Cauda equina syndrome (CES) is a surgical emergency requiring urgent spinal decompression. Our objective is to analyze the association between race and in-hospital outcomes (complications, mortality, length of stay, and discharge disposition) in patients with CES. This is a retrospective cohort study of CES patients identified via the Nationwide Inpatient Sample (NIS) from 2000 to 2010 in inpatient hospitalizations in the USA. The patient sample includes 46,017 patients ≥18 undergoing spine surgery for CES. Using ICD9-CM codes, patient records with a diagnosis of CES from 2003 to 2010 were sel

Disparities in access to care for surgical intervention in craniosynostosis have been suggested as a cause in discrepancies between the surgical approach and consequently perioperative outcomes following surgery. This work aimed to investigate the influence of race, insurance status, and the presence of craniosynostosis-related conditions on the short-term outcomes after the surgical management of craniosynostosis. Using the National Inpatient Sample database for the years 2010 to 2012, sociodemographic predictors for 30-day postoperative complication rates and requirements for blood transfusi

BACKGROUND: Prior studies have demonstrated that socioeconomic factors can influence decisions regarding care. The purpose of this study is to evaluate the impact of insurance type on the treatment of distal radius fractures.
METHODS: This was a retrospective study of patients in the New York Statewide Planning and Research Cooperative System database undergoing treatment for distal radius fractures. We stratified patients by insurance type, including private, Medicare, Medicaid, Workers' Compensation, self-pay, or other coverage (disability insurance, government programs, no charge, no payme

INTRODUCTION: Surgical outcomes and healthcare utilization have been shown to vary based on patient insurance status. We analyzed whether patients' insurance affects case urgency for and readmission after craniotomy for meningioma resection, using benign meningioma as a model system to minimize confounding from the disease-related characteristics of other neurosurgical pathologies.
METHODS: We analyzed 90-day readmission for patients who underwent resection of a benign meningioma in the Nationwide Readmission Database from 2014-2015.
RESULTS: A total of 9783 meningioma patients with private

PURPOSE: The Affordable Care Act's Medicaid expansion increased insurance coverage nationally; its impact on colorectal cancer surgery access and outcomes remains unclear, particularly in rural regions with structural barriers. This study evaluated whether Medicaid Expansion was associated with changes in colorectal cancer surgery utilization, access to elective surgery, and short-term outcomes within 8 Midwestern states.
METHODS: A population-based difference-in-differences analysis used State Inpatient Databases from 2010 to 2022, including 4 expansion and 4 late or nonexpansion states. Adu

BACKGROUND: Traumatic brain injury is a major public health concern with a rising incidence in the United States. Prior studies have looked at associations between insurance status and traumatic brain injury, but none have focused specifically on traumatic subdural hematomas (SDH). It is important to evaluate whether insurance and/or other social determinants of health play a role in treatment and outcomes of traumatic SDH.
METHODS: A retrospective analysis of the National Trauma Data Bank was conducted from 2012 to 2016 to look at associations between insurance status and management of SDH w

Although surgical and medical treatment options are available for the treatment of chronic lymphedema, there is no cure. Recent advances in microsurgery have provided an opportunity to perform immediate lymphatic reconstruction after lymphadenectomy for disease prevention. In this review, we provide the historical background leading to a paradigm shift in performing this procedure. We will also discuss the current evidence for immediate lymphatic reconstruction, potential oncologic procedures amenable to this approach, and detail ongoing challenges.

BACKGROUND: The number of annual endoscopic carpal tunnel releases (ECTR) has been increasing due to comparable outcomes with open carpal tunnel release (CTR). Many studies have demonstrated cost-saving measures with open CTR, but similar studies for ECTR are lacking.
PURPOSE: The purpose of this study was to analyze differences in the costs associated with ECTR under local anesthesia and monitored anesthesia care (MAC).
MATERIALS AND METHODS: We retrospectively compared the surgical costs of 844 patients undergoing isolated ECTR between 2018 and 2024. Patients undergoing isolated ECTR were

OBJECTIVE: We examined the relationship between socioeconomic status (SES) and features of cerebrospinal fluid (CSF) diversion at our academic center in a socially disadvantaged urban setting.
METHODS: We retrospectively analyzed 303 surgeries among 229 patients (0.01-89 years), grouped according to median household income for zip-code (i.e., <$40,000, $40,000-$70,000, $70,000-$100,000, and >$100,000) and insurance type.
RESULTS: The median age at surgery was significantly younger for publicly insured patients compared to privately insured patients, despite the 65-year age requirement for Me

