Self-medication, over-the-counter medication, and injudicious use of eye drops are not uncommon in our society. Use of such over-the-counter drugs without proper knowledge may be a convenient or cheaper alternative in some scenarios but can have serious implications. With this case series, we intend to highlight one of the risks associated with inappropriate steroid use. We describe the case of three patients who presented with Fusarium keratitis after an episode of conjunctivitis and prolonged use of steroids. All three patients had varying outcomes, ranging from successful medical management…

Nowadays, keratitis, corneal infection due to wearing contact lens means an increasingly serious problem. Neglected cases may lead to corneal damage that can cause blindness in cases of otherwise healthy eyes. Early diagnosis based on the clinical picture and the typical patient history is an important way of prevention. Prophylaxis is substantial to avoid bacterial and viral infection that is highly essential in this group of diseases. Teaching contact lens wearers the proper contact lens care, storage, sterility, and hygiene regulations is of great importance. In case of corneal inflammation…

Metarrhizium anisopliae (M. anisopliae) is an entomopathogenic fungus used as an agricultural pesticide in many countries, including the United States. M. anisopliae grows optimally at 25°C and is therefore considered safe to humans. We report a case of a 12-year-old girl who is a soft contact lens wearer with M. anisopliae corneal ulcer who was treated effectively with topical natamycin after identification of the organism by corneal culture. Topical amphotericin B was not effective.

PURPOSE: To report cases of microbial keratitis after LASIK from a tropical climatic zone.
METHODS: This retrospective case series included 17 eyes of 15 patients managed at our center from January 1999 to December 2007. All patients had a detailed clinical evaluation followed by corneal scrapings. The material obtained on scraping was subjected to detailed microbiology evaluation. Initial medical treatment was based on microbiology results and included irrigation of the interface with appropriate antimicrobial solutions. The flap was amputated in patients who developed flap necrosis. N-butyl…

PURPOSE: To report the clinical course, microbiologic findings, treatment, and outcomes of overnight orthokeratology associated microbial keratitis.
METHODS: Four cases of overnight orthokeratology associated microbial keratitis are reported.
RESULTS: Four patients aged 14-23 years (mean 18 years) who had central or paracentral corneal ulcers were included. Visual acuity at presentation ranged from 20/30 to hand motion. In all cases Pseudomonas aeruginosa was cultured from corneal scrapings or storage solution. In all patients the infection resolved with intensive topical antimicrobial treat…

PURPOSE: To report a case of fungal keratitis of the eye caused by Pichia anomala in a patient with systemic lupus erythematous and Stevens-Johnson syndrome.
METHODS: This was a retrospective chart review.
RESULTS: A 50-year-old woman with systemic lupus erythematosus presented with ocular pain of 4-day duration. Culture of corneal scrapings was positive for P. anomala. Clinically, the organism appeared to respond to topical natamycin, amphotericine B, and oral itraconazole.
CONCLUSIONS: A rare case of P. anomala-associated keratitis was successfully treated with topical amphotericin B, nat…

The onset of peripheral ulcerative keratitis in the course of a connective tissue disorder, such as rheumatoid arthritis, relapsing polychondritis, or systemic lupus erythematosus, may reflect the presence of potentially lethal systemic vasculitis. Moreover, peripheral ulcerative keratitis may be the first sign of systemic necrotizing vasculitis in patients with Wegener's granulomatosis, polyarteritis nodosa, microscopic polyangiitis, or Churg-Strauss syndrome. Although the exact pathogenesis of this severe corneal inflammation and destruction is not well understood, evidence points to a dysfu…

PURPOSE: To differentiate the clinical features of fungal keratitis with predominantly deep stromal and endothelial (FSKE) involvement and HSV Endothelial Keratitis (HEK).
METHODS: Medical records of patients diagnosed with FSKE or HEK between January 2019 and March 2024 were reviewed. We included cases presenting with predominant posterior stromal infiltrates and endothelial exudates that were later confirmed as fungal etiology by microbiology or confocal imaging. These were compared with confirmed HSV endothelial keratitis, diagnosed clinically and/or by PCR (Polymerase Chain Reaction).

PURPOSE: We present the clinical and histopathological findings of a geographically unique Lasiodiplodia theobromae fungal keratitis case in North Carolina. L. theobromae is a rare cause of fungal keratitis, and all but one of the 51 previously reported cases have occurred in patients living in the tropics.
OBSERVATIONS: A man in his early 50s developed L. theobromae keratitis after being struck in the left eye by a piece of debris while using a flexible-cord weed trimmer. Intracapsular lensectomy and penetrating keratoplasty were necessary when initial antimicrobial therapy was ineffective. …

AIM: To investigate the frequency, outcomes, and risk factors for acanthamoeba keratitis (AK) in England during the past 4 years.
METHODS: An ophthalmologist in 12 of the 14 regional health authorities (RHAs) coordinated identification of patients in their region presenting with AK between 1 October 1992 and 30 September 1996. Clinical and postal patient questionnaire data were analysed.
RESULTS: 243 patients (259 eyes) with an AK diagnosis were identified, equating to an annualised incidence of 0.14 per 100,000 individuals. UK resident patients for each year numbered 50, 71, 73, and 32 resp…

PURPOSE: To differentiate Pythium keratitis from fungal keratitis using clinical signs, to explore usefulness of various signs as diagnostic prognosticators, and develop a clinical scoring system.
METHODS: A retrospective review of medical records and archived clinical photographs of patients with culture-positive Pythium keratitis and hyaline filamentous fungal keratitis was conducted at a tertiary eye institute to explore characteristics of ulcers that may aid diagnosis.
RESULTS: Full-thickness corneal stromal keratitis (P = 0.055), a dry ulcer surface (P = 0.010), tentacles (P < 0.0001), …
