Visual outcome and changes in corneal endothelial cell density following aphakic iris-fixated intraocular lens implantation in pediatric eyes with subluxated lenses.

PURPOSE: To evaluate the visual outcome and corneal endothelial cell density after Artisan aphakic intraocular lens (IOL) implantation (Ophtec, Groningen, the Netherlands) in pediatric eyes with subluxated lenses. METHODS: Artisan aphakic IOLs were implanted in 18 eyes of 11 children with subluxated lenses. Idiopathic subluxations and ectopia lentis due to Marfan syndrome were included, whereas subluxations due to trauma or buphthalmos were excluded. Best-corrected visual acuity (BCVA) and endothelial cell density were monitored. Mean postoperative BCVA and endothelial cell density at last fo

Diverse presentations of ectopia lentis and lens coloboma in Marfan's syndrome.

Marfan's syndrome (MFS) is an autosomal dominant connective tissue disorder with defect in the fibrillin-1 gene. The most common ocular manifestation is subluxated lens in the superotemporal direction, accounting for 50%-85% of total cases. The association of lens coloboma with MFS has been described in literature, but the coexistence of lens coloboma with ectopia lentis is a rare feature. Here, we describe three cases of MFS including a case of bilateral lens coloboma with ectopia lentis: case 1 - a 39-year-old male with inferotemporal lens subluxation in the right eye and superotemporal lens…

[Morphological changes in failure of the lenticular ligamentous-capsular system].

Morphological changes in case of failure of the lenticular ligamentous-capsular system are described. Histological changes in this complex led the authors to the conclusion that morphological substrata of the "traumatic" and "surgical" lenticular subluxation are similar and are characterized by coarse mechanical changes in the lens capsule, with the relatively intact structure of the ligamentous system at a considerable length. Subluxation of the lens resultant from oral drug therapy is characterized by pronounced changes in the lens capsule and in the short cinnous ligament fibers. Age-associ

Pharmacologic pupillary constriction after dilated fundus examination for ectopia lentis to prevent further subluxation.

PURPOSE: To describe two cases of ectopia lentis with different preoperative management strategies following the initial diagnostic dilated exam. OBSERVATIONS: In both cases, the patients presented with bilateral subluxation of the crystalline lens. Neither patient had a known history of Marfan's disease, homocystinuria or other systemic disorders that affect the body's connective tissue possibly leading to lens subluxation. Patient 1 was sent home after dilated fundus examination with no special precautions. That same night, he developed severe right eye pain and further decreased vision. He…

Femtosecond laser-assisted cataract surgery in a spherophakic lens: An unusual case report.

RATIONALE: Spherophakia is a rare diagnosis which is often associated with a shallow anterior chamber, angle-closure glaucoma, lens subluxation, and lenticular myopia. When cataracts occur with subluxation of the lens, vision is often markedly affected. This often presents surgeons with a unique challenge of maintaining good visual outcomes while minimizing potential complications. PATIENT CONCERNS: A 48-year-old female was referred for ophthalmological assessment due to decreased vision in the left eye. In the left eye, the best-corrected visual acuity at distance was 20/125 with manifest re…

[Surgical management of Marfan-associated and idiopathic lens dislocations].

BACKGROUND: The ideal and safe surgical method for Marfan-associated or idiopathic lens subluxations is still a matter of debate. PATIENTS AND METHODS: Between 1990 and 1995, 23 eyes were operated for lens subluxations, mainly because of decreased visual acuity, but also because of conservatively uncontrolled secondary glaucoma. Marfan patients were 27.0 (5-62) years old at surgery; patients with idiopathic lens subluxations were 38.5 (11-63) years old. Surgical procedure depended on patient age and anatomical conditions. RESULTS: All patients achieved an increase in visual acuity. Amblyopia…

Spontaneous dislocation of a transparent lens to the anterior chamber--a case report.

INTRODUCTION: The causes leading to dislocation of the natural lenses are different involving injuries, hereditary diseases and spontaneous dislocation. Spontaneous dislocation of a transparent natural lens is extremely rare, especially dislocation of the anterior eye chamber. We report a case of spontaneous dislocation of the transparent natural lens to the anterior eye chamber in a patient who had no history of eye injuries. CASE OUTLINE: The patient was a 17-year-old boy. Lens dislocation was spontaneous, and the patient presented for ophthalmological consultation due to a sudden vision im

[New modified method of phacoemulsification in lenticular subluxation].

A new method for cataract removal by phacoemulsification in impaired lenticular joints consists in fixation during surgery of the anterior lenticular chamber along the edge of the capsulorrhexis by 4 retractor hooks and subsequent implantation of a soft intraocular lens. The operation was performed in 6 patients with cataracts of different types complicated by subluxation of the lens. The operation did not involve such complications as vitreous prolapse or displacement of the incompletely dislocated lens. All operations were finished with complete evacuation of lenticular mass and implantation

[Is the implantation of a second intraocular lens prior to the removal of the first implants remaining in the vitreous cavity incorrect medical treatment?].

BACKGROUND: The implantation of an intraocular lens into the capsular bag is a common procedure in modern cataract surgery. In case of a break of the posterior capsule it is possible to implant the intraocular lens by rhexis-, ciliary sulcus- or pars-plana-fixation. The following case is in so far remarkable, as a second intraocular lens has been implanted while a dislocated intraocular lens was still in the vitreous cavity. PATIENT: We report on a 70-year-old patient who underwent cataract surgery. After a break in the posterior capsule the intraocular lens dislocated into the vitreous cavit

[Topographic relation of dislocated lenses to the ciliary body and vitreous base].

There are three forms of lens dislocation in the posterior segment: intravitreal, prebasal, and intrabasal. The location of the dislocated lens depends on gravity and the condition of the vitreous body. As a rule the dislocated lens lies at the bottom of the vitreous cavity; it may be completely embedded in the vitreous substance or completely outside the vitreous body, or partially within the vitreous base. The topographic relations between dislocated lenses and the vitreous body cannot be determined by ophthalmoscopic examination, but biomicroscopy generally permits an accurate assessment. B