Lung cancer is the most common cause of cancer-related mortality worldwide. With the introduction of low-dose computed tomography (LDCT), detection of adenocarcinoma in situ (AIS) and minimally invasive adenocarcinoma (MIA) are increasingly being found in young, never-smoking females in Asia. There are survival studies suggesting that these early cancers, with resection, have no recurrence in 5 and even 10 years and therefore might be considered cured, allowing for favorable underwriting consideration for life insurance. However, other studies have shown incidences of secondary primary lung ca

BACKGROUND/OBJECTIVE: Although uncommon, invasive meningococcal disease (IMD) is a serious and life-threatening illness. In the United States (US), incidence is highest among infants aged <1 year, but research characterizing pediatric IMD burden is limited. This study aimed to evaluate health outcomes and economic burden (healthcare resource utilization [HCRU] and costs [associated with health outcomes and HCRU]) in US individuals aged <11 years with IMD and without IMD, and their caregivers, from the patient and payer (commercial and Medicaid insurance) perspective.
METHODS: Retrospective cl

CONTEXT: This article chronicles the political history of efforts by the U.S. Congress to enact a law requiring "parity" for mental health and addiction benefits and medical/surgical benefits in private health insurance. The goal of the Paul Wellstone and Pete Domenici Mental Health Parity and Addiction Equity (MHPAE) Act of 2008 is to eliminate differences in insurance coverage for behavioral health. Mental health and addiction treatment advocates have long viewed parity as a means of increasing fairness in the insurance market, whereas employers and insurers have opposed it because of concer

BACKGROUND: Disparities in thyroid cancer care are driven by demographic and socioeconomic factors such as insurance status, income, and access to specialized care.
METHODS: Retrospective chart review. Demographic and clinical data from 281 adult patients who underwent thyroidectomy or hemithyroidectomy for papillary or follicular thyroid cancer at a single institution from 2015 to 2023. We evaluated the effect of demographic variables with stage at diagnosis, tumor size, and time to surgery. We assessed predictors of tumor size > 2 cm, advanced cancer stage (stage III+), and surgical delay >

Since 2014, public and private insurance coverage for transgender Americans' surgical care has increased exponentially. Training clinicians and equipping institutions to meet the surge in demand has not been as rapid. Through ethnographic research at a surgical workshop focused on trans- genital reconstruction and in a U.S. hospital working to grow its transgender health program, this article shows that effects of the decades-long insurance exclusion of trans- surgery are not easily remedied through the recent event of its inclusion because patient access is not the only thing that has been re

Introduction. Despite the amount of orthopaedic research evaluating access to care based on insurance status, no study quantifies the effects of insurance status on the care of acute Achilles tendon ruptures. Methods. Using Current Procedural Terminology codes, we identified all patients who underwent surgical management of Achilles tendon rupture between December 31, 2013, and December 31, 2018, and followed-up at either a county hospital-based orthopaedic surgery clinic and/or private university-based clinic. Inclusion criteria included patients who (1) underwent surgical management of an Ac

BACKGROUND: Extensive literature documents the adverse sequelae of delayed diagnosis of slipped capital femoral epiphysis (SCFE), including worsening deformity and surgical complications. Less is known about predictors of delayed diagnosis of SCFE, particularly the effects of social determinants of health. The purpose of this study was to evaluate the impact of insurance type, family structure, and neighborhood-level socioeconomic vulnerability on the delay of SCFE diagnosis.
METHODS: We reviewed medical records of patients who underwent surgical fixation for stable SCFE at a tertiary pediatr

BACKGROUND: Reinterventions after congenital heart disease surgery include not only reoperations but also medical catheter interventions, and the details of these treatment realities are often unclear. This study aimed to elucidate the medical and surgical reinterventions and associated medical costs after the tetralogy of Fallot (TOF) repair using Japanese health insurance claims data.
METHODS AND RESULTS: We analyzed reinterventions and medical costs from insurance claims data of patients who underwent TOF repair between 2005 and 2021. Of 174 patients who underwent TOF repair, 23 (13.2%) re

Craniosynostosis is the premature fusion of the skull. There are two forms of treatment: open surgery and minimally invasive endoscope-assisted suturectomy. Candidates for endoscopic treatment are less than 6 months of age. The techniques are equally effective; however, endoscopic surgery is associated with less blood loss, minimal tissue disruption, shorter operative time, and shorter hospitalization. In this study, the authors aimed to evaluate the impact of race/ethnicity and insurance status on age of presentation/surgery in children with craniosynostosis to highlight potential disparities
