What do we actually know about a common cause of plantar heel pain? A scoping review of heel fat pad syndrome.

BACKGROUND: The heel fat pad is an important structure of the foot as it functions as a cushion to absorb shock and distribute plantar force during ambulation. Clinical practice guidelines or decision support platforms emphasize that heel fat pad syndrome (HFPS) is a distinct pathology contributing to plantar heel pain. We aimed to identify and synthesize the prevalence, etiology and diagnostic criteria, and conservative management of HFPS. METHODS: A comprehensive search was conducted in May 2021 and updated in April 2022, using MEDLINE, Scopus, Cinahl, EMBASE, Cochrane Library, SPORTDiscus,…

Haglund Syndrome: A Mechanical Cause of Posterior Heel Pain.

Haglund syndrome has become an important consideration in the differential diagnosis of posterior heel pain. It results from mechanical conflict between a prominent posterosuperior calcaneal tuberosity, the Achilles tendon, and the surrounding bursae. We report the case of a 45-year-old woman presenting with chronic posterior heel pain associated with a visible deformity of the posterior ankle. Imaging demonstrated a prominent calcaneal tuberosity, Achilles tendinopathy, and mild retrocalcaneal bursitis. The clinical presentation and imaging findings are discussed, highlighting the role of ima…

Centromere Protein F in Tumor Biology: Cancer's Achilles Heel.

BACKGROUND: Centromere protein F (CENP-F) is an important nuclear matrix protein that regulates mitosis and the cell cycle, and plays a crucial role in recruiting spindle checkpoint proteins to maintain the accuracy of chromosome segregation. Studies have shown that CENP-F is closely involved in the pathogenesis of various diseases, particularly in the development and progression of malignant tumors, where it exhibits significant oncogenic activity. OBJECTIVE: This review aims to systematically summarize the molecular structure, subcellular localization, expression regulation, intracellular t

The Heel Complex: Anatomy, Imaging, Pathologic Conditions, and Treatment.

The differential diagnosis for heel pain is broad but primarily involves abnormalities of the Achilles tendon, calcaneus, and plantar fascia. Achilles tendon disorders include tendinosis, tendinitis, and partial or complete tears. Tendinosis refers to tendon degeneration, while tendinitis is inflammation after acute overload. Untreated tendinosis can progress to partial or complete tears. Tendon disorders can be accompanied by paratenonitis or inflammation of the loose sheath enclosing the tendon. Initial management involves rehabilitation and image-guided procedures. Operative management is r…

Injected corticosteroids for treating plantar heel pain in adults.

BACKGROUND: Plantar heel pain, commonly resulting from plantar fasciitis, often results in significant morbidity. Treatment options include nonsteroidal anti-inflammatory drugs (NSAIDs), orthoses, physical therapy, physical agents (e.g. extracorporeal shock wave therapy (ESWT), laser) and invasive procedures including steroid injections. OBJECTIVES: To assess the effects (benefits and harms) of injected corticosteroids for treating plantar heel pain in adults. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register, the Cochrane Central Register of C…

Heel reconstruction with free instep flap: a case report.

INTRODUCTION: Reconstruction of weight-bearing heel defects remains a challenge because of the unique characteristics of the plantar skin. Though numerous surgical reconstructive options have been reported, the instep flap represents an ideal option and seems to be more acceptable to patients than others. However, when the heel defect expands to the instep area, the ipsilateral instep is not available for flap elevation. A free instep flap harvested from the contralateral foot can be a good solution, but this method has been scarcely reported. CASE PRESENTATION: A 41-year-old Asian man presen…

Cement mantle stress under retroversion torque at heel-strike.

The paper presents a theory of fixation failure and loosening in cemented total hip prostheses and proceeds to investigate this using an experimentally validated finite element model and two prosthesis types, namely the Charnley and the C-Stem. The study investigates the effects of retroversion torque occurring at heel-strike in combination with a loss of proximal cement/bone support and distal implant/cement support with a good distal cement/bone interface. A 3D finite element model was validated by comparison of femoral surface strains with those measured in an in vitro experimental simulati…

Diagnostic significance of radiologic measurements in posterior heel pain.

BACKGROUND: Posterior heel pain is a common complaint in both athlete and non-athlete. For diagnosis of etiology certain angles, lines and soft tissue parameters have been developed in the literature to describe the calcaneal prominence and its relationship to Achilles tendon and its bursae. OBJECTIVES: A prospective study was undertaken to evaluate the diagnostic values of these angles, lines and soft tissue parameters in posterior heel pain. MATERIALS AND METHODS: Seventy-one painful heels in 58 patients (30 males and 28 females, mean+/-S.D. age; 41.4+/-10.82 years) were clinically and rad…

Heel cord advancement in children with spastic equinus deformity.

Heel cord advancement has been advocated for treatment of spastic equinus deformity. Transferring the gastrosoleus anteriorly weakens it by changing the lever arm rather than the resting length. A retrospective review of 90 children with 122 limbs undergoing heel cord advancement revealed 11% excellent, 53% good, and 35% poor results (average follow-up 9.7 years). The results were statistically better in diplegics and community walkers. The better results in previous studies may be due to procedure modifications and shorter follow-up. Since we obtain comparable results with simpler heel cord l…

Heel coverage by a plantar myocutaneous island pedicle flap.

Defects of the heel represent a difficult reconstructive problem. Previously described methods have not always been ideal especially for the posterior heel. The weight-bearing functional requirements of the heel tissue over the calcaneus are a sensitive, well padded, durable cover. The technique of choice should provide local similar tissue plus involve a single reliable operative procedure. Skin grafts placed on the calcaneus or on a muscle transposition flap, such as the flexor digitorum brevis, abductor hallucis, or abductor digiti minimi muscle, provide a thin, insensitive, and dissimilar …

Pyrimidine addiction: an Achilles' heel of NF2-altered mesothelioma.

Therapeutic options for patients with mesothelioma remain scarce and five-year survival is a meager ten percent. According to data from three large studies, 36% of mesotheliomas harbor neurofibromatosis 2 (NF2) mutations or loss (https://bit.ly/4nGcRXk; accessed on 06.30.2025), yet no targeted therapy exists for this molecular subtype of the disease. In the current issue of EMBO Molecular Medicine, Xu et al, demonstrate that NF2-deficiency of mesothelioma cells releases an NF2-mediated Hippo-pathway restraint, activates the Yes-associated protein (YAP)–carbamoyl-phosphate synthetase 2, asparta

Baxter's nerve: the hidden culprit of chronic heel pain.

Baxter's nerve entrapment, an underrecognized cause of medial plantar heel pain, is frequently misdiagnosed as plantar fasciitis, leading to inadequate treatment. This condition involves compression of the first branch of the lateral plantar nerve, often resulting in chronic, burning pain with neurological symptoms. Despite accounting for up to 20% of chronic heel pain cases, clinical awareness remains low, delaying diagnosis and management. Differential diagnosis from plantar fasciitis, tarsal tunnel syndrome, and calcaneal stress fractures is crucial. High-resolution ultrasound, electromyogr…
