Stent stoma: Endoscopic stent insertion for refractory small intestine fistulas.

BACKGROUND AND AIMS: The therapeutic management of fistulas presents significant challenges, often involving both conservative and surgical approaches. Despite these interventions, recurrence and postoperative mortality rates remain high. Endoscopic stent insertion into the fistula, along with the creation of a stent stoma, may offer a promising alternative for patients who fail surgical or conservative therapies. This study aimed to evaluate the feasibility, effectiveness, and safety of endoscopic stent insertion in the treatment of refractory small intestinal fistulas. METHODS: Patients wit…

[Rhabdomyolysis complicated with a fistula between ileal neobladder and intestine: a case report].

A 79-year-old male with an ileal neobladder was hospitalized with the chief complaints of fever, dark colored urine and systemic muscle pain. The clinical diagnosis was rhabdomyolysis complicated with hypokalemia, which was caused by chronic diarrhea due to a fistula between ileal neobladder and intestine. The patient was treated successfully with efficient drip infusion.

[Use of ferromagnetic rheological suspensions in the treatment of non-formed intestinal fistulas].

Data on treatment of 42 patients with nonformed intestinal fistulas with the help of ferromagnetic rheological suspensions are presented. The authors have shown that their method tested under experimental conditions is close to universal since it does not require selection of the individual obturator. The use of the method is not possible in patients with one or multiple abscesses of the abdominal cavity, through which the formed fistula passes.

[Treatment of a small intestine-vaginal fistula by means of Paragalalith (tissue adhesive)].

The Paragalalith method, to dry off a fistula proved to be a complete success, when a complex vagina/small intestine fistula was removed from a 75 year old patient (woman). We had developed the method previously in our clinic and used it sucessfully to close 30 fistulas in the abdominal region.

Secondary aortoduodenal fistula complicating aortic grafting, as a cause of intermittent chronic intestinal bleeding.

Intermittent intestinal bleeding persisted in a 77-year-old male, who had undergone grafting for abdominal aortic aneurysm. Each attack lasted for a few weeks and spontaneously resolved. Only a minute abnormality was found in the third portion of the duodenum; barium studies showed a segmental narrowing, but endoscopy disclosed only a small erosion in that portion. Massive and fatal gastrointestinal hemorrhage broke out 6 months after the onset of bleeding. Autopsy revealed an adhesion area with a small fistula formation between the duodenum and aorta. Even slight endoscopic findings should be

Right common iliac arterio-intestinal fistula caused by tubercular peritonitis: report of a case.

A 59-year-old female was admitted with massive hematemesis and melena. A hematological examination revealed that the red blood cell count was 1.31 x 10(6)/mm3, Hb 3.4 g/dl, and Hct 12%. No source of bleeding was found by an emergency endoscopic examination of the esophagus, stomach and duodenum, or by superior mesenteric angiography. At laparotomy a right common iliac arterio-intestinal fistula was found. The microscopic examination of this part of the ileum, including the fistula, revealed the presence of tubercular peritonitis. An extra-anatomic bypass graft using a prosthetic graft was perf…

Incidental Appendico-Ileal Fistula in a Patient With Adhesive Intestinal Obstruction: A Case Report.

A fistulous communication between the appendix and any viscus is rare. Such fistula is most often acquired due to recurrent appendicitis, cystic fibrosis, Crohn's disease, tuberculosis, and malignancy. Here in, we report a rare case of an appendico-ileal fistula incidentally detected during laparotomy for adhesive small bowel obstruction. The fistula was divided, the ileal opening was sutured, and appendectomy was performed. Postoperative recovery was uneventful, with no evidence of malignancy, tuberculosis, or inflammatory bowel disease on the histopathological examination of the appendix.

Gastrointestinal bleeding as a result of entero-iliac fistula due to intestinal foreign body.

Ingested toothpicks are a relatively rare event, but they may cause serious gut injuries and can be listed among rare causes of perforation, peritonitis, sepsis or death. Unless the foreign bodies were intentionally swallowed, many patients who ingested them fail to remember the event and they do not refer it during the medical history collection; this makes diagnosis problematic. In this work, a case of perforation of the sigmoid colon is described, caused by a toothpick ingestion. The patient had to be surgically treated because of a complication: the formation of an entero-iliac fistula wit

Laparoscopically assisted ileocecal resection for Crohn's disease associated with intestinal stenosis and ileovesical fistula.

We describe a 22-year-old man with Crohn's ileocolitis accompanied by intestinal stenosis and ileovesical fistula in whom laparoscopically-assisted surgery was successfully performed after thorough nutritional therapy. Laparoscopic procedures are characterized by minimal access and minimal invasion, features which can contribute to the early recovery of patients who undergo surgery. It is suggested that laparoscopic (or laparoscopically-assisted) surgery after strict nutritional therapy can be effective in the treatment of patients with Crohn's disease who have intestinal complications.

[Immediate complications of radical cystectomy. Intestinal fistula: our experience in the treatment with somatostatin].

During the period of time between November 1986 and May 1988, 14 total cystectomies with one-step urinary derivations were performed in our Service. The surgical techniques utilized include: 7 Bricker, 2 Camey, 1 ileocecal segment derivation, 2 cutaneous transureterostomies and 2 cutaneous ureterostomies. Among the complications observed there were 3 intestinal fistulas which were treated at first in a conservative way with parenteral nutrition and somatostatin. An immediate response was obtained in all three cases with a decrease in the fistula deficit. Surgical treatment was required in two …
