[Portal hypertension in chronic lymphocytic leukemia].

Portal hypertension in chronic lymphocytic leukemia (CLL) is rare. A 64-year-old woman with CLL for 5 years and increasing hepatosplenomegaly developed portal hypertension and bleeding gastric varices. There was no portal vein thrombus by abdominal echography and angiography. Following splenectomy and devascularization of the fornix, the gastric varices disappeared. The liver biopsy showed dense leukemic cell infiltration in portal triads, but no fibrosis. The portal hypertension in this case may be mainly due to increased portal flow from the enlarged spleen and leukemic cell infiltration in

Central nervous system complications in childhood leukemia. Correlation between clinical and computed tomographic findings.

We used cranial computed tomography (CT) to evaluate 51 leukemic patients with or without central nervous system (CNS) symptoms. Among 17 symptomatic patients, nine had gross abnormalities on CT scans; leukemic infiltrations, infections (CNS aspergillosis), hemorrhages, and therapy-related complications were all evident. One with a leukemic infiltration showed a periventricular low density on the CT scans. The differential diagnosis of CT findings and the correlation between clinical and CT findings is described. The significance of a low-density area observed in an asymptomatic patient on lon…

Relapsed acute myelogenous leukemia of brachial plexus after marrow transplant.

We present a detailed description of brachial plexus infiltration by acute myelogenous leukemia (AML) in the setting of a remission bone marrow biopsy, without evidence of leukemia by flow cytometric analysis. This case illustrates the possibility of dormant leukemic cells in the peripheral nervous system (PNS) in a patient in apparent clinical remission. In patients with an unexplained brachial plexopathy and a history of AML, leukemic infiltrate of the PNS must be considered.

Congestive heart failure and endocardial fibroelastosis caused by chronic lymphocytic leukemia.

Endocardial fibroelastosis developed in the heart of a patient who had chronic lymphocytic leukemia. Leukemic infiltrates were found in the endocardial scar tissue, myocardium, coronary arteries, and other viscera. In view of the absence of any other known cause of endocardial fibroelastosis, it is postulated that endocardial fibroelastosis was caused by myocardial leukemic infiltration. This is the first reported case of endocardial fibroelastosis in a patient with chronic lymphocytic leukemia.

Establishment of a nude mice model of human monocytic leukemia with CNS and multiorgan extramedullary infiltration.

OBJECT: To establish a human monocytic leukemia model with central nervous system infiltration in BALB/c nude mice. METHODS: BALB/c nu/nu mice, pretreated by splenectomy, cytoxan intraperitoneal injection, and sublethal irradiation, were transplanted intravenously with human monocytic leukemic cell line SHI-1. The leukemic cells engrafted in the mice were traced by RT-PCR, histopathological examination, immunohistochemistry and FCM. RESULT: After engraftment of SHI-1 cells in SCI-nu/nu mice, multiple organs were involved and green solid neoplasms were formed in some organs. Paralysis was dev

Extramedullary relapse of acute myeloid leukemia presenting as an epidural spinal mass with vertebral and acetabular leukemic involvement: A case report.

Extramedullary relapse of acute myeloid leukemia (AML) refers to the infiltration of leukemic cells into tissues outside the bone marrow and … ae and the left acetabulum. Surgical decompression and biopsy of the spinal epidural mass confirmed leukemic infiltration. To our knowledge, this is the first reported case of extramedullary AML relapses simultaneously involving the epidural space, multiple vertebrae and acetabulum.

[Therapeutic of respiratory manifestations at the early phase of acute myeloid leukaemia].

The effective management of the respiratory manifestations at the early phase of acute myeloid hemopathies, especially acute myeloid leukaemia, frequently requires a close collaboration between hematologists, pulmonologists and intensivists. Dominated by infectious etiologies, there are however "specific" disease entities that should not be neglected in the diagnostic and therapeutic approach. These include lung leukostasis, leukemic lung infiltration, the cell lysis pneumopathy and the secondary alveolar proteinosis. These were the subject of a review in the Revue des Maladies Respiratoires p

Leukemia cutis in a patient with acute monocytic leukemia presenting as unique facial erythema.

A 67-year-old woman was referred to our department with a 1-month history of facial exanthemas. She had been diagnosed as having acute monocytic leukemia (French-American-British classification, M5b) based on the histological findings of bone marrow. Physical examination revealed diffuse edematous erythema on her cheeks, eyelids and glabella with scattered reddish papules. Histological examination demonstrated dense infiltration of atypical mononuclear cells in the dermis. Specific cutaneous lesions could occur in acute monocytic leukemia more frequently than in other types of leukemia, but ra

[Preferential infiltration of liver sinusoids in acute lymphoblastic leukemia].

