Growth in congenital adrenal hyperplasia.

Individuals with congenital adrenal hyperplasia (CAH) are shorter, on an average, than the general population. A recent meta analysis of final height in CAH indicated that the height deficit is typically 1 to 2 standard deviations below the mean in both males and females. Growth in CAH due to 21-hydroxylase deficiency is influenced by a number of factors, related both to the underlying disease and its treatment. In general, males with the simple virilising form have the poorest height prognosis. This relates in part to late diagnosis and treatment and the bone age advancement seen in individua…

Non-classic congenital adrenal hyperplasia.

Congenital adrenal hyperplasia results from a defect in the biosynthetic pathway of cortisol and/or aldosterone. Non-classical ( NCAH) forms are characterized by milder enzyme dysfunction and manifests commonly in adolescence or adulthood. The most common form of NCAH is 21 hydroxylase (21-OH) deficiency. Also, a number of NCAH patients with 11 beta hydroxylase (11 beta-OH) deficiency have been reported in some populations. Although some patients with NCAH may be asymptomatic, hirsutism is the most common symptom. It may be clinically indistinguishable from PCOS. Mutations in the CYP21 (CYP21A…

Coronoid hyperplasia. A case report.

Coronoid hyperplasia is a rare condition which is macroscopically characterized by an increase in the dimensions of the coronoid process resulting from an abnormal bony elongation of histologically normal bone. Unilateral cases are more frequent than bilateral ones and can recognize a number of etiological factors, such as exostoses, osteochondroma, traumatic events, inflammatory reactions, neoplasia and manifestations secondary to other pathologies. Etiopathogenesis of bilateral forms has not yet been clarified: in the literature hints to both developmental and endocrine abnormalities are pre…

Investigation of benign prostatic hyperplasia.

PURPOSE OF REVIEW: This is a concise and timely update of the most recent developments in the clinical research on the investigation of benign prostatic hyperplasia. RECENT FINDINGS: Analysis of the peer-review literature confirms a high level of consensus as to the management of prostatic hyperplasia; analysis of practice trends suggests a satisfactory degree of adoption of the proposed guidelines. The evaluation of lower urinary tract symptoms remains the mainstay in the investigation of prostatic hyperplasia although problems concerning the evaluation of nocturia and the differential diagn…

Congenital adrenal hyperplasia in pregnancy.

The congenital adrenal hyperplasias (CAH) are a group of inherited enzymatic defects of adrenal steroid biosynthesis. Deficiencies of each enzyme required in the steroid biosynthesis pathway are known, and these deficiencies are all inherited as autosomal recessive disorders. During pregnancy, maternal and fetal problems are confined to women who have 21-hydroxylase deficiency (P450c21 deficiency), 11-hydroxylase deficiency (P450c11 deficiency), and 3 beta-hydroxysteroid dehydrogenase deficiency (3 beta HSD deficiency), because other adrenal enzyme deficiencies are not compatible with fertilit…

Management of benign prostatic hyperplasia.

Benign prostatic hyperplasia (BPH) is a common cause of morbidity among older men. Primary care physicians need to develop expertise in its management. The causes of BPH are unknown; aging and the presence of male androgens are the dominant risk factors. Obtaining a medical history is a key step in assessing whether lower urinary tract symptoms are due to BPH or to some other process. A reliable and valid seven-item questionnaire has been developed to objectively assess symptom severity. A focused physical examination including a digital rectal exam should be performed, as well as a urinalysis…

[Mandibular condyle hyperplasia. Therapeutic review].

Excessive cartilage growth on the condyle process of the mandible can result from primary hyperreactivity of the growth cartilage or be a secondary adaptation to an imbalance in occlusive and/or cervicofacial conditions. Treatment depends on the distinction between these two forms. Primary active overgrowth is treated by condylectomy sparing the distal apparatus although conservative surgery to re-centering the temporomandibular joint and re-establish symmetry without condylectomy may be used in quiescent moderately active forms saving the joint. For secondary forms, the joint is re-centering

Pseudoangiomatous hyperplasia of male breast.

Ninety-three male breast specimens have been examined for the presence of pseudoangiomatous hyperplasia of the mammary stroma which has hitherto been described almost exclusively in females. Forty-four cases (47.4%) showed some degree of hyperplasia, varying from small microscopic foci to extensive change involving 90% of the mammary tissue. All but one were found in association with gynaecomastia, early and intermediate stage. The association between pseudoangiomatous hyperplasia and benign proliferative lesions mirrors that reported in the female breast, and our findings suggest that the cha…

Etiology of benign prostatic hyperplasia.

