Posterior fracture dislocation of the hip from a Warrior turret injury.

Fracture of the femoral head after hip dislocation is a relatively rare injury and is often associated with a poor functional outcome. Most result from high impact motor vehicle accidents. We report the case of a soldier who trapped his foot in a traversing Warrior turret and suffered a severe abduction injury to his hip. He sustained a Pipkin Type I fracture-dislocation of the hip not amenable to closed reduction and required transfer to the American Hospital in Baghdad where he underwent emergency open reduction and internal fixation of the fracture. This case report illustrates an unusual m

Hip disorders in children who have spinal cord injury.

Little has been written regarding the assessment and treatment of hip disorders in children who have underlying paralysis. Each year approximately 2000 people younger than 20 years of age suffer a spinal cord injury (SCI). This compares with a larger number of children who have other forms of neurologic disorders, such as myelodysplasia, which affects approximately 6000 newborns annually in the United States, and for which there is a large body of literature describing the natural history and treatment of hip disorders in children who have myelodysplasia. This article focuses on hip disorders

Traumatic Obturator Hip Dislocation: Injury Characteristics and Clinical Outcomes.

Purpose Traumatic hip dislocation into the obturator foramen is poorly understood due to the rarity of the injury. The purpose of this study was to analyze the mechanism of trauma, associated injuries, treatment, and complications in patients who sustained obturator hip dislocation and to evaluate long-term outcomes, including patient-reported outcome measures (PROMs). Methods Patient demographics, trauma mechanisms, concomitant injuries, and the treatment of all consecutive patients who sustained an obturator hip dislocation at three level-one trauma centers from 2009 to 2024 were analyzed. Outcome of surgery for nerve injury following total hip arthroplasty.

PURPOSE: During hip replacement, nerves in the operative field can be injured accidently. Additional surgical procedures can help restore function and reduce pain if there has been no recovery. METHODS: One-hundred sixty-two patients presented with nerve injuries, and 113 were candidates for surgical treatment. The author performed 91 procedures on the injured limb (80 patients; 11 had 2 procedures) consisting of shortening, sympathectomy, tendon transfer, neurolysis or nerve repair/graft and 30 spinal and three fibular-neck decompressions. A meaningful recovery was defined as an increase of

Sciatic nerve injury in total hip resurfacing: a biomechanical analysis.

The condition of the gluteal sling was a significant factor in determining the pressure experienced by the sciatic nerve during acetabular exposure in total hip resurfacing via a posterior approach. The position of the knee did not play a significant role at this stage of the procedure. Average pressures were not elevated above a predefined injury level during positioning for femoral preparation. During hip reduction, knee positioning seemed to play a significant role in pressures placed on the sciatic nerve. These findings suggest that releasing the gluteal sling during a posterior approach f

Irreducible fracture-dislocation of the hip: a severe injury with a poor prognosis.

OBJECTIVE: To determine the outcome of and prognostic factors associated with irreducible fracture-dislocations of the hip. DESIGN: Retrospective review of a prospectively gathered trauma database. METHODS: Using a prospectively gathered trauma database, we identified twenty-five patients with fracture-dislocations of the hip that were irreducible with closed means under a general anaesthetic. All were victims of high-energy trauma with severe injuries (mean Injury Severity Score = 25, range 9 to 54). Eighteen patients had multiple fractures, fifteen had associated systemic injury, and only

Neurologic injury in the upper extremity after total hip arthroplasty.

The results of 7150 consecutive primary and revision total hip arthroplasties performed between 1976 and 1990 were reviewed retrospectively. Sixteen upper extremity neurologic palsies were identified in 16 patients. The incidence of upper extremity nerve palsies after total hip arthroplasty was 0.22%. There were five men and 11 women (average age, 59.5 years; range, 27-81 years). The neurologic injuries consisted of 10 ulnar palsies, four brachial plexopathies, one axillary nerve palsy, and one median nerve palsy. Patients were evaluated with respect to age, gender, preoperative diagnosis, typ

Vascular injury during total hip arthroplasty: the anatomy of the acetabulum.

We encountered a massive retroperitoneal haemorrhage due to perforation of the external iliac vein by a drill tip passing through the antero-superior quadrant of the acetabulum during cementless total hip arthroplasty. An anatomical study of the acetabulum was undertaken to determine the safe zones for transacetabular screw fixation. Five cadavera were studied to find the proper locations for screws using acetabular quadrants. The postero-superior and postero-inferior acetabular quadrants have good bone stock and are safe for the transacetabular replacement of screws. The antero-superior and i

Hip and groin injury is the most common non-time-loss injury in female amateur football.

