Prospective study of immunological factors in non-inflammatory bowel disease enterocutaneous fistulas.

BACKGROUND: Enterocutaneous fistulas (ECF) are debilitating and usually result following complex abdominal surgery. While there is an association with inflammatory bowel disease (IBD), a large number of fistulas occur after surgery not related to IBD. The consequences of ECF include short bowel syndrome and the need for long term parenteral nutrition.ECF can heal spontaneously and in the case of IBD can be cured by medical therapy in some instances. Those that do not resolve spontaneously have to be cured by surgery which is complex and associated with a high morbidity. It is not considered tr

Molecular biology and immunology for clinicians 20: Angiogenesis and vascular growth factors and signals.

If we accept the perfectly reasonable premise that the mass of inflammatory tissue in rheumatoid arthritis (and psoriatic arthritis or any other inflammatory joint disease) requires oxygen and nutrition to survive and grow, we are confronted with a novel concept for therapy: if we can block the nutritional supply of the pannus, we can suppress or prevent its growth and the subsequent destruction of the joint. Thus, an understanding of how new blood vessels nourish the inflammatory mass could be pivotal in successfully treating our patients. Angiogenesis is the process whereby new blood vessels

Immunological and viral factors associated with the response of vulval intraepithelial neoplasia to photodynamic therapy.

Topical 5-aminolevulinic acid-based photodynamic therapy (PDT) has produced complete response rates of >90% for nonmelanoma skin carcinomas, which are mostly human papillomavirus (HPV) negative. Using a similar treatment protocol, we observed a short-term response in only one third (10 of 32) of high-grade vulval intraepithelial neoplasia (VIN 2-3) lesions. Unifocal lesions were found more responsive than multifocal and pigmented lesions. Animal model studies have suggested that long-term PDT response involves an immune reaction in which CTLs play a crucial role. In this study, we have assesse

Prognostic factors in Sézary syndrome: a multivariate analysis of clinical, haematological and immunological features.

BACKGROUND: Sézary syndrome (SS) prognostic factors are not well defined because of the rarity of this disease. The specific goal of this prospective study was to assess by multivariate analysis the predictive value with respect to survival of a series of clinical, haematological and immunological parameters taken at SS diagnosis. PATIENTS AND METHODS: A cohort of 62 SS patients diagnosed and followed since 1975 was examined, and 51 were included in the multivariate analysis model. RESULTS: The median survival time was 31 months (range: 1 month-15.7+ years), and the five-year survival rate 3

The expanding role of nerve growth factor: from neurotrophic activity to immunologic diseases.

Numerous studies published in the last 10-15 years have shown that nerve growth factor (NGF), a polypeptide originally discovered in connection with its neurotrophic activity, also acts on cells of the immune system. NGF has been found in various immune organs including the spleen, lymph nodes, and thymus, and cells such as mast cells, eosinophils, and B and T cells. The circulating levels of NGF increase in inflammatory responses, in various autoimmune diseases, in parasitic infections, and in allergic diseases. Stress-related events both in animal models and in man also result in an increase

An immunoperoxidase study of immunological factors in high immune and low resistance granulomas in leprosy.

The epithelioid cell granuloma in high resistant tuberculoid (TT) leprosy was contrasted with the pure macrophage granuloma of anergic lepromatous leprosy (LL) by evaluating various immunological factors operating in these lesions. The immunoperoxidase technique using antisera to immunoglobulin IgG, IgM, complement C3, C3d and C1q and other products of macrophage secretion, lysozyme, plasminogen, a1 antitrypsin and C-reactive protein and of Ia antigens revealed peak levels in tissues of most of these factors in both types of granuloma. The tuberculoid response was linked to low antigenic load

[Activity of nonspecific immunological factors and serum immunoglobulin levels in children with diabetes mellitus].

The activity of non-specific protection factors (complement titer, lysozyme level, completed phagocytosis index and serum immunoglobulin (IG) concentration) was studied in 100 children, aged 3 to 14 years: 70 with diabetes mellitus and 30 normal subjects. The results obtained indicate a decreased reactivity of the child organism in diabetes mellitus, the reduction degree being directly proportional to the main process severity and decompensation extent. Serum IG level in children, suffering from diabetes mellitus, is disturbed, i.e. high IgA concentration is often seen in the presence of norma

[Treatment of hemophilia with immunologic inhibitor of factor VIII by using "activated" coagulant fractions].

Bleeding episodes in hemophiliacs with inhibitor to factor VIII are often dramatic situations. High level inhibitors do not respond to massive amount of factor VIII and are often refractory to immunosuppressive drugs such as cyclophosphamide. In this situation it has been proposed to use "activated prothrombin complex" to control bleeding in hemophilic patients with inhibitors. A plasmatic fraction "Auto IX" has been used successfully for the treatment of an intestinal hemorrhage in a patient with an inhibitor to F. VIII. Possible mechanisms of action as well as potential thrombotic danger of

Immunological properties of isolated IgG and IgM anti-gamma-globulins (rheumatoid factors).

