Anatomy of Ankle Syndesmotic Ligaments: A Systematic Review of Cadaveric Studies.

Diagnosis and management of isolated syndesmotic injuries are controversial and highly debated. Hence, the aim of this study is to explore and gain the current understanding pertaining to detailed anatomy of syndesmotic ligaments through a systematic review of published cadaveric studies. A systematic review was conducted online for literature published in English using PubMed and Google Scholar, as per PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, up to April 30, 2019. Predefined eligibility criteria were applied, and the data thus compiled was analyz

Magnetic resonance imaging and histopathological evaluation of equine oblique sesamoidean ligaments.

BACKGROUND: Pathology involving the oblique sesamoidean ligaments (OSLs) is commonly diagnosed during magnetic resonance imaging (MRI) of the equine distal limb. Presence of striations within the ligament and magic angle artefact can result in an incorrect diagnosis of pathological change. No studies have been performed using histopathology, the gold standard, to corroborate their imaging diagnosis. OBJECTIVES: (a) To determine which MRI characteristics are associated with normal vs abnormal OSLs and (b) to evaluate the ability of MRI to correctly identify abnormal vs normal oblique sesamoide

Arthroscopic Reconstruction of the Coracoclavicular Ligaments Using a Coracoid Cerclage Technique.

This technical note discusses the arthroscopic coracoid cerclage technique for double-bundle coracoclavicular ligament reconstruction in patients with nonacute symptomatic high-grade acromioclavicular separation injuries. This technique allows for an anatomic graft reconstruction of the coracoclavicular ligaments through an arthroscopic approach without the requirement to drill into the coracoid process. Early results are promising with high patient satisfaction and excellent reported clinical and radiographic outcomes. We believe this technique to be an anatomic, less invasive alternative to

Cruciate ligaments in arthritic knees: a histologic study with radiologic correlation.

Anterior cruciate ligaments (ACLs) and posterior cruciate ligaments (PCLs) from 45 osteoarthritic knees were histologically examined to evaluate the frequency and grade the severity of degenerative changes, which were correlated with radiologic grade of arthritis and severity of deformity at the knee. Immunohistochemical staining was used to identify neurofilaments in 10 knees. A histologic score was generated for both cruciates based on changes found on light microscopy. The ACL was severely degenerated, absent, or disrupted in knees with radiologic arthritis higher than grade 3 and varus def

Assessment of the lateral ligaments of the ankle after healed fractures.

BACKGROUND: Lateral ligament complex injuries are difficult to diagnose immediately after ankle fracture, and treatment is focused on the fracture. This study examines the prevalence of ligamentous injuries after severe ankle fractures. METHODS: Lateral ligament instability can be revealed by inversion and anterior stress views after fracture healing. The results of 54 inversion and anterior stress examinations of the ankle after fracture healing, using a Telos stress device at 15 kPa force, were compared with uninjured ankles. RESULTS: No patient had 5 degrees or more of talar tilt or 6 mm

MR imaging of ligaments and triangular fibrocartilage complex of the wrist.

Imaging of the wrist with MR imaging can be difficult because of the small size of this joint, its complex anatomy, and its sometimes poorly understood pathologic lesions. A recent study by Hobby and coworkers of 98 patients revealed that MR imaging of the wrist influences clinicians' diagnoses and management plans in most patients. This article summarizes the current diagnostic criteria that can be useful in interpreting abnormalities of the wrist ligaments and triangular fibrocartilage complex (TFCC) of the wrist in this difficult topic in joint MR imaging.

The anterior and superior malleal ligaments in otosclerosis: a histopathologic observation.

HYPOTHESIS: The aim of this study was to assess, in otosclerosis, whether the anterior and superior malleal ligaments show histologic changes that can lead to reduced malleal mobility and eventual fixation, and also to evaluate whether these changes are related to the degree of histologic otosclerosis. BACKGROUND: Fixation of the malleus seems to be one of the most controversial clinical entities in the acquired condition of otosclerosis. It has even been postulated that persistent conductive deafness, or progression of conductive deafness after initial improvement after stapedotomy, could be

Anatomy and function of the glenohumeral ligaments in anterior shoulder instability.

