Independent predictors of cesarean delivery in women with diabetes.

OBJECTIVES: To identify independent risk factors for Cesarean delivery in women with pregnancy complicated by diabetes. METHODS: Retrospective analysis of pregnancies from 5735 diabetic women delivering liveborn infants. Maternal demographic, medical, obstetric historical factors and index pregnancy obstetric, glycemic and neonatal outcome parameters were evaluated for association with Cesarean delivery after a trial of labor. Individual risk factors were analyzed for association by chi2 and ANOVA. Independent predictors of Cesarean delivery and adjusted relative risk (RR) were identified by

Less-Restrictive Food Intake During Labor in Low-Risk Singleton Pregnancies: A Systematic Review and Meta-analysis.

OBJECTIVE: To evaluate benefits and harms of food intake during labor. DATA SOURCES: Electronic databases such as MEDLINE and ClinicalTrials.gov were searched from their inception until October 2016. METHODS OF STUDY SELECTION: We included randomized trials comparing a policy of less-restrictive food intake with a policy of more restrictive food intake during labor. The primary outcome was the mean duration of labor. Meta-analysis was performed using the random-effects model of DerSimonian and Laird to produce summary treatment effects in terms of either a relative risk or a mean difference

Perceived Barriers Among Midwives to Implementation of the Modified Partograph in a Public Hospital in Gauteng Province, South Africa.

INTRODUCTION: Partograph monitoring aims to assist midwives, obstetricians, and other healthcare professionals in a midwifery/obstetric unit to diagnose and manage labor to reduce the risk of prolonged labor, augmentation, cesarean sections, shoulder dystocia, and intrapartum stillbirths. The modified partograph is a new evidence-based labor monitoring tool to assist with assessment and decision-making during labor. Delayed implementation of the modified partograph was observed in the obstetric unit of the particular public hospital, while the previous partograph remained in use. It was not cl

Adverse Birth and Obstetric Outcomes in the Offspring of Male Adolescent and Young Adult Cancer Survivors: A Nationwide Population-Based Study.

BACKGROUND: The growing population of male adolescent and young adult (AYA, ages 15-40 years) cancer survivors has heightened interest in their reproductive health. However, studies have reported conflicting findings on the potential risks of cancer and its treatments on birth and obstetric outcomes. METHODS: We used encrypted identification numbers for both fathers and mothers to link three nationwide Taiwan datasets from 2004 to 2019, identifying 3,785 births with a paternal history of AYA cancer. For comparison, we included 37,850 matched fathers without a cancer history, matched by patern

Maternal and neonatal outcome of births planned in alongside midwifery units: a cohort study from a tertiary center in Germany.

BACKGROUND: For healthy women entering birth after uneventful pregnancy, midwife-led models of care have the potential to reduce interventions and increase the vaginal birth rate. In Germany, 98.4% of women are giving birth in consultant-led obstetric units. Alongside midwifery units (AMU) have been established in 2003. We compared the outcome of women registered for planned birth in the AMU at our hospital with a matched group of low-risk women who gave birth in standard obstetric care during the same period of time. METHODS: We used a retrospective cohort study design. The study group consi

Obstetric events and risk of periventricular hemorrhage in premature infants.

The relationships between selected obstetric factors and the occurrence of periventricular-intraventricular hemorrhage (PIH) was studied in a group of 77 infants weighing 1,200 g or less at birth. The diagnosis of PIH was made using ultrasonography or at autopsy in 42 (55%) of these infants. Univariate associations between PIH and gestational age (less than 30 weeks), duration of labor (greater than or equal to six hours), and vaginal delivery were found. Multivariate analysis, however, did not substantiate an association between PIH and any single obstetric factor. Log-linear analysis of mult

Prediction of labor onset type: Spontaneous vs induced; role of electrohysterography?

BACKGROUND AND OBJECTIVE: Induction of labor (IOL) is a medical procedure used to initiate uterine contractions to achieve delivery. IOL entails medical risks and has a significant impact on both the mother's and newborn's well-being. The assistance provided by an automatic system to help distinguish patients that will achieve labor spontaneously from those that will need late-term IOL would help clinicians and mothers to take an informed decision about prolonging pregnancy. With this aim, we developed and evaluated predictive models using not only traditional obstetrical data but also electro

The obstetric nightmare of shoulder dystocia: a tale from two perspectives.

