Idiopathic hydrops fetalis successfully treated in utero.

Using ultrasound, nonimmunologic hydrops fetalis can be diagnosed antenatally, but the antenatal management remains debatable. To treat a fetus with idiopathic hydrops fetalis, we injected albumin into the fetal abdomen in utero. After this treatment, the hourly fetal urine production rate increased significantly, and edema and ascites disappeared subsequently at two weeks and three weeks, respectively. Edema and ascites were absent at birth. The child remains well at 18 months of age.

[Non-immunologic hydrops fetalis and congenital chylothorax].

A child was born at the 37th week of pregnancy with hydrops fetalis. Hydramnios and hydrothorax had been proven by fetal ultrasonography. No fetal or maternal etiology was found. At age 4 days, at the beginning of enteral nutrition, the pleural effusion became characteristic of chylothorax. Recovery occurred after 2 weeks of parenteral nutrition. Chylothorax might be an unrecognized etiology of non immune hydrops fetalis. The relationships between both conditions and the interest of prenatal treatment are discussed.

Nonimmune hydrops fetalis: antenatal diagnosis and management.

An 8-year (1975-1982) study of 26 cases of nonimmune fetal hydrops revealed the main causes to be: anomalies (11/26), tachyarrhythmias (4/26), and twin-to-twin transfusion (4/26). The perinatal mortality was 19/26 (76%). Based on this series, and a review of the literature, a plan for the antenatal diagnosis and management of these cases is outlined.

Non-immunological hydrops fetalis. a case report.

A case of hydrops fetalis which was not due to isoimmunization is presented. The condition was diagnosed antenatally by means of ultrasonography and the infant was delivered at 32 weeks' gestation. He required intensive care, but survived and is well at 18 months of age. The causation, diagnosis and management of this problem are discussed.

Hydrops fetalis: sonographic evaluation and clinical implications.

A systematic sonographic evaluation of hydropic fetus is presented, based on 21 cases and a literature review. The clinical implications of fetal ascites with or without anasarca, maternal hydramnios, maternal oligohydramnios, or an abnormally thick placenta are discussed as they relate to fetal outcome. It is concluded that sonography could play a major role in determining the optimal approach to perinatal management of the fetus in hydrops fetalis, and thus contribute to a reduction in the perinatal mortality and morbidity associated with this disorder.

Differential Diagnosis of Hydrops Fetalis: An Imaging Guide.

Hydrops fetalis is a critical diagnosis given the poor prognosis and vast differential of potential causes. After a detailed anatomic survey, the first step in differentiating among the various causes is measurement of the middle cerebral artery peak systolic velocity to evaluate for anemia. Anemia is the key decision point in the diagnostic algorithm that categorizes hydrops. This approach is more practical than an immune versus nonimmune classification because it capitalizes on clues available at the time of sonographic diagnosis and expedites care toward possible therapies, such as intraute…

Perinatal-lethal Gaucher disease presenting as hydrops fetalis.

Perinatal-lethal Gaucher disease is very rare and is considered a variant of type 2 Gaucher disease that occurs in the neonatal period. The most distinct features of perinatal-lethal Gaucher disease are non-immune hydrops fetalis. Less common signs of the disease are hepatosplenomegaly, ichthyosis and arthrogryposis. We report a case of Gaucher's disease (type 2) diagnosed in a newborn who presented with Hydrops Fetalis.

A catecholamine-secreting neuroblastoma leading to hydrops fetalis.

A case of fetal neuroblastoma of the right adrenal gland, with rapid development of hydrops fetalis due to catecholamine-induced cardiomyopathy, is reported. A fetus with a right suprarenal mass detected during ultrasonography at 32 weeks gestation progressively developed into hydrops fetalis by 35.2 weeks gestation. An emergent cesarean section was performed. At birth, the female neonate was hypertensive, with markedly elevated catecholamine levels; echocardiography showed poor contractility. Morphine, human atrial natriuretic peptide, milrinone, nitroprusside and dobutamine were initiated an…

Accessory diaphragm associated with non-immune hydrops fetalis.

An accessory diaphragm, also known as diaphragmatic duplication, is a congenital anomaly in which there is a fibromuscular membrane on top of the normally formed diaphragm dividing the hemithorax into two compartments trapping part of the pulmonary parenchyma. It is a very rare anomaly with less than 40 cases reported in the literature, of which only five were diagnosed in the newborn period, with no reports on any clinical clues for the antenatal diagnosis of this condition. We describe a case of congenital accessory diaphragm presenting in the antenatal period as hydrops fetalis. We also des…

Etiology of nonimmune hydrops fetalis: a systematic review.

Hydrops fetalis (HF) indicates excessive fluid accumulation within the fetal extravascular compartments and body cavities. HF is not a diagnosis in itself but a symptom, and the end-stage of a wide variety of disorders. In the era before routine immunization of Rhesus (Rh) negative mothers, most cases of hydrops were due to erythroblastosis from Rh alloimmunization, but nowadays, nonimmune hydrops fetalis (NIHF) is more frequent, representing 76-87% of all described HF cases. We performed a systematic review of the pertinent literature based on the QUality Of Reporting Of Meta-analyses (QUOROM…

Anti-C(w) alloimmunization presenting as hydrops fetalis.

UNLABELLED: C(w) is a low frequency red cell antigen that belongs to the Rh blood groups system. While not uncommon, anti-C(w) is rarely associated with clinically significant haemolytic disease of the newborn (HDN). When it does occur, it is often subclinical or of mild to moderate clinical severity. In the majority of pregnancies it is considered to be a naturally occurring antibody and has not been reported to cause hydrops fetalis or stillbirth. We report a case of anti-C(w) alloimmunization, which was associated with significant anaemia and hydrops fetalis, presenting at 35 wk gestation. …