The anatomy of the glenohumeral ligaments has been shown to be complex and variable and their function is highly dependent on the position of the humerus with respect to the glenoid. The superior glenohumeral ligament with the coracohumeral ligament was shown to be an important stabilizer in the inferior direction, even though the coracohumeral ligament is much more robust than the superior glenohumeral ligament. The middle glenohumeral ligament provides anterior stability at 45 degrees and 60 degrees abduction whereas the inferior glenohumeral ligament complex is the most important stabilizer

Replantation of avulsed permanent teeth with avital periodontal ligaments: case report.

Five maxillary anterior teeth lost during a grand mal epileptic seizure, were replanted using the technique recently described by Andreasen (1), after an extra-alveolar period of more than 72 h. The teeth were replanted surgically, 10 days after being avulsed, after they had been immersed in a 2.4% sodium fluoride solution and root filled with gutta-percha. Ankylosis was evident 3 months post-operatively. After 24 months there was evidence of considerable replacement resorption on the lateral incisors with little evidence of resorption on the other replanted teeth. Even though the long term pr

Can muscle co-contraction protect knee ligaments after injury or repair?

A computer-based model of the knee was used to study forces in the cruciate ligaments induced by co-contraction of the extensor and flexor muscles, in the absence of external loads. Ligament forces are required whenever the components of the muscle forces parallel to the tibial plateau do not balance. When the extending effect of quadriceps exactly balances the flexing effect of hamstrings, the horizontal components of the two muscle forces also balance only at the critical flexion angle of 22 degrees. The calculations show that co-contraction of the quadriceps and hamstring muscles loads the

Complete dislocation of the knee without disruption of both cruciate ligaments.

Complete dislocation of the knee is recognized to be an extremely severe injury, representing one of the few true orthopedic emergencies. There is major trauma to the ligaments, capsule, and tendons, with a high incidence of associated damage to the popliteal artery and peroneal nerve. Resulting disability can be great, reflected by pain, stiffness, or instability of the knee; neurologic deficits in the foot and ankle; and even amputation if the vascular injury is not addressed promptly. Most accounts of knee dislocations in the literature have suggested that disruption of both cruciate ligame

The results of reconstruction of the lateral ligaments of the ankle.

Eighty-one reconstructions of the lateral ligaments of the ankle were carried out at our clinics between 1967 and 1983. Three methods were used and 53 patients were followed up. The only two poor results followed infection after dural reconstruction. Peroneus brevis reconstruction, dural reconstruction and ligament advancement all had good results in over 80% of the patients. There was restriction of supination and an increase of osteoarthritis after the peroneus brevis reconstruction operations. Statistical analysis did not show that any one of these procedures was better than any other.

Fluoroscopic identification of torn intercarpal ligaments after injection of contrast material.

Wrist arthrography is often used to evaluate the integrity of the scapholunate and lunotriquetral ligaments. This may be difficult when the ligaments are obscured by contrast material in standard radiographic projections, despite filming under fluoroscopic observation during the injection of contrast material. When superimposition occurs, we recommend fluoroscopic positioning of the wrist to optimally visualize the individual ligaments in profile. We applied this technique in performing 240 wrist arthrograms, 88 (37%) of which had scapholunate and/or lunotriquetral ligament tears. As a result,

Magnetic resonance imaging of the ligaments and menisci of the knee.

Although the technique is still developing and the data are preliminary, MRI will likely be the best noninvasive method for evaluating the ligaments and menisci of the knee. It can provide valuable and accurate information, especially in the acutely injured knee. The presence, extent, and exact location of cruciate ligament disruption can be visualized. This is useful in patients in whom the clinical diagnosis is uncertain and provides valuable information in preoperative planning. Equally important is the ability of MRI to provide diagnostic information about associated injuries to the collat
