinnitus in the affected ear, were recorded at the time of the initial audiometric evaluation. Information regarding treatment approaches and intensity was collected by ch…

Hearing Loss After Vestibular Implantation in Sand Rats With Normal Hearing.

IMPORTANCE: Bilateral vestibular failure is a debilitating condition that may lead to oscillopia and adversely affect quality of life. Researchers have suggested that vestibular function might be restored with implantation of an external mechanical stimulation device. However, it is essential that such a device must not interfere with normal hearing. OBJECTIVE: To evaluate the effect of vestibular implant insertion on hearing in a sand rat model with normal hearing. DESIGN, SETTING, AND SUBJECTS: The study was conducted in a tertiary medical center in accord with the guidelines of the Rabin …

[Treating age-related hearing loss: hearing aids are not very popular].

Presbycusis means the deterioration of hearing resulting from the ageing process. Presbycusis can greatly affect one's quality of life; impaired hearing restricts communication and untreated presbycusis could result in social isolation and even depression. Nevertheless, only a minority of elderly with impaired hearing use a hearing aid. It is preferable to propose a hearing-aid fitting in the earliest stage possible, provided the current reimbursement criteria are met. The Dutch consensus model ('Veldnorm Hoortoestelverstrekking 2009') is the most important guideline in this regard.

Cochlear microphonics in sensorineural hearing loss: lesson from newborn hearing screening.

The diagnostic dilemma surrounding the presence of cochlear microphonics (CM) coupled with significantly elevated auditory brainstem response (ABR) thresholds in babies failing the newborn hearing screening is highlighted. A case report is presented where initial electo-diagnostic assessment could not help in differentiating between Auditory Neuropathy/Auditory Dys-synchrony (AN/AD) and sensorineural hearing loss (SNHL). In line with the protocol and guidelines provided by the national Newborn Hearing Screening Programme in the UK (NHSP) AN/AD was suspected in a baby due to the presence of CM …
