OBJECTIVE: The aim of this article is to review the main treatment options for odontogenic sinusitis that are used today. MATERIAL AND METHODS: Search on PubMed, Cochrane Library, PMC, Science Direct data bases. For a literature review search keywords were used: odontogenic sinusitis, odontogenic maxillary sinusitis treatment OR diagnosis, maxillary sinusitis of dental source OR dental origin OR etiology. [Odontogenic maxillary sinusitis: peculiarities of diagnostics and treatment].

The objective of the present work was to improve the quality of diagnostics and the choice of optimal therapy for the management of odontogenic maxillary sinusitis associated with the localization of foreign bodies in the lateral parts of the maxillary sinuses. To this effect, multispiral computed tomography was used to enable the exact location of the foreign body inside the sinus and to choose the optimal approach for the surgical intervention. The modified Coldwell-Luc procedure was employed as the most adequate technique in the given clinical condition.

[Odontogenic maxillary sinusitis in elderly and old age].

On the basis of a detailed analysis of the clinical picture of odontogenic maxillary sinusitis the peculiarities of its flow in elderly and senile patients are demonstrated. The causes of odontogenic inflammation of the maxillary sinuses, the clinical features of inflammation of the maxillary sinus in older age groups, including those with oroantral communication are shown.

[Odontogenic maxillary sinusitis caused by a fractured tooth].

In a study of six cases of odontogenic maxillary sinusitis caused by fractured teeth, we found the following: (1) The pathophysiology of odontogenic maxillary sinusitis differs in cases in which the pulp cavity is not exposed from those in which it is exposed. (2) When the pulp cavity is not exposed, dental pulp at the apical foramen is injured, causing apical periodontitis that, in turn, causes odontogenic maxillary sinusitis via apical lesions. (3) When the pulp cavity is exposed, pulpitis followed by apical periodontitis and apical lesions causes odontogenic maxillary sinusitis. (4) Physici…

Incidence of fungal infections in chronic maxillary sinusitis.

Fifty patients with chronic maxillary sinusitis were included in our study. Antrat aspirate of chronically inflamed maxillary sinuses collected by antrat puncture were subjected to fungal culture. The study revealed the presence of fungi in 3 cases. Aspergillus fumigatus was isolated in 2 cases while Candida albicans was found in 1 case. The role of fungi, particularly aspergillus as pathogen is discussed in the context of antibiotics and immuno-suppressive therapy and local predisposing factors.

Bacterial findings in acute maxillary sinusitis--European study.

Bacteriology of acute maxillary sinusitis was studied in 569 patients in 16 centers of 6 countries located throughout Europe during 1992-1994 by ENT specialists. Patients with symptoms of acute sinusitis lasting less than 3 weeks with ongoing purulent nasal discharge were included. Diagnosis was verified by sinus x-ray or ultrasonography and a positive aspiration finding in maxillary sinus puncture. One or more pathogens were isolated from the maxillary sinus aspirates of 375 (66%) patients. Fifty-six percent of patients harboured 1 pathogen and 10% multiple pathogenic organisms, respectively.…

Incidence of fungal infections in chronic maxillary sinusitis.

A study conducted on seventy-five cases of chronic maxillary sinusitis presenting at the E.N.T. Outpatients' Department of Dayanand Medical College, Ludhiana revealed the presence of fungi in 8 cases. The return fluids of proof puncture and antral lavage of the sinuses were subjected to standard extraction procedures. Methods of isolation and identification of the causal fungi are described. The role of fungi, particularly Aspergillus, as pathogen is discussed in the context of antibiotic and immuno-suppressive therapy and local predisposing factors.

Acute maxillary sinusitis caused by a guttapercha point.

A case of acute sinusitis caused by a gutta-percha point is presented. The foreign body was introduced into the sinus during root therapy. Antibiotic therapy was ineffective. Surgical treatment consisted of radical antrostomy and removal of the foreign body.

Odontogenic Maxillary Sinusitis With Sequestrum-Like Bone Formation Mimicking Fungal Sinusitis: A Case Report.

Calcified materials in an opacified maxillary sinus often suggest fungal sinusitis, particularly aspergillosis. However, odontogenic maxillary sinusitis may rarely be associated with a bone-like calcified mass, creating diagnostic difficulty. We report a case of chronic odontogenic maxillary sinusitis with sequestrum-like bone formation that mimicked fungal sinusitis. An 88-year-old man was referred for mobility and occlusal pain of the maxillary left first molar. He had no nasal obstruction, rhinorrhea, headache, facial pain, or history of maxillary sinus surgery. Intraoral examination showed…

Long-term outcomes of endoscopic maxillary mega-antrostomy for refractory chronic maxillary sinusitis.

BACKGROUND: Endoscopic maxillary mega-antrostomy (EMMA) is a revision surgical procedure for recalcitrant maxillary sinusitis in which medical therapy and endoscopic antrostomy have been unsuccessful. In 2008 our group published favorable outcomes of EMMA in 28 patients with relatively short follow-up and nonvalidated outcome measures. This study reports an update of long-term outcomes of this same cohort, as well as outcomes of an interval cohort of 94 patients using validated outcome measures. METHODS: Retrospective review was performed for 122 patients (163 sides) who underwent EMMA betwee…

Healing of Unilateral Maxillary Sinusitis by Endodontic and Periodontal Treatment of Maxillary Teeth.

Inflammatory conditions of dental origin may spread to the bone tissue, causing its destruction, and to anatomical structures located in the vicinity of the tooth affected with inflammation. Maxillary premolars and molars may develop inflammatory lesions of the Schneiderian membrane and lead to tooth-borne lesions in the maxillary sinuses. Unilateral inflammation of the maxillary sinuses should be diagnosed and treated. The aim of this study was to determine whether and after what time from the applied endodontic or nonsurgical periodontal treatment the inflammation in the maxillary sinus was

[Therapeutic approach in chronic maxillary sinusitis. Apropos of 469 patients and 640 sinuses].

Between 1982 and 1987 we treated 469 patients who accounted for a total of 640 involved sinuses. These patients suffered from various stages of chronic sinusitis. Endoscopy of the maxillary sinuses enabled us to collect important information regarding the state of the mucosa and secretions, to collect bacteriological samples and to inspect the ostium and take pressure measurements. There was an 80% success rate in cases of sinusitis treated by drainage. In our opinion the indication for inferior meatotomy is as an extraction procedure and is not of value for aeration drainages. Nevertheless, o
