Actions needed to improve maternal health.

The health of mothers and their children is of critical importance, both as a reflection of the current health status of a large segment of the world's population and as a predictor of the health of the next generation. A range of indicators of maternal and neonatal health exist-those primarily affecting pregnant and postpartum women, and those affecting the health and survival of infants. Pregnancy outcome may be affected by toxicant exposure, maternal habits, occupational hazards, psychosocial factors, socioeconomic status, racial disparity, chronic stress, and infections. An increase in obs

Obsessive-compulsive symptoms, intrusive thoughts and depressive symptoms: a longitudinal study examining relation to maternal responsiveness.

BACKGROUND: The postpartum period is a vulnerable time for the development of depression. While perinatal depression has been well studied, intrusive thoughts related to the infant and classic obsessive-compulsive (OC) symptoms (e.g. chequering, ordering and cleaning) are also common in the postpartum and less well understood. OBJECTIVE: The present study investigated the associations among depressive symptoms, intrusive thoughts, and OC symptoms and their relation to the quality of the mother-infant relationship, particularly in the realm of maternal responsiveness. METHODS: Participants (N

Update on group B streptococcal infections: perinatal and neonatal periods.

Group B Streptococcus (GBS), one of the beta-Hemolytic streptococci, remains a leading cause of neonatal sepsis in the United States. The first consensus guidelines for the prevention of neonatal GBS disease were published in 1996, recommending intrapartum antibiotic prophylaxis on the basis of screening-based or risk-based strategies. Since then, there has been a 70% decrease in the rate of early-onset GBS disease. On the basis of evidence-validating superiority of this screening-based strategy, new national guidelines were released in 2002. Data from the Centers for Disease Control and Preve

Clinical assessment and treatment in paediatric wards in the north-east of the United Republic of Tanzania.

OBJECTIVE: We assessed paediatric care in the 13 public hospitals in the north-east of the United Republic of Tanzania to determine if diagnoses and treatments were consistent with current guidelines for care. METHODS: Data were collected over a five-day period in each site where paediatric outpatient consultations were observed, and a record of care was extracted from the case notes of children on the paediatric ward. Additional data were collected from inspection of ward supplies and hospital reports. FINDINGS: Of 1181 outpatient consultations, basic clinical signs were often not checked;

[Longitudinal study of the mother and child population in an urban region of southern Brazil, 1993: methodological aspects and preliminary results].

All babies born in the hospitals of the city of Pelotas, Brazil, in 1982 were studied soon after delivery and followed up prospectively during the first years of their lives. In 1993, this study was repeated with a similar methodology, with the aim of assessing eventual changes in the level of maternal and child health. All five maternity hospitals in the city were visited daily and the 5,304 babies born included in the study. They were weighed and measured, and their gestational age was assessed using the Dubowitz method. Their mothers were examined and interviewed regarding a large number of

Bottle-feeding legislation in Papua New Guinea.

Research in Papua New Guinea and elsewhere in the developing world has indicted the dangers of bottle feeding infants. Following a failure to obtain the voluntary agreement of shopkeepers to restrict sales of infant-feeding bottles and teats, legislation was passed which placed these items on prescription. In order to obtain a prescription certain conditions have to be satisfied. A follow-up survey which was carried out in Port Moresby two years after the introduction of legislation indicated a significant decline in the extent of bottle feeding.

Retaining infants and young children who experience transitions in care in longitudinal studies of child health and development: Considerations from the HEALthy Brain and Child Development study.

A transition in care (TIC) is a significant change in the primary adults who provide care for a child, involving a move to informal or formal non-parental care, including kinship and foster care. In this paper, we address three issues: (1) the theoretical and empirical reasons for retaining infants and children who experience TIC in longitudinal studies of child health and development; (2) the import of retaining infants and children who experience TIC in studies focusing on parental substance use; and (3) methodological strategies for following children with TIC. We discuss the HEALthy Brain

Incidence of the respiratory distress syndrome in infants of diabetic mothers--perinatal influences.

This survey of 73 infants of insulin dependent (ID) diabetic mothers and 70 infants of gestational diabetics (GD) born at King George V Memorial Hospital between January, 1968 and December, 1975, showed that all 12 babies with RDS were from the ID group. RDS was the cause of all 6 neonatal deaths and together with the 5 stillbirths gave this high risk group a 15% mortality rate. Since no deaths occurred in the GD group, the overall mortality was 8%. In the 6 years, 1970-1975, RDS occurred 21 times more often in babies of ID mothers than in control babies born to non-diabetics. Babies born to I

Theory of obstetrics: an epidemiologic framework for justifying medically indicated early delivery.

