Dislocated intrauterine devices: clinical presentations, diagnosis and management.

OBJECTIVE: Intrauterine devices (IUDs) are globally one of the most popular methods of contraception. Uterine perforation is one of the most significant complications of IUD use and commonly occurs at the time of IUD insertion rather than presenting as delayed migration. This paper reports a series of 13 cases of displaced IUDs requiring retrieval by laparoscopy or laparotomy. All the IUDs were copper bearing and most perforations occurred immediately after IUD insertion. CASES: In two patients with sigmoid colon injury and IUD penetration of the appendix, laparoscopic management had failed a

Laparoscopic retrieval of a levonorgestrel intrauterine device in a patient presenting with abdominal pain.

Uterine perforation is a potentially serious uncommon complication of intrauterine device (IUD) use. It has an incidence of one in 1000 insertions. It can manifest with abnormal uterine bleeding and pain. The most common site of implantation is the omentum. Risk factors include breastfeeding, <6 months postpartum, lack of experience of the healthcare professional, congenital uterine anomalies and nulliparity. Transvaginal ultrasound scan (USS) is accurate in localisation of the Mirena IUD. If the IUD is not evident within the uterus, an abdominal radiograph should be performed. Laparoscopic su

Transmural migration and perforation of a levonorgestrel intrauterine system: a case report and review of the literature.

INTRODUCTION: Uterine perforation is an uncommon yet well-known complication of copper intrauterine devices and the levonorgestrel intrauterine system (IUS). While initial extrauterine placement at the time of insertion is felt to be the cause of perforation in most cases, some hypothesize that delayed transmural migration and subsequent perforation can occur with slightly malpositioned or even properly placed devices. CASE: A 46-year-old female had a 52-mg levonorgestrel IUS inserted for menstrual management and contraception. We arranged a follow-up ultrasound as the uterus was enlarged on

Intrauterine contraceptive device with rectal perforation and strings presenting at the anus.

A 36-year-old grand multiparous midwife presented to the gynaecological unit of the University of Uyo Teaching Hospital with the string of an intrauterine contraceptive device (IUCD) protruding from her anus. She had had a copper IUCD inserted at a health centre 8 years earlier. However, 4 months later she noticed that she no longer felt its strings and had another IUCD inserted in the same facility when the first device was not located. Four months prior to presentation, she presented to a private clinic when she no longer felt the strings of the second IUCD. The device in the uterus was remo

Twelve-month outcomes of rotated copper intrauterine devices after immediate postpartum insertion.

OBJECTIVE(S): To describe clinical outcomes of copper intrauterine devices (Cu-IUDs) inserted immediately postpartum that subsequently assumed a rotated position. STUDY DESIGN: Secondary analysis of a postpartum IUD cohort using medical record data from women with rotated Cu T380A IUDs identified by transvaginal ultrasound 30-120 days after delivery who elected to maintain the device and were followed for 12 months. RESULTS: Among 2105 insertions, 1813 had follow-up data available. Ninety-three devices (5.1%) were rotated. Of these, 73 asymptomatic women elected to maintain the device and we

Erosion of an intrauterine contraceptive device through the bladder wall causing calculus: management and review of the literature.

Intrauterine contraceptive devices have been in use for more than 30 years. Although perforation of the uterus by an intrauterine device is not uncommon, intravesical migration with secondary stone formation is a rare complication. A 46-year-old women is described in whom intravesical migration of the intrauterine device was complicated by bladder stone formation. Endoscopic management was applied with an excellent outcome.

The role of multimodality radiological imaging in extrauterine misplaced IUCD: A case report.

Intrauterine contraceptive device (IUCD) is among the commonly used contraceptive methods in the world including sub-Saharan Africa but have been associated with a number of complications that although rare, can occur. Uterine perforation is the most serious complication of the IUCD. Surgical management is required in cases with complete uterine perforation, in which the IUCD is partially or completely within the peritoneal cavity. We present a case of a 27year old para 2 woman reporting with mild lower abdominal pains and the absence of IUCD string in the vagina after its insertion following

An unusual cause of vesical stone: a migrant intrauterine device.

Intrauterine contraceptives devices have been in use for many years. Although perforation of the uterus by an intrauterine device is not uncommon, intravesical migration with secondary stone formation is a rare complication. We report on a 32-year-old woman in whom an intrauterine contraceptive device (Copper T) migrated from her uterus into the bladder. She had lower urinary tract symptoms for nearly 2 years but sought no treatment. The onset of hematuria and fear of cancer prompted her to seek treatment. On investigation, a vesical stone was discovered that had formed on a copper T contracep

Persistent subumbilical discharge associated with actinomycosis caused by intrauterine contraceptive device: a case report.

Intrauterine devices are a very effective form of contraception used worldwide for more than 2 centuries. One of the serious, but rare, complications of intrauterine contraception is perforation through the uterine wall into the pelvic or abdominal cavity. In this report we describe, persistent periumbilical discharge associated actinomycosis caused by migration of a copper intrauterine contraceptive device. To our knowledge, this is the first report of persistent periumbilical discharge caused by migration of a copper intrauterine contraceptive device. We recommend consideration of this minor

[The 'lost' intra-uterine contraceptive device (author's transl)].

The authors report five cases of lost intra-uterine devices, which means that the threads were not visible at the external os of the cervix. This gives them an opportunity to review the literature and to conclude the following: 1. Ultra-sound is the treatment of choice to find the position of the IUD that has been lost, whether is is intra- or extra-uterine. 2. In doubtful cases an antero-posterior and a lateral hysterogram makes it possible to determine the relative position of the IUD to the uterine cavity. Laparoscopy gives useful complementary information. 3. If the IUD is intra-uterine, e

A novel hysteroscopic hook for extracting retained contraceptive intrauterine devices under direct vision.

OBJECTIVE: The study evaluated the efficacy of removing retained intrauterine devices (IUDs) under direct vision using a novel hysteroscopic hook. METHODS: In a retrospective observational study, 83 patients (group 1) underwent IUD extraction using a hysteroscopic IUD removal hook (HIRH) and 60 patients (group 2) underwent traditional hysteroscopic IUD extraction. We recorded the blood loss, operation time and success rate. RESULTS: The operation time was shorter (10.7 vs. 17.7 min; p < 0.001) and the success rate higher in group 1 compared with group 2 (odds ratio 1.09; p = 0.027). CONCLUS
