Choroidal folds associated with increased intracranial pressure.

PURPOSE: To determine whether a relationship exists between increased intracranial pressure and the presence of idiopathic choroidal folds. METHODS: A prospective study in which 12 consecutive patients presenting with choroidal folds were evaluated by imaging studies (ultrasonography, magnetic resonance imaging, and/or computed tomography) to rule out known causes of choroidal folds, such as orbital disease, choroidal tumor, posterior scleritis, hypotony, and choroidal neovascular membrane. A lumbar puncture was performed on each of these patients, and measurement of opening pressure of cereb…

High-dose barbiturates for intracranial pressure control.

Approximately 10% of severely head-injured patients will manifest elevated intracranial pressure uncontrollable by any currently known medical or surgical treatments, with a resulting mortality of > 90%. High-dose barbiturate therapy used in this setting has been shown to control intracranial pressure and reduce mortality in a small but definite number of patients. The pharmacologic, physiologic, and systemic effects of high-dose barbiturates must be fully appreciated, and appropriate clinical safeguards must be established when this therapy is implemented after severe head injury.

Symptomatic low intracranial pressure in shunted hydrocephalus.

Fourteen patients with ventricular cerebrospinal fluid shunts in place for chronic hydrocephalus presented with a history and neurological deficits usually associated with high intracranial pressure (ICP) caused by an obstructed shunt system. However, the symptoms were characteristically present when the patient was upright and active, and were usually relieved by lying down. The symptoms of intermittent headache, nausea, emesis, lethargy, and diplopia were associated with paresis of upward gaze or minimal strabismus. Measurement of ICP showed unexpected dramatically low levels with a marked d…

Intracranial pressure monitoring in severe hypertensive encephalopathy.

Treatment of arterial hypertension in severe hypertensive encephalopathy does not always result in clinical improvement in the patient's central nervous system. In order to elucidate further the status of the brain, the authors have measured intracranial pressure (ICP) and arterial pressure in three cases of severe hypertensive encephalopathy. ICP was elevated in two of the three cases with peak values ranging from 32-70 mm Hg. In these 2 patients, therapy to lower ICP, including hyperventilation, steroids, barbiturates, and furosemide was begun early in the course. Cerebral perfusion pressure…

Relationship between intracranial pressure and phase contrast cine MRI derived measures of intracranial pulsations in idiopathic normal pressure hydrocephalus.

Phase contrast cine MRI with determination of pulsatile aqueductal cerebrospinal fluid (CSF) stroke volume and flow velocity has been suggested to assess intracranial pulsations in idiopathic normal pressure hydrocephalus (iNPH). We aimed to compare this non-invasive measure of pulsations to intracranial pressure (ICP) pulse wave amplitude from continuous ICP monitoring. We hypothesised that a significant correlation between these two markers of intracranial pulsations exists. Fifteen patients with suspected iNPH had continuous computerised ICP monitoring with calculation of mean ICP pulse wav…

Can chronic increased intracranial pressure or exposure to repetitive intermittent intracranial pressure elevations raise your risk for Alzheimer's disease?

Over a decade ago, I formulated the hypothesis that cumulative effects of exposure to high intracranial pressure (ICP) may contribute to the development of Alzheimer's disease (AD), though not necessarily in an exclusive way. In addition to individual ICP characteristics (high 'physiological' ICP) and diseases causing ICP elevation, various activities with significant Valsalva effort, such as weightlifting and wind instrument playing, can generate very high ICPs. Recent studies of normal-pressure hydrocephalus (NPH), glaucoma and Alzheimer's disease provide supportive evidence for this hypothe…

Extrapolation to zero-flow pressure in cerebral arteries to estimate intracranial pressure.

BACKGROUND: Cerebral perfusion pressure (CPP) is commonly calculated from the difference between arterial blood pressure (AP) and intracranial pressure (ICP). ICP can be considered the effective downstream pressure of the cerebral circulation. Consequently, cerebral circulatory arrest would occur when AP equals ICP. Estimation of AP for zero-flow pressure (ZFP) may thus allow estimation of ICP. We estimated ZFP from cerebral pressure-flow velocity relationships so that ICP could be measured by transcranial Doppler sonography. METHODS: We studied 20 mechanically ventilated patients with severe…

Effects of fentanyl on intracranial pressure and cerebral perfusion pressure during hypocapnia.

