Three related myopic individuals with primary angle closure glaucoma are reported. They had true myopia and not pseudomyopia secondary to increased lenticular index of refraction. We believe one of these individuals (-8.62 spherical equivalent) to have the most myopic case of primary angle closure glaucoma reported in the literature. Although myopia is associated with anatomical factors that offer considerable protection from primary angle closure glaucoma, its presence does not eliminate the possibility of this disease. Laser iridectomy was effective in the treatment of these patients.

Nine cases are presented, six of them in detail, which suggest that there is a high risk of bullous keratopathy developing when the lens is removed for the treatment of acute glaucoma due to an intumescent cataract. In some cases the keratopathy is due to detachment of Descemet's membrane from the cornea. Acute glaucoma due to an intumescent cataract should be treated medically or by peripheral iridectomy. The lens should be removed only after the intraocular pressure has been normal and the cornea free of edema for at least 6 weeks.

Iris specimens obtained during the peripheral iridectomy in 12 cases (18 eyes) of primary angle closure glaucoma were studied histologically. The iris specimens taken during the senile cataract extraction in otherwise normal eyes were used as controls. The remarkable differences of the two groups are the changes of the quality and quantity of the mitochondria in the cells of iris tissue. The changes in primary glaucoma, especially in the intermittent stage of the angle closure glaucoma, were found in the mitochondria of the cells.

Primary angle-closure glaucoma is a major cause of blindness worldwide. It is a disease of ocular anatomy that is related to pupillary-block and angle-crowding mechanisms of filtration angle closure. A peripheral iridectomy specimen which included a portion of the ciliary body from an advanced lepromatous leprosy patient was studied histopathologically. The lepromatous granuloma in the iris was similar in content and appearance to that of skin lesions. Clinical examination of 113 patients in South Korea with lepromatous leprosy and severe visual impairment showed that the main cause of visual loss was the combined effect of corneal and lens opacities associated with small nonreacting pupils and iris atrophy. Cataract surgery with broad iridectomy and inferior sphincterotomy offers these patients with chronic lepromatous complications the best chance of preserving vision. Eighty-one cataract operations were performed under local anaesthesia, and in 90% vision improved; in 60% this improvement was 2 Snellen's lines or more.

A 67-year-old man had vitreous wick syndrome after an uncomplicated intracapsular cataract extraction with a sector iridectomy in the left eye. The intraocular infection cleared with antibiotic therapy including intravitreal antibiotic injection, but a residual anterior cyclitic membrane and shallow anterior chamber persisted. An anterior vitrectomy was performed later in the course of treatment after the intraocular infection subsided, to relieve the shallow anterior chamber, prolonged hypotony, and ciliary body detachment. Final visual acuity was 6/15+(20/50+) in the left eye.

Secondary anterior chamber implantation has become relatively simple since the advent of viscoelastic materials. Still, glaucoma, cystoid macular edema, endophthalmitis, and astigmatism remain vision-threatening complications. We studied intraocular pressures (IOPs) following this surgery in 102 patients (124 eyes) over 6 years. Elevated IOP was noted in 32 eyes (25.8%), but only 14 (11.3%) needed long-term medical treatment. None, however, required laser iridectomy, trabeculoplasty, or trabeculectomy.
