The aim of this study was to investigate the association between preoperative sagittal spinopelvic alignment and postoperative clinical outcomes after total hip arthroplasty (THA). This retrospective study included 92 patients with hip osteoarthritis who underwent primary THA between May 2013 and October 2015. Patients' characteristics, radiographic sagittal spinopelvic parameters and modified Harris Hip Scores, including function scores (gait scores and functional activities scores), were investigated. Multivariate linear regression analysis was performed to determine the associations between…

PURPOSE: Hip fusion conversion has shown mixed results, in particular a higher rate of failure than primary total hip replacement. Conversion is usually carried out by a lateral approach.
METHODS: We reported a series of 37 hip fusion conversions performed by an anterior approach. Clinical and radiographic outcomes of this unusual approach were reported at eight years of follow up.
RESULTS: At eight years of follow up, survivorship was 86. 6 % (IC 95 %: 62.4-95.7 %). Sixteen patients reported good relief of the pre-operative back spine or knee pain. PMA score was significantly improved. Two …

Total hip arthroplasty (THA) is an increasingly common procedure among elderly individuals. Although conversion THA is currently bundled in a diagnosis related group (DRG) with primary THA, there is a lack of literature supporting this classification and it has yet to be identified whether conversion THA better resembles primary or revision THA. This editorial analyzed the intraoperative and postoperative factors and functional outcomes following conversion THA, primary THA, and revision THA to understand whether the characteristics of conversion THA resemble one procedure or the other, or are…

We compared a minimally invasive surgical technique to the conventional (open approach) surgical technique used in fixation of hip fractures with the dynamic hip screw (DHS) device. Using a case-control design (44 cases and 44 controls), we tested the null hypothesis that there is no difference between the two techniques in the following outcome measures: duration of surgery, time to mobilisation and weight bearing postoperatively, length of hospital stay, mean difference of pre- and postoperative haemoglobin levels, position of the lag screw of the DHS device in the femoral head, and the tip-…

PURPOSE OF THE STUDY: Revision total hip arthroplasty (THA) after hip arthrodesis is an uncommon and challenging operation. The task would appear to be even more difficult if the arthrodesis was performed because of septic arthritis due to the theoretical risk of recurrent infection. We report our fifteen-year experience.
MATERIAL AND METHODS: This retrospective study concerned 17 procedures performed in 17 patients (11 women, 6 men) between 1988 and 2003 on 5 right and 12 left hips. All of the patients had arthrodesis for sepsis: eight subsequent to tuberculosis and nine subsequent to septic…

This study investigated the rotational response of the hip with different repairs of posterior structures after using the posterior approach in total hip replacement. Five groups were tested: (1) the normal hip without a replacement, (2) the normal hip with a vented capsule, (3) no repair of the capsule and external rotators after total hip replacement, (4) repair of only the piriformis tendon, and (5) repair of the capsule and external rotators as a flap of tissue (capsule, piriformis, obturator internus, gemellae, and quadratus) to the posterior aspect of the greater trochanter. Hemipelvis c…

Primary total hip arthroplasty using an uncemented AML total hip prosthesis (trispiked cup and a 4/5 porous coated stem) was performed in 50 patients (52 hips). The average age of the patients at the time of surgery was 47.6 years (range, 19-68 years), and the diagnosis was osteonecrosis of the femoral head in 18 hips, osteoarthritis in 16, fracture of the femoral neck in 14, osteoarthrosis secondary to childhood pyogenic arthritis in two, childhood tuberculous arthritis in one, and traumatic arthritis in one. The average followup was 11.3 years (range, 11-12 years). The average preoperative H…

A prospective study was conducted to quantitatively measure the relative muscle torque strengths in 20 consecutive women with unilateral osteoarthritic hips and 20 consecutive men with unilateral osteonecrotic hips before and after primary cementless total hip arthroplasty. Maximal isometric torque strengths in the muscles of flexion, extension, and abduction around the hip joint were measured preoperatively and postoperatively at six months and one year, using a Cybex 340 dynamometer. The results showed improved muscle function after total hip arthroplasty in both diseased and healthy hips. I…

A retrospective review was performed on 27 consecutive patients with total hip arthroplasty (THA) following failure of internal fixation of fractures of the proximal femur. The results were comparable to primary THA in femoral neck fractures. Considerably less satisfactory results were obtained in THA in intertrochanteric fractures. Bone loss and medial displacement of the femoral shaft led to high incidence of intraoperative complications and postoperative dislocations. Extreme care must be taken to avoid fracture and penetration of the femoral shaft. Autograft, allograft, or head and neck re…

Hip dislocation is a rare complication following fixation of an intertrochanteric fracture of the proximal femur, and only a few cases of this condition have been documented in the literature. Atraumatic dislocation and deep infection are the most commonly reported. We report a case of hip dislocation presenting with pain and reduced mobility six months after dynamic hip screw (DHS) fixation. Latent deep infection was diagnosed, and organisms were isolated through intraoperative sampling. The case was managed with debridement, Girdlestone excision arthroplasty, and targeted antibiotics. Latent…

Hip preservation surgery (HPS) is increasingly performed in young and active patients with the aim of delaying or avoiding total hip arthroplasty (THA). Despite advances in preservation techniques, a proportion of patients eventually progress to end-stage hip disease and require conversion to THA. Performing THA after prior HPS presents unique challenges because of altered anatomy, retained hardware, soft-tissue scarring, and compromised bone quality. This narrative review evaluates the existing literature on THA following various hip preservation procedures, including hip arthroscopy, periace…

Introduction Total hip replacement (THR) dislocation occurs when the prosthetic joint components completely separate, indicating a failure in the intended mechanics of the hip joint established by the prosthesis. This complication poses significant challenges for patients and surgeons and financially burdens healthcare systems. Methods This retrospective observational study evaluated cases of prosthetic hip dislocation within our health board that required manipulation under anesthesia (MUA) from January 1, 2021, to March 5, 2024. A total of 60 patients met the inclusion criteria. Patient data…

This study is designed to compare the extent of sacroiliac joint (SIJ) degeneration at total hip arthroplasty (THA) for two pathologies: osteoarthritis of the hip (OA) and osteonecrosis of the femoral head (ON). We also assessed the prevalence of SIJ degeneration in patients with lumbar spondylolisthesis or degenerative scoliosis. A total of 138 hips from 138 patients (69 OA and 69 ON) were assessed in this study, including 66 hips affected by OA secondary to developmental dysplasia of the hip. The degenerative changes in the SIJ and lumbar spine were evaluated prior to THA using radiographs a…

Recent data indicate that the contemporary prevalence of dislocation after primary total hip arthroplasty (THA) is up to 5-fold to 10-fold greater in those patients with spinal deformities that lead to stiffness and/or significant pelvic tilt. To put the scope of the problem into perspective, the dislocation risk in these patients approaches that of revision THAs with compromised trochanters and megaprostheses. The next several years will see an explosion in the knowledge we as a community garner in regards to the interplay between the hip and spine, incrementally improving the outcomes of THA…