Ten patients presenting for carniotomy were studied. Each was anaesthetized with thiopentone or Althesin followed by tubocurarine and the lungs were hyperventilated with nitrous oxide in oxygen. Fentanyl 0.2 mg was administered i.v. and the intracranial pressure (i.c.p.) and mean arterial pressure were recorded continuously for 10 min. At the time of administration of fentanyl nine of the 10 patients were hypocapnic (PaCO2 less than 4 kPa). The changes in i.c.p. were small. Cerebral perfusion pressures less than 50 mm Hg were observed in two patients who had moderate hypotension before the dru…

Retinal microvascular and structural changes in intracranial hypertension patients correlate with intracranial pressure.

AIMS: We aimed to evaluate the retinal microvascular and structural changes in intracranial hypertension (IH) patients compared with an age- and sex-matched control group. We also investigated the association between clinical parameters and retinal changes in IH patients. METHODS: Intracranial hypertension patients were divided into eyes with papilledema (IH-P) and eyes without papilledema (IH-WP). IH patients underwent lumbar puncture to measure intracranial pressure (ICP); visual acuity was performed using the Snellen chart. Optical coherence tomography (OCT) was used to image and measure t…

Telemetric intracranial pressure monitoring: our experience with 22 patients investigated for intracranial hypertension.

INTRODUCTION: We present the application of the Raumedic® P-tel telemetric device that monitors Intracranial Pressure (ICP) over long periods, in 22 patients, with suspected intracranial hypertension. METHODS: A telemetric device (Raumedic®, Neurovent® P-tel) was surgically implanted in 22 patients aged between 21 and 65 years. Among the patients, the inconclusive diagnosis of benign intracranial hypertension was set in 10, the possible diagnosis of postoperative hydrocephalus in 3, and the possible diagnosis of aqueduct stenosis in 2. Additionally, shunt malfunction and Normal Pressure Hydro…

Intracranial volume versus static and pulsatile intracranial pressure values in children with craniosynostosis.

OBJECTIVE: Reduced intracranial volume (ICV) and raised intracranial pressure (ICP) are assumed to be principal pathophysiological mechanisms in childhood craniosynostosis. This study examined the association between ICV and ICP and whether ICV can be used to estimate the ICP. METHODS: The authors analyzed ICV and ICP measurements from children with craniosynostosis without concurrent hydrocephalus and from age-matched individuals without craniosynostosis who underwent diagnostic ICP measurement. RESULTS: The study included 19 children with craniosynostosis (mean age 2.2 ± 1.9 years) and 12 …

Managing children with raised intracranial pressure: part two (brain tumours and intracranial bleeds).

This article is the second of two examining the causes of increased intracranial pressure in children. Key features and management associated with brain tumours and intracranial bleeds are highlighted. The conditions are accompanied by illustrative case studies to give an idea of what children's nurses may encounter in a patient presenting with raised intracranial pressure. Part one, published in December 2013, focused on the signs and symptoms of raised intracranial pressure and meningitis.

Diagnostic value of optical coherence tomography for intracranial pressure in idiopathic intracranial hypertension.

BACKGROUND: Idiopathic intracranial hypertension (IIH) is a condition of raised intracranial pressure (ICP) in the absence of space-occupying lesions or other known etiology. It primarily affects young obese females, and potentially causes permanent visual loss due to papilledema and secondary optic atrophy. The aim of this study was to evaluate the diagnostic value of optical coherence tomography (OCT) as a marker for CSF opening pressure in patients with idiopathic intracranial hypertension (IIH). METHODS: We conducted a case-control study of 20 newly diagnosed, 21 long-term IIH patients, a…

Relationships between dopamine infusions and intracranial hemodynamics in patients with raised intracranial pressure.

Dopamine, 1-10 micrograms/kg body weight/min was infused in 6 patients suffering from cerebrovascular diseases with elevated intracranial pressure and a critical cerebral perfusion pressure. Dopamine decreased intracranial pressure in 3 and increased it moderately in the other 3 patients. In all patients, the dopamine-induced rise of mean arterial pressure led to an increase of cerebral perfusion pressure. Transcranial Doppler ultrasonographic recordings of the middle cerebral artery in patients whose intracranial pressure declined revealed a decrease of the pathologically elevated cerebrovasc…