BACKGROUND: Modern obstetrics is faced with a serious paradox. Obstetric practice is becoming increasingly interventionist based on empirical evidence but without a theoretical basis for such intervention. Whereas obstetric models of perinatal death show that mortality declines exponentially with increasing gestational duration, temporal increases in medically indicated labour induction and cesarean delivery have resulted in rising rates of preterm birth and declining rates of postterm birth. Other problems include a disconnection between patterns of gestational age-specific growth restriction

Why are babies getting bigger? Temporal trends in fetal growth and its determinants.

OBJECTIVES: To describe temporal trends in fetal "growth" and to examine the roles of sociodemographic, anthropometric, and other determinants. STUDY DESIGN: Hospital-based cohort study of 61,437 nonmalformed singleton live births at 22 to 43 weeks' gestational age. Four main measures were examined: (1) birth weight, (2) birth weight-for-gestational-age Z score, (3) small-for-gestational-age (SGA), and (4) large-for-gestational age (LGA), with the latter 3 measures based on a recently developed population-based Canadian reference. Gestational age was based on the last normal menstrual period

Availability and utilization of obstetric and newborn care in Guinea: A national needs assessment.

OBJECTIVE: To assess the availability and utilization of emergency obstetric and neonatal care (EmONC) in Guinea given the high maternal and neonatal mortality rates. METHODS: We used the Guinea 2012 needs assessment data collected via a national cross-sectional census of health facilities conducted from September to October 2012. All public, private, and faith-based health facilities that performed at least one delivery during the period of the study were included. RESULTS: A total of 502 health facilities were visited, of which 81 were hospitals. Only 15 facilities were classified as fully

Moving beyond essential interventions for reduction of maternal mortality (the WHO Multicountry Survey on Maternal and Newborn Health): a cross-sectional study.

BACKGROUND: We report the main findings of the WHO Multicountry Survey on Maternal and Newborn Health (WHOMCS), which aimed to assess the burden of complications related to pregnancy, the coverage of key maternal health interventions, and use of the maternal severity index (MSI) in a global network of health facilities. METHODS: In our cross-sectional study, we included women attending health facilities in Africa, Asia, Latin America, and the Middle East that dealt with at least 1000 childbirths per year and had the capacity to provide caesarean section. We obtained data from analysis of hosp

Maternal and child health in Israel: building lives.

Israel is home to a child-oriented society that values strong family ties, universal child benefits, and free education for all children from 3 years of age to school grade 12. Alongside the universal health-care services that are guaranteed by the National Health Insurance Law and strong, community-based primary and preventive care services, these values have resulted in good maternal and child health. In 2015, infant and maternal mortality (3·1 deaths per 1000 livebirths and 2·0 deaths per 100 000 livebirths, respectively) were lower than the mean infant and maternal mortality of countries w

Territorial differences in infant mortality in Latvia in the first decade of the third millennium.

OBJECTIVE: Infant and child mortality are some of the most substantial indicators of country welfare. Infant mortality (IM) in Latvia is constantly the highest among 25 Member States of the European Union. Since the regaining of independence in 1991, IM has decreased by almost 50%, however, it is still high enough to cause concern that the country will not be able to meet the UN Millennium Development Goals to decrease IM in Latvia by 2015. The Medical Faculty at the University of Latvia has conducted several studies identifying correlations between IM and GDP, total expenditure on health, une

Hypertension in pregnancy. Pathophysiology and management.

Hypertension in pregnancy has implications for both maternal and fetal welfare. Extrapolation from concepts of mechanisms operating in hypertension in general to pregnancy-related hypertension is not justified. In the latter, the major features are a hyper-adrenergic state, plasma volume reduction and an increased systemic resistance. A reduction in uteroplacental perfusion may result from or may activate the mechanisms that elevate blood pressure. Humoral factors (e.g. hormonal attenuation of vascular reactivity) and prostacyclin deficiency may be central to the disordered physiology. Treatme

Trajectories of change in caregiver social supports: Associations with child maltreatment among families with child welfare involvement.

BACKGROUND: Evidence suggests that families with child welfare contact experience substantially limited access to social supports, a key protective factor against child maltreatment. OBJECTIVE: This study examined dynamic trajectories of caregivers' functional social supports (i.e., confidant, instrumental, and affective support) over time in relation to risk for maltreatment. METHODS: Participants were 707 caregivers of infants and toddlers with child welfare contact. Repeated-measures latent class analysis was used to profile trajectories of caregiver social supports across a five-year per

Perforation of a central venous catheter into the pelvis of the kidney: a rare complication.

Most adverse events related to central venous catheter (CVC) use in severely ill neonates who require total parenteral nutrition (TPN) are caused by malposition of the tip of the catheter, followed by complications such as infection, sepsis, thrombosis or, rarely, perforation of the vessel. Malpositioned tip of a CVC into the renal vein followed by thrombosis is well known. We report two cases of perforation of a CVC into the renal pelvis. To prevent this complication, the value of intra- or postoperative use of fluoroscopy with injection of contrast medium through the catheter is discussed.
