Hospital characteristics affect outcomes for common pediatric surgical conditions.

Appendicitis, hypertrophic pyloric stenosis (HPS), and intussusception are common conditions treated in most hospitals. In which hospital settings are children with these conditions treated? Are there differences in outcomes based on hospital characteristics? Our purpose was to use a nationwide database to address these questions. Data were extracted from Kids' Inpatient Database 2000. Data were queried by International Classification of Diseases procedure code for appendectomy and pyloromyotomy and by diagnosis code for intussusception. Length of stay (LOS) and hospital charges were analyzed …

Saving the split: the benefits of VATS thymectomy.

BACKGROUND: With the advent of minimally invasive techniques, the standard approaches to many surgeries have changed. We compared the financial costs and health care outcomes between standard thymectomy via sternotomy and video assisted thoracoscopic surgery (VATS). METHODS: A 3-year review [2010-2012] of the National Inpatient Sample (NIS) was performed. All patients undergoing thymectomy were included. Patients undergoing VATS thymectomy were identified. Outcomes measured were hospital length of stay (LOS), hospital charges, and mortality. Univariate and multivariate analyses were performed…

A prospective randomized cost billing comparison of local fasciocutaneous perforator versus free Gracilis flap reconstruction for lower limb in a developing economy.

UNLABELLED: Distal half leg complex wounds are usually a formidable problem that necessitates either local or free flap coverage. The aim of this study was to compare cost billing charges in free Gracilis flap (fGF) and local fasciocutaneous perforator flap (lFPF) in reconstructing complex soft tissue leg and foot defects. PATIENTS AND METHODS: Thirty consecutive adult (>15-year-old) patients with soft tissue defects in the leg and/or foot requiring tissue coverage with a flap in the period between 2012 and 2015 were randomly assigned (block randomization) to either an fGF or lFPF procedure.

Osteomyelitis related to pressure ulcers: the cost of neglect.

Twelve patients with documented chronic osteomyelitis of the pelvis resulting from truncal pressure ulcers were examined retrospectively to identify the cost of treatment for this significant health care problem. The retrospective review of each case spanned an 18-month period--6 months prior to the initial positive bone biopsy to 1 year following bone biopsy. The financial charges associated with treatment of osteomyelitis were identified using the University of Michigan Health System's databases for hospital charges, professional charges, and pharmacy charges. Prior treatment of these patien…

Laparoscopic vs. open surgery for acute adhesive small-bowel obstruction: patients' outcome and cost-effectiveness.

BACKGROUND: Numerous studies have demonstrated the feasibility of laparoscopy in the management of acute adhesive small-bowel obstruction (AASBO). However, comparative data with laparotomy are lacking. The aim of this study was to compare laparoscopy and laparotomy for the treatment of AASBO in terms of patient outcome and cost-effectiveness. METHODS: A retrospective chart review of all patients who underwent surgery for AASBO from 1999 to 2005 was conducted. Data recorded included operative and postoperative course, among others. Operative and total hospital charges were estimated from the P…

National trends in hospital outcomes among patients with Guillain-Barré syndrome requiring mechanical ventilation.

Several new treatments have been introduced for Guillain-Barré syndrome over the last decade. To assess the impact of these new strategies on outcomes and hospitalization charges among patients with Guillain-Barré syndrome requiring mechanical ventilation, we compared pertinent variables between nationally representative data derived from 1992 and 2002. Compared with patients admitted in 1992, the patients admitted in 2002 showed an increase in hospital charges ($168,600 versus $116,300, P = 0.007), longer hospitalization (52.6 +/- 23.3 versus 40.3 +/- 36.3 days, P = 0.017), and greater in-hos…

Impact of urinary tract infection definitions on colorectal outcomes.

BACKGROUND: Hospital-acquired urinary tract infections (UTIs) significantly impact hospital outcomes. Colorectal surgery is inherently high risk for postoperative infections including UTI, and these patients may have unique outcomes as compared to other medical and surgical hospitalizations. We aim to assess the impact of the differing definitions of UTI captured by our hospital quality measures on hospital charges, length of stay (LOS), and mortality after colorectal resections at our institution. MATERIALS AND METHODS: Existing hospital quality surveillance was used to retrospectively ident…

Clostridioides difficile infection in hematologic malignancy patients with and without bone marrow transplantation: A nation-wide cross-sectional study.

Clostridioides difficile infection (CDI) is a frequent and consequential healthcare-associated complication in patients with hematologic malignancies (HMs), particularly in the setting of bone marrow transplant (BMT); however, national data comparing the impact of CDI across HM hospitalizations with and without BMT remain significantly limited. To evaluate associations between CDI and key inpatient outcomes like in-hospital mortality, length of stay (LOS), and total hospital charges, this retrospective cross-sectional analysis was conducted using the Nationwide Inpatient Sample. Multivariable …

Hospital frailty risk score predicts postoperative outcomes after endoscopic endonasal resection of non-functioning pituitary adenomas.

PURPOSE: Frailty indices are invaluable resources in risk stratification and predicting high-value care outcomes for neurosurgical patients. The Hospital Frailty Risk Score (HFRS) is a recently developed and validated method for evaluating frailty; however, its implementation has yet to be assessed in patients with non-functioning pituitary adenomas undergoing endoscopic endonasal resection. In this study, we aimed to evaluate HFRS's predictive ability for high-value care outcomes, namely postoperative complications, length of stay (LOS), and hospital charges, and to compare it to other tradit…