PURPOSE: Hip and groin injuries in football are problematic due to their high incidence and risk of chronicity and recurrence. The use of only time-loss injury definitions may underestimate the burden of hip and groin injuries. Little is known about hip and groin injury epidemiology in female football. The first aim of this study was to examine the within-season (2014-2015) prevalence of total injury with and without time-loss in female amateur football players. The second aim was to study the within-season and preseason (2015-2016) prevalence of hip/groin injuries with and without time-loss.

Postoperative Femoral Shaft Malrotation Leading To Recurrent Hip Dislocation In A Patient With Floating Hip Injury: A Case Report.

Floating hip injuries represent high-energy trauma with significant biomechanical instability. When associated with hip dislocation, unrecognized femoral shaft malrotation may alter femoral version and compromise the maintenance of a stable reduction. We present a case of postoperative recurrent hip dislocation following fixation of a floating hip injury, in which femoral shaft malrotation was identified as a key contributing factor. In the initial procedure, the femoral head was reduced prior to femoral shaft fixation and posterior acetabular wall reconstruction. However, due to persistent su…

Pain as a Driver of Myocardial Injury in Hip Fracture Patients: A Hip Fracture Accelerated Surgical Treatment and Care Track (HIP ATTACK) Trial Secondary Analysis.

BACKGROUND: Pain is a common yet underaddressed issue in surgical inpatients. Inadequate pain control in patients with hip fractures can lead to adverse hemodynamic changes, potentially increasing the risk of myocardial ischemia. The authors aim to investigate whether perioperative pain is associated with the development of myocardial ischemic events in patients undergoing hip fracture surgery. METHODS: HIP fracture Accelerated surgical TreaTment And Care tracK (HIP ATTACK) was a multicenter international randomized controlled trial comparing the effects of accelerated surgical repair of hip

Posterior hip dislocation associated with concomitant ipsilateral femoral head and shaft fractures: an unusual combination of injuries.

The combination of posterior hip dislocation with an ipsilateral femoral head and shaft fractures is unusual. While cases of concomitant fractures of femoral head and shaft have been previously reported, the treatment of such injuries is challenging. Presence of an associated hip dislocation further complicates the matter. A timely diagnosis and treatment are crucial to have a good outcome. We are presenting the case of a 20-year-old man who sustained a traumatic posterior hip dislocation with ipsilateral femoral shaft and femur head fractures. After reducing the hip, we fixed the femoral shaft

Relationship between upper limb injuries and hip range of motion and strength in high school baseball pitchers.

We aimed to examine the relationship between hip range of motion (ROM) and abduction strength and throwing-related shoulder/elbow injuries in high school baseball pitchers. The study included 135 baseball pitchers. We asked them to fill out a questionnaire at the checkups, that included the dominant arm and the years of baseball experience. To avoid a confirmation bias, the examiners were blinded to the participants' hand dominance. All players underwent physical function measurements, such as height, weight, shoulder and hip strength, and shoulder and hip ROM. Shoulder and elbow injury was de

Altered hip control during a standing knee-lift test is associated with increased risk of knee injuries.

A few prospective studies have investigated hip and pelvic control as a risk factor for lower extremity (LE) injuries. The purpose of this study was to investigate whether deficits in hip and lumbopelvic control during standing knee-lift test are associated with increased risk of acute knee and LE injuries in youth team sports. At baseline, 258 basketball and floorball players (aged 12-21 years) participated in a standing knee-lift test using 3-dimensional motion analysis. Two trials per leg were recorded from each participant. Peak sagittal plane pelvic tilt and frontal plane pelvic drop/hike

Obturator dislocations of the hip associated with pelvic ring injuries: Case report and review of the literature.

CASE: An obturator hip dislocation with an associated open book pelvic ring injury is an extremely rare injury pattern. This case report will discuss challenges to closed reduction, acute management strategies and review the literature on combined hip dislocations and open book pelvic ring injuries. CONCLUSION: This injury pattern presents unique reduction challenges that should be recognized early in order to provide effective resuscitation and preserve the femoral head blood supply. Failing to close reduce the hip delays reducing the pelvic ring volume because sheets and binders are preclud

Prospective study of nerve injuries associated with hip arthroscopy in the lateral position using the modified portals.

To access the central compartment of the hip, distraction is essential in hip arthroscopy (HA); nerve injuries have long been accepted as a complication of this surgical procedure, with an incidence ranging from 0 to 46%. Only one previous article collected data prospectively, and the authors utilized a supine technique, with a modified mid-anterior portal. Our study also used prospectively collected data, from a group of 200 consecutive patients who had HA performed in the lateral position using the paratrochanteric portals. Our results were that four patients (2%) reported symptoms of neurol
