Surveillance from the high ground: sentinel surveillance of injuries and poisonings associated with cannabis.

INTRODUCTION: In October 2018, Canada legalized the nonmedical use of cannabis for adults. The aim of our study was to present a more recent temporal pattern of cannabis-related injuries and poisonings found in the electronic Canadian Hospitals Injury Reporting and Prevention Program (eCHIRPP) database and provide a descriptive summary of the injury characteristics of cannabis-related cases captured in a nine-year period. METHODS: We conducted a search for cannabis-related cases in the eCHIRPP database reported between April 2011 and August 2019. The study population consisted of patients bet

Patient rights and law: tobacco smoking in psychiatric wards and the Israeli Prevention of Smoking Act.

In August 2001, the Israeli Ministry of Health issued its Limitation of Smoking in Public Places Order, categorically forbidding smoking in hospitals. This forced the mental health system to cope with the issue of smoking inside psychiatric hospitals. The main problem was smoking by compulsorily hospitalized psychiatric patients in closed wards. An attempt by a psychiatric hospital to implement the tobacco smoking restraint instruction by banning the sale of cigarettes inside the hospital led to the development of a black market and cases of patient exploitation in return for cigarettes. Postpartum length of stay and newborn health: a cost-effectiveness analysis.

OBJECTIVE: To evaluate the cost-effectiveness of increasing lengths of brief postpartum hospitalizations. METHODS: A cost-effectiveness model extrapolating from secondary data was used. Social costs in 2000 US dollars were estimated using several sources, including a randomized controlled trial, a retrospective study, and survey data. Life-years saved from reduced infant mortality were estimated from administrative data from Washington State. A total of 113147 singleton newborns who were born in nonmilitary hospitals in Washington State in 1989 or 1990 and had postpartum stays short enough to

The impact of two related prevention strategies on head injury reduction among nonfatally injured motorcycle riders, California, 1991-1993.

Although the traumatic injury death rate in the United States decreased during the last 20 years, the percent of all injury deaths attributable to brain injuries has remained steady. Head injuries are a leading cause of injury among motorcycle riders in crashes, and the helmet is an effective measure to reduce these injuries. To reduce the burden and cost of motorcycle injuries, many states have increased helmet use by introducing mandatory helmet legislation. This report presents evidence on the effects of the motorcycle helmet and helmet use legislation--two interrelated prevention strategie

The perils of complementary alternative medicine.

More than 11,000 articles lauding alternative medicine appear in the PubMed database, but there are only a few articles describing the complications of such care. Two patients suffering from complications of alternative medicine were treated in our hospital: one patient developed necrotizing fasciitis after acupuncture, and the second developed an epidural hematoma after chiropractic manipulation. These complications serve as a clarion call to the Israeli Health Ministry, as well as to health ministries around the world, to include complementary medicine under its inspection and legislative au

Implementing a Prison Medicaid Enrollment Program for Inmates with a Community Inpatient Hospitalization.

In 2011, North Carolina (NC) created a program to facilitate Medicaid enrollment for state prisoners experiencing community inpatient hospitalization during their incarceration. The program, which has been described as a model for prison systems nationwide, has saved the NC prison system approximately $10 million annually in hospitalization costs and has potential to increase prisoners' access to Medicaid benefits as they return to their communities. This study aims to describe the history of NC's Prison-Based Medicaid Enrollment Assistance Program (PBMEAP), its structure and processes, and pr

Medicare program; hospital inpatient prospective payment systems for acute care hospitals and the long-term care hospital prospective payment system and fiscal year 2015 rates; quality reporting requirements for specific providers; reasonable compensation equivalents for physician services in excluded hospitals and certain teaching hospitals; provider administrative appeals and judicial review; enforcement provisions for organ transplant centers; and electronic health record (EHR) incentive program. Final rule.