PURPOSE: Shoulder dystocia is one of the most terrifying of obstetric emergencies. In this secondary analysis of two qualitative studies, the experiences of shoulder dystocia are compared and contrasted from two perspectives: the mothers and the labor and delivery nurses. METHOD: In the first study mothers' experiences of shoulder dystocia and caring for their children with obstetric brachial plexus injuries were explored. The second study explored secondary traumatic stress in labor and delivery nurses due to exposure to traumatic births. Krippendorff's content analysis technique of clusteri

Bridging borders in obstetric anesthesia in 2026: shared global priorities for the Society for Obstetric Anesthesia and Perinatology (SOAP), the Obstetric Anaesthetists' Association (OAA), and the World Federation of Societies of Anesthesiology (WFSA).

Obstetric anesthesiologists are central to modern maternity care, providing labor analgesia, anesthesia for operative delivery, and peripartum medical expertise while contributing leadership in safety, quality improvement, education, research, and advocacy. As childbirth increasingly occurs within complex and heterogeneous health systems, the scope of obstetric anesthesia has expanded beyond technical care to encompass workforce sustainability, patient experience, and global equity. This commentary explores the converging priorities of the Society for Obstetric Anesthesia and Perinatology (SOA

The Effect of Immediate Kangaroo Mother Care During Third Stage of Labor on Postpartum Blood Loss and Uterine Involution: A Quasi-Experimental Comparative Study.

BACKGROUND: Obstetric hemorrhage is the leading cause of maternal death worldwide. Obstetric hemorrhage accounts for 27.1% of all maternal death worldwide. Of all obstetric hemorrhages, postpartum hemorrhage (PPH) accounts for 72%. The physiological management of the third stage of labor is a growing area as a preventive measure to control postpartum blood loss. Immediate kangaroo mother care (KMC) is suggested as one of the physiological management methods of the third stage of labor to reduce postpartum blood loss. The duration of the third stage of labor, uterine involution, and amount of p

Type of attending obstetrician call schedule and changes in labor management and outcome.

OBJECTIVE: To estimate whether a night-float call schedule for attending obstetricians is associated with different labor management or obstetric outcomes compared with a traditional call schedule. METHODS: A chart review was performed for all women admitted for labor and delivery during two 3-month periods. One period occurred immediately before a single group of generalist obstetricians changed from a traditional call schedule to a night-float call schedule, whereas the second 3-month period occurred immediately after this change. A control group of women who were managed during the same 6-

The value of intrapartum factors in predicting maternal morbidity.

BACKGROUND: The rates of severe maternal morbidity and mortality in the United States exceed those in other high-income nations. To aid providers and hospitals in recognizing the risk factors, there have been multiple attempts to develop stratification systems for morbidity based on maternal comorbidities. However, most women giving birth are healthy and do not have comorbidities to suggest that they are at an increased risk for severe maternal morbidity. There are small but inherent maternal risks to labor, and the events after admission may further influence a woman's risk for morbidity even

Awareness, attitude, and anticipated willingness to use labor analgesia: a single-center antenatal survey study from a tertiary referral center in Saudi Arabia (2024).

BACKGROUND: Although labor analgesia is a standard component of maternity care, awareness and use remain variable. In Saudi Arabia, high antenatal attendance provides an opportunity for counseling, yet information gaps may still limit informed decision-making. This study assessed awareness of labor analgesia among pregnant women in Riyadh and examined attitudes, previous use, and willingness to use labor analgesia. METHODS: A cross-sectional study was conducted among 431 pregnant women attending antenatal care at King Abdulaziz Medical City, Riyadh, Saudi Arabia. Data were collected using a s

Third Stage of Labor and Acupuncture.

Background: Acupuncture use in obstetrics has been increasing in Western medicine, especially to alleviate complications of pregnancy, the most important cause of maternal mortality worldwide. One quarter of maternal deaths are caused by complications in the third stage of labor, an interval between complete delivery of the baby and the complete expulsion of the placenta. Methods: This review analyzes the increased acupuncture use for pregnancy complications as a potential method of reducting maternal mortality during the third stage of labor. This overview focuses on acupuncture points and te
