Bladder diverticula can be either congenital or acquired. They are the formation of a urothelial hernia through the muscle wall fibers of the bladder. Congenital bladder diverticula are rare and there are no previous reports showing them to have been found through an antenatal ultrasound exam revealing the presence of hydronephrosis. Here we describe the case of a large congenital bladder diverticulum revealed through an antenatal hydronephrosis diagnosis and whose proximity to the ureter led to its removal and a ureteral reimplantation.

The aim of this study is to evaluate the significance of renal pelvis aspiration (RPA) in the management of antenatal hydronephrosis (AHN). This study enrolled 15 AHN cases (one twin pregnancy) that necessitated RPA for AHN. We report the case of a malrotated right kidney associated with giant prenatal hydronephrosis due to a non vascular extrinsic ureteropelvic compression. The kidney presented an hyper-rotation of 180° in relation to the original fetal position, with the renal hilum backward looking. At neonatal surgery we discover that the inferior pole of the kidney pushes laterally the ureteropelvic junction like a violin bridge. As many as 1% of newborn infants have a prenatal diagnosis of hydronephrosis or significant renal pelvic dilation. Hydronephrosis often is caused by nonobstructive conditions. Twelve cases of traumatic rupture of a massive hydronephrosis are reported. The key points in the diagnosis are: injury around the loin, peritonitis, a fall of pH in the abdominal paracentesis fluid, a rise of blood urea nitrogen (BUN) and structural abnormality of the affected renal pelvis seen on ultrasonography. The treatment differed according to the disease causing the hydronephrosis before the injury. All patients were treated surgically with a satisfactory overall cure rate. Follow-up of 12 years has not revealed any significant sequelae.

A retrospective study was carried out on patients with gastrointestinal complaints, who were accidentally found to have varying degrees of hydronephrosis between October 1989 and April 1991. The criteria for hydronephrosis given by Ellenbogen et al were used for ultrasonographic diagnosis. The management of neonates with mild hydronephrosis diagnosed antenatally is still debated. Although some of these infants are normal, it is recognised that others will have mild obstruction of the ureteropelvic junction or vesicoureteric reflux (VUR). Renal resistive indexes were measured by Doppler sonography in 12 children undergoing other standard diagnostic studies to evaluate hydronephrosis. Measurement of renal resistive indexes was modified by prior placement of a bladder catheter, oral hydration and administration of 1 mg./kg. furosemide after baseline measurement. Newly developed diagnostic techniques allowed us to select a fetus with potentially reversible renal damage from the usually fatal group with bilateral hydronephrosis and severe oligohydramnios early in gestation. Fetal surgery to marsupialize the fetal bladder at 24 weeks gestation restored normal amniotic fluid dynamics and allowed sufficient pulmonary and renal development to insure survival after delivery near term.