We are revising the Medicare hospital inpatient prospective payment systems (IPPS) for operating and capital-related costs of acute care hospitals to implement changes arising from our continuing experience with these systems. Some of these changes implement certain statutory provisions contained in the Patient Protection and Affordable Care Act and the Health Care and Education Reconciliation Act of 2010 (collectively known as the Affordable Care Act), the Protecting Access to Medicare Act of 2014, and other legislation. These changes are applicable to discharges occurring on or after October

Pedestrian deaths resulting from road traffic accidents seen at the University of Port Harcourt Teaching Hospital--six-year review.

BACKGROUND: Road traffic accidents form an increasing bulk of trauma deaths worldwide. The aim of this study was to highlight the pedestrian deaths in our hospital experience and to suggest ways of curbing the menace. METHODS: A six-year retrospective review of mediocolegal autopsy consultations at the University of Port Harcourt Teaching Hospital. Multiway frequency tables were used for analysis. RESULTS: Cars constituted the commonest cause of pedestrian deaths (51.2%). This was followed by motorcycles (39.5%), with heavy vehicular deaths contributing 9.3%. The male to female ratio was 1.5

The twenty-year war over England's National Health Service: a report from the battlefield.

This article analyzes the latest battle in the twenty-year war to change England's National Health Service (NHS), starting with the internal market reforms introduced by the Thatcher government and now carried one step farther by David Cameron's coalition government. The government's program of change is characterized by (1) its wide scope and the organizational upheavals involved and (2) the fact that it is being introduced at a time when the NHS faces unprecedented fiscal pressures. The legislation faced strong political, public, and professional hostility both from those who saw it as a cri

The use of guardianship legislation for anorexia nervosa: a report of 15 cases.

OBJECTIVE: This paper investigates compulsory treatment under guardianship legislation for 15 anorexia nervosa patients admitted to four eating disorders units in New South Wales (NSW), Australia, between 1991 and 1994. METHODS: A retrospective follow-up was conducted. This involved an analysis of sociodemographic, clinical, eating and weight history, and Guardianship Order details obtained from medical records. This small sample was compared to a larger sample of anorexia nervosa patients admitted voluntarily to a specialised eating disorder unit in NSW. Further follow-up included a structur

Problems associated with the microbiological control of the breast milk from hospital milk bank units.

The microbiological tests on breast milk performed when samples of pasteurized breast milk are added to hospital milk banks are covered by French regulations dating from December 3rd 2007. They involve counts of the aerobic total flora and of Staphylococcus aureus in a sample of milk before pasteurization, and culture after pasteurization to check that the treated milk is sterile. The regulations specify the nature of the agar plates to be used, together with the conditions for plating and incubation, but they lack detail in other areas. We developed a quality assurance system, modified our pr

Change of substance-related hospitalisation in adolescents after cannabis legalisation in Thailand in 2022.

BACKGROUND AND AIMS: On 9 June 2022, Thailand changed its cannabis legislation, expanding access to individuals aged >18 years. Evidence suggests that such changes could increase the risk of cannabis use and related harms, and could influence patterns of use of other substances among adolescents. This study measured the potential impact of these legislative changes on adolescent substance-related hospital admissions. DESIGN: An interrupted time-series study to assess the impact of cannabis legalisation on substance-related hospitalisations using anonymised admission records from the Thai Heal

Review of tracheobronchial foreign body aspiration in the South African paediatric age group.

Children, and in particular young children under the age of three, are the most vulnerable for aspiration and ingestion of foreign bodies (FBs). The Red Cross War Memorial Children's Hospital in Cape Town is the only children's hospital in South Africa and is unique in having a dedicated trauma unit for children under the age of 13 as part of its institution. Core activities of Childsafe South Africa (CSA), located at the hospital, are data accumulation and interpretation, development of educational programmes, health inculcation and advising in legislation involving child health. To achieve t

Rubella immunization policies for health care personnel.

Rubella vaccination of children in the United States has resulted in a shift in the age of affected and susceptible individuals toward older age groups. In an effort to reduce the incidence of congenital rubella syndrome, the American Academy of Pediatrics and the Advisory Committee on Immunization Practices have recommended rubella vaccine for all male and female medical personnel who might contract and spread rubella to pregnant women. A survey of hospitals and outpatient practices in Little Rock, Arkansas, revealed extremely low compliance with the recommendation, suggesting the need for fu