The natural history of benign prostatic hyperplasia (BPH) involves two phases. The first, or pathologic phase, is composed of two stages, microscopic and macroscopic, neither of which produces symptomatic clinical dysuria. Nearly all men throughout the world eventually develop microscopic BPH if they live long enough. In only about 50% of men, however, does microscopic BPH grow to produce a macroscopic enlargement of the gland, suggesting that additional factors are required for BPH progression. The second, or clinical, phase of BPH involves the progression of pathologic BPH to its clinical fo…

[Management of congenital adrenal hyperplasia].

This review deals with the current problems in the management of classical 21-hydroxylase deficiency from the fetal life to the puberty. The clinical consequences of 21-hydroxylase deficiency reflect the disordered physiology--impaired secretion of glucocorticoids and mineralocorticoids, and excessive secretion of androgens. Current therapy is intended to correct the disordered physiology by replacing mineralocorticoid and glucocorticoid hormones, thereby reducing the ACTH-driven increase in adrenal androgen secretion. Treated patients should expect a normal life span and reproductive potentia

Familial pulmonary nodular lymphoid hyperplasia.

We describe three persons in one family with diffuse lung infiltrates, digital clubbing, and chronic hypoxia. Elevated immunoglobulin levels and antinuclear antibodies were found in all patients; pathologic findings included hyperplastic lymphoid follicles infiltrating the epithelium of the small airways. These cases may represent a familial systemic autoimmune disorder seen primarily with pulmonary compromise.

Mineralocorticoids in congenital adrenal hyperplasia.

While hypertension is observed in only two of the three major subtypes of congenital adrenal hyperplasia (CAH), 11 beta- and 17 alpha-hydroxylase deficiencies, deoxycorticosterone (DOC) production is increased in all. The elevated zona fasciculata (ZF) DOC produces mineralocorticoid hypertension with suppressed renin and reduced potassium concentrations. The DOC levels in 21-hydroxylase deficiency are in part produced by renin stimulation of the Zona glomerulosa (ZG) along with aldosterone. Assessment of the mineralocorticoid hormones of the ZF and ZF (17-deoxy steroids) provides additional un…

Chronic cholecystitis with lymphoid hyperplasia.

The clinico-pathologic features of six cases of chronic cholecystitis with focal lymphoid hyperplasia (CCLH) are described and compared with five examples of malignant lymphoma involving the gallbladder. The clinical presentation of CCLH was identical to that of conventional chronic cholecystitis with cholelithiasis. Microscopically there was chronic inflammation and fibrosis in the wall of the gallbladder as well as extensive lymphoid hyperplasia with many lymphoid follicles having germinal centers. The five patients with malignant lymphoma also had chronic cholecystitis; four had a previous …

Cushingoid adrenal hyperplasia in infancy.

Cushing's syndrome, a characteristic pattern of obesity with hypertension due to the hyperfunction of the adrenal cortex, is relatively rare in infancy. Thirty-six infants have been reported in world literature, most of whom have had adrenal tumours. There are only eight reported cases of infants under the age of 1 year with adrenal hyperplasia responsible for Cushing's syndrome. This is a report of an 8 month old child with bilateral nodular adrenal hyperplasia.

Kaposi's sarcoma: a reversible hyperplasia.

Kaposi's sarcoma has many unusual features: for example, the pronounced male preponderance; its appearance in "crops" rather than as primary tumour with metastases; a substantial rate of spontaneous remission; the predictability of involved sites; the lack of aneuploidy; and the strong association with immunodeficiency. These features and other evidence suggest that it is not a malignant neoplasm but a benign, potentially controllable and reversible hyperplasia.

Nodular hyperplasia of Brunner's glands.

A clinical, endoscopic, and histological study of 206 cases of nodular hyperplasia of Brunner's glands was carried out. Firm nodules with a reddened surface due to hyperplastic Brunner's glands were limited mainly to the first part of the duodenum and affected almost exclusively male patients. Gastric acid secretion after pentagastrin stimulation was significantly increased compared to normal. In most cases, biopsies of the nodule center revealed spreading of Brunner's glands from within the lamina propria to the surface epithelium, whereas in biopsies performed between nodules, Brunner's glan…
