Associations between health insurance status, neighborhood deprivation, and treatment delays in women with breast cancer living in Georgia.

BACKGROUND: Little is known regarding the association between insurance status and treatment delays in women with breast cancer and whether this association varies by neighborhood socioeconomic deprivation status. METHODS: In this cohort study, we used medical record data of women diagnosed with breast cancer between 2004 and 2022 at two Georgia-based healthcare systems. Treatment delay was defined as >90 days to surgery or >120 days to systemic treatment. Insurance coverage was categorized as private, Medicaid, Medicare, other public, or uninsured. Area deprivation index (ADI) was used as a

Health insurance coverage in Mexico: progress, inequalities and remaining challenges towards UHC2030.

BACKGROUND: Universal health coverage (UHC) requires strong institutional capacity, equity-oriented policies, sustained political and financial commitment and public trust. However, public confidence in many health systems, including Mexico's, has been chronically undermined. This study aims to document Mexico's health coverage trajectory by offering a comprehensive, disaggregated and longitudinal assessment of insurance coverage from 2000 to 2023 - highlighting both achievements and setbacks in the context of UHC2030 goals. METHODS: This study used nationally representative data from Mexico'

Associations between childhood vaccination coverage, insurance type, and breaks in health insurance coverage.

OBJECTIVES: This study explored how vaccination coverage is associated with not being insured and with insurance type among children who are insured and to show how these associations are modified by race/ethnicity. METHODS: We determined whether 8324 children sampled in the National Immunization Survey in 2001 and 2002 were covered by private insurance only, Medicaid/State Children's Health Insurance Program, or another insurance type or were uninsured at the time of the National Immunization Survey interview or were uninsured at some time before the interview. Children were up to date if, b

Strategies to decrease tuberculosis in us homeless populations: a computer simulation model.

CONTEXT: The rate of tuberculosis (TB) among US homeless persons may be 20 times that of the general adult population. Studies suggest that the majority of urban homeless TB cases are attributable to ongoing transmission of TB. Optimal TB-control strategies in both chronically and transiently homeless populations are not known. OBJECTIVE: To examine the effects of TB-control strategies on projected TB cases and deaths in US homeless populations using a computer-based simulation model. DESIGN, SETTING, AND POPULATION: The US general population and a theoretical population of 2 million homeles

Self-Help Groups And Medication Use In Opioid Addiction Treatment: A National Analysis.

Self-help groups and medications (buprenorphine, methadone, and naltrexone) both play important roles in opioid addiction treatment. The relative use of these two treatment modalities has not been characterized in a national study. Using national treatment data, we found that self-help groups were rarely provided in conjunction with medication treatment: Among all adult discharges from opioid addiction treatment in the period 2015-17, 10.4 percent used both self-help groups and medications, 29.2 percent used only medications, 29.8 percent used only self-help groups, and 30.5 percent used neith

The Impact of Coronavirus Disease 2019 (COVID-19) on Vulnerable Communities: What the Clinician Should Know.

The COVID-19 pandemic unmasked striking health disparities within the US healthcare system and significantly impacted vulnerable populations. While much attention on vulnerability has been directed toward older individuals with underlying medical conditions, key populations-including the uninsured, racial and ethnic minorities, and pregnant individuals-were considerably affected and require intervention. This review examines the barriers these populations encountered and outlines how clinicians can engage at the provider, practice, community, and national levels to advance health equity and im

Extreme delay in care for enlarging breast mass.

Phyllodes tumor (PT) of the breast is an uncommon fibroepithelial tumor accounting for 0.37% of primary breast cancers. Most PT are <5.0 cm, averaging 4-7 cm. We present a case of PT measuring 28 cm in diameter with an extreme computed tomography image with tumor size larger than the adjacent thoracic circumference. A 59-year-old uninsured woman without significant medical history presented with a massive ulcerated chest wall tumor. The mass was resected, and microscopic examination demonstrated spindled cells within a myxoid stroma, consistent with PT.

Socioeconomic status is an independent predictor of biochemical recurrence among patients with prostate cancer who undergo radical prostatectomy.

