Chlamydial infections.

Infections due to Chlamydia are exceedingly important and common in emergency medicine, perhaps more so than any other single infectious agent. Recent studies have documented Chlamydia as a major cause of such commonly seen entities as urethritis, epididymitis, cervicitis, conjunctivitis, and neonatal pneumonia. Because clinical diagnosis and treatment is usually straightforward, and the already tremendous incidence of these "outpatient" infections is certain to increase, emergency physicians must become familiar with their diagnosis and management.

[Legionella infections].

The authors review the bacteriological features of Legionella infections, their clinical symptoms and the methods of diagnosis. They stress the unusual ecological features of Legionella which generally lives in natural water reservoirs or in artificial reservoirs (drinking water piping, air conditioning). In the clinical situation, current emphasis is on the extrapulmonary infections whose pathogenesis has not yet been fully explained. The incidence and prevalence of Legionella in pneumonia still needs to be defined. The methods of bacteriological and serological diagnosis are described as wel…

Obstetric infections.

Maternal infections are unique because of their implications for the outcome of pregnancy. Bacterial infections that can be favorably influenced by prompt diagnosis and preventive management include urinary tract infections and chorioamnionitis. However, viral infections such as genital herpes and cytomegalovirus, and the parasitic disease toxoplasmosis, require specialized forms of testing and a complicated management plan. Altered physiological and immunological states during pregnancy are important in understanding the pathophysiology of maternal infections.

Cytomegalovirus infections.

Cytomegalovirus infections are common throughout the world. Certain populations, including pregnant women and their fetuses, immunosuppressed patients, and recipients of large amounts of transfused blood, are at increased risk. Although the majority of infections in all groups of patients are clinically inapparent, variable symptoms, including fever, rash, pneumonitis, and hepatitis, can occur. The infected host develops antibodies against CMF, but frequently, despite this appropriate immune response, infection becomes chronic with prolonged excretion of virus. In some instances, a latent infe…

Maxillofacial infections.

Maxillofacial infections, either surgical or nonsurgical, are relatively infrequent, probably because of the excellent regional blood supply. Established infections are usually recognized early owing to the obvious signs of infection and the early symptoms. Fortunately, most infections respond to the penicillin group of antibiotics. However, spread of maxillofacial infections may be disastrous, not only locally and regionally but possibly even systemically. There is always a chance of a lethal complication from a maxillofacial infection.

Shigella infections.

Shigellosis is a complex disease involving two distinct organs and two distinctive clinical presentations of intestinal involvement. It is also associated with a wide variety of extra-intestinal manifestations. Because these may precede the onset of diarrhoea/dysentery, and confuse the diagnosis, it is safe to denote shigellosis as a protean clinical problem. The disease is readily spread by contact because so few organisms are required to establish infection. It may also be spread through contaminated food or water, related to either defective sewage or a human carrier, and explosive outbreak

[Salmonella infections].

Inspite of a high standard of hygiene, infections due to salmonellae are increasing in the highly developed countries. In the Federal Republic of Germany 36 000 cases were reported in 1976; the number of unreported cases is probably five times higher. Practical and clinical aspects make it necessary to differentiate between typhoidal, systemic infections and enteric salmonellosis, which normally remains localized in the gut. In the treatment of typhoid fever, co-trimoxazole seems to be a potent drug which can replace chloramphenicole. In cases of enteritis due to salmonellae the most important…