We report a case of acute lymphoblastic leukemia (ALL) presenting as severe jaundice. The patient, a 59-year-old man, was found to have abnormal liver function, including an elevated total bilirubin level (13.5 mg/dl) with hepatosplenomegaly, but no detectable lymphadenopathy. A liver biopsy and bone marrow examination revealed a lymphoid neoplasm. Pathologic features included invasion of an abnormal clone into the sinusoidal region of the liver, diffuse bone marrow involvement (41.6% of all nucleated cells) and splenomegaly. Small numbers of malignant cells were also detected in the periphera

An urologic face of chronic lymphocytic leukemia: sequential prostatic and penis localization.

We report a patient with chronic lymphocytic leukemia (CLL) in whom a leukemic involvement of prostate and penis occurred in the advanced phase of his disease. Obstructive urinary symptoms were indicative of prostatic CLL infiltration, followed by the occurrence of an ulcerative lesion on the glans. Histologic examination confirmed the neoplastic B-cell infiltration. Both localizations responded to conventional treatments. A review of the literature confirms that leukemic involvement of the genito-urinary system is uncommon in CLL patients. However, it should be considered in CLL patients with

Acute lymphoblastic leukemia presenting in fulminant hepatic failure.

A previously healthy 4-year-old boy was admitted because of acute liver failure. He was icteric, lethargic, had elevated ammonia and abnormal liver function tests. Serology was negative for viral hepatitis. There was no history of hepatotoxic drugs. Family history was unremarkable. The child was taken to the operating room for a living-related hepatic transplant. Frozen section showed massive hepatic leukemic infiltration and hepatocellular necrosis. Bone marrow aspiration confirmed the diagnosis of acute lymphoblastic leukemia (ALL). Transplant was withheld and chemotherapy was attempted. He …

Skin changes in hairy cell leukemia.

Skin lesions have been reported in about 10-12% of hairy cell leukemia (HCL) patients. Most are etiologically related to autoimmune or infectious processes, although secondary cutaneous neoplasms and drug-induced lesions are also reported. However, leukemia cutis with the direct infiltration of the skin by leukemic cells is extremely rare in HCL patients. This paper reviews the epidemiology, pathogenesis, clinical symptoms, diagnosis, and approach to treating skin lesions in HCL. A literature review of the MEDLINE database for articles in English concerning hairy cell leukemia, skin lesions, l

Cutaneous manifestations of B-cell chronic lymphocytic leukemia.

Chronic lymphocytic leukemia (CLL) is a malignant lymphoproliferative disease characterized by the accumulation of immature monoclonal B lymphocytes in blood cells, bone marrow, spleen and lymph nodes. This is the most common type of leukemia among the Caucasoid race. When CLL skin lesions occur in about 25% of patients, they are extremely diverse. These lesions can be divided into specific, including infiltration of the skin by leukemic cells and the skin form of Richter's syndrome, secondary skin tumors, nonspecific lesions and associated skin diseases.Leukemic infiltration of the skin in pa…

Leukemia and the nervous system.

Leukemia affects the central and peripheral nervous system. Neurologic complications are a consequence of direct leukemic infiltration, as occurs with leukemic meningitis, and due to complications of either antileukemic treatment (thrombocytopenic or disseminated intravascular coagulation-related intracranial hemorrhage, steroid myopathy, vinca alkaloid peripheral neuropathy) or immune compromise (Herpes zoster shingles or Aspergillus meningitis).

Bilateral optic nerve and retinal infiltration as an initial site of relapse in a child with T-cell acute lymphoblastic leukemia.

PURPOSE: To describe a case of leukemic infiltration of bilateral optic nerves and retina as a site of relapse in a child with T-Cell Acute Lymphoblastic Leukemia (ALL). OBSERVATIONS: We report a 7 year old female who presented one year following initial treatment for T-Cell ALL with visual acuity impairment, bilateral optic nerve infiltration and infiltration of the retina of both eyes. OCT demonstrated subretinal fluid in both eyes, which eventually resolved, and perivascular hyperreflectivity within the inner retinal layers. She was treated with systemic and intrathecal chemotherapy, total…

HRCT findings of chest complications in patients with leukemia.

High-resolution CT (HRCT) findings of several chest complications occurring in leukemic patients were reviewed. Although most entities show non-specific HRCT findings including ground-glass opacity and air-space consolidation, characteristic findings are observed in several pulmonary complications including Pneumocystis carinii pneumonia, fungal infections, miliary tuberculosis, leukemic infiltration, pulmonary edema, bronchiolitis obliterans, and bronchiolitis obliterans organizing pneumonia. A combination of these characteristic HRCT findings and the information obtained from the clinical se…