Anti-gamma-globulins of the IgG and IgM classes have been isolated from sera of normal individuals and from patients with osteoarthritis, rheumatoid arthritis and juvenile rheumatoid arthritis. All of the isolated antibodies gave precipitin curves with heat-aggregated, reduced and alkylated gamma-globulin. IgM anti-gamma-globulins gave a positive latex fixation test at 4°C and 37°C while IgG anti-gamma-globulins generally gave a positive test only at 4°C. Anti-gamma-globulins from normals did not fix complement but IgG and IgM anti-gamma-globulins from rheumatoids fixed complement to a similar

An immunologic approach to induction of epidermal growth factor deficiency: induction and characterization of autoantibodies to epidermal growth factor in rats.

Epidermal growth factor (EGF) in pharmacologic doses is able to induce growth and development in the fetus and the newborn. To investigate the opposite situation, the effects of insufficient amounts of EGF during development, we wanted to establish an in vivo model with a state of EGF deficiency. This was attempted by induction of autoimmunity to EGF in rats. Twenty rats were immunized with EGF. Fifteen of these developed autoantibodies against EGF, which, as judged by Scatchard analysis, had a median apparent affinity constant of 14 x 10(9) L/mol and a median concentration of binding sites of

Hypothesis: dysregulation of immunologic balance resulting from hygiene and socioeconomic factors may influence the epidemiology and cause of glomerulonephritis worldwide.

Glomerular diseases show diverse epidemiological characteristics throughout the world, which has been suggested to be caused by differences in genetics of the underlying populations or environmental exposure to the putative antigens or agents that either trigger or induce the disease. Recently, an alteration in immune balance of the T helper 1 (T(H)1) and T helper 2 (T(H)2) subsets has been implicated as a mechanism to explain the relative increase in allergic diseases in industrialized nations. According to the Hygiene Hypothesis, overcrowding and poor hygiene early in life may protect from a

Immunologic studies on human factor VIII (anti-hemophilic factor A, AHF) components produced by low-ionic-strength dialysis.

Since dialysis of human factor VIII against buffers of low ionic strength yielded two distinct components, and since the factor VIII fraction isolated from normal plasma showed von Willebrand factor activity as defined by the corrective effect on abnormal platelet retention and ristocetin aggregation in von Willebrand's disease, the present studies were performed to determine if the correcting activities could be attributed to one or both of the components. Dialysis of factor VIII against buffers of low ionic strength led, however, to a decrease in factor VIII procoagulant activity and the red

Tolerance to factor VIII in the era of nonfactor therapies: immunologic perspectives and a systematic review of the literature.

Persons with hemophilia A lack clotting factor (F)VIII (FVIII) due to a genetic mutation in the F8 gene. The administration of FVIII concentrate leads to the development of neutralizing anti-FVIII antibodies (inhibitors) in about 30% of children with severe hemophilia A. The other 70% of children do not mount a detectable antibody response, suggesting that they may have developed tolerance toward FVIII. Our knowledge on the underlying immunologic mechanisms that determine formation of inhibitors or apparent tolerance to FVIII is limited. Up to recently, FVIII concentrates were regularly used a

No immunological changes after factor VIII product switch: An in depth analysis in haemophilia A patients.

BACKGROUND: A challenging complication in the treatment of haemophilia A is the formation of neutralizing anti-FVIII antibodies (inhibitors). There is ongoing debate on the effect of FVIII product and inhibitor risk, rendering patients and physicians reluctant to switch FVIII-products. AIM: This study aimed to evaluate changes in the immune profile of haemophilia A patients after switching FVIII products and their possible relation to inhibitor development. Secondary, FVIII efficacy after switching were assessed. METHODS: Patients, who switched FVIII-products between 2017-2019, were included

The role of immune cells and associated immunological factors in the immune response to spinal cord injury.

Spinal cord injury (SCI) is a devastating neurological condition prevalent worldwide. Where the pathological mechanisms underlying SCI are concerned, we can distinguish between primary injury caused by initial mechanical damage and secondary injury characterized by a series of biological responses, such as vascular dysfunction, oxidative stress, neurotransmitter toxicity, lipid peroxidation, and immune-inflammatory response. Secondary injury causes further tissue loss and dysfunction, and the immune response appears to be the key molecular mechanism affecting injured tissue regeneration and fu

Common Variable Immunodeficiency-Associated Cancers: The Role of Clinical Phenotypes, Immunological and Genetic Factors.

OBJECTIVE: The aim of this study was to investigate the prevalence of cancer and associating clinical, immunological, and genetic factors in a German cohort of patients with common variable immunodeficiency (CVID). METHODS: In this retrospective monocenter cohort study, we estimated the standardized incidence ratio (SIR) for different forms of cancer diagnosed in CVID patients. Furthermore, we evaluated the likely association of infectious and non-infectious CVID-related phenotypes with the diagnosis of cancer by calculation of the odds ratio. The genetic background of CVID in patients with c