PURPOSE: Socioeconomic status (SES) may influence cancer characteristics and behavior in several aspects. We analyzed PCa characteristics and behavior among low income uninsured men, and compare them to high income patients with health insurance in a developing country. MATERIALS AND METHODS: A retrospective case-control study was performed on 934 patients with clinically localized PCa who underwent radical prostatectomy between March, 1999 and July, 2009. Patients were divided in two groups, according to their SES. In group 1 (n =380), all had low income, low educational levels and couldn '

A retrospective case study of successful translational research: Cardiovascular disease risk assessment, experiences in community engagement.

In underserved communities across New York City, uninsured adults encounter a greater risk of cardiovascular disease (CVD) and diabetes. The Heart-to-Heart Community Outreach Program (H2H) addresses these disparities by screening for CVD risk factors, identifying healthcare access barriers, and fostering community engagement in translational research at the Weill Cornell Medicine Clinical and Translational Science Award (CTSA) hub. Screening events are hosted in partnership with faith-based institutions. Participants provide a medical history, complete a survey, and receive counseling by clini

On Pause: How a Group of Medical Students Engaged with Their Community During the COVID-19 Pandemic.

During academic clinical suspensions related to the COVID-19 pandemic, a group of medical students in Washington, D.C. collaborated with a local federally qualified health center to launch a free COVID-19 testing site to increase access to testing in the community. The patients who accessed the testing site were predominantly Black/African American and Hispanic/Latino, some of whom were uninsured or without access to testing or a timely physician's referral. In this article, medical students reflect on their experiences at this testing site and provide commentary on how existing racial and soc

The Association Between Federally Qualified Health Centers' Payer Mix and Clinical Outcomes Pre- and Post-COVID-19.

OBJECTIVE: Federally Qualified Health Centers (FQHCs) serve as critical safety net providers for over 30 million patients annually, operating with diverse payer mixes that create unique financial and operational challenges. This study examined associations between payer mix patterns and clinical quality outcomes in FQHCs, comparing relationships between 2019 and 2022 to capture COVID-19-related coverage transitions. METHODS: Using Resource Dependence Theory, we conducted a repeated cross-sectional analysis of Uniform Data System data from 1,190 FQHCs in 2019 and 1,150 FQHCs in 2022, examining

Medicaid Expansion Through Ballot Initiatives Increased Medicaid Enrollment And Reduced Uninsurance Among Adults.

Between 2019 and 2021, six states expanded Medicaid through voter-approved ballot initiatives, overcoming legislative opposition. We used 2016-23 data from the American Community Survey to compare health insurance coverage outcomes in these six ballot initiative states with those in ten nonexpansion states. We estimated stacked event study regression models to account for variation in expansion timing and control for socioeconomic differences. Overall, ballot initiative expansions increased Medicaid enrollment by 3.2 percentage points and decreased uninsurance by 2.2 percentage points among no

Drug-related charges and their association with social determinants of health.

OBJECTIVE: Drug-related arrests persist at high levels despite the negative health consequences of incarceration, particularly for those with a substance use disorder. To understand community-level factors potentially contributing to high rates of drug-related arrests, we conducted an ecological study estimating associations between county-level social determinants of health and rates of drug-related charges in North Carolina from 2016 to 2019. METHOD: Allowing a one-year lag between variables, we regressed the county-level rates of drug-related charges on each county-level social determinant

Changes in racial and ethnic disparities in lumbar spinal surgery associated with the passage of the Affordable Care Act, 2006-2014.

BACKGROUND CONTEXT: Since implementation of the Patient Protection and Affordable Care Act (ACA) in 2010, more Americans have health insurance, and many racial/ethnic disparities in healthcare have improved. We previously reported that Black and Hispanic patients undergo surgery for spinal stenosis at lower rates than do white patients. PURPOSE: To assess changes in racial/ethnic disparities in rates of lumbar spinal surgery after passage of the ACA. STUDY DESIGN: Retrospective analysis. PATIENT SAMPLE: Approximately 3.2 million adults who underwent lumbar spinal surgery in the US from 2006

Ensuring HIV care to undocumented migrants in Israel: a public-private partnership case study.

BACKGROUND: Undocumented migrants in Israel, mostly originating from HIV endemic countries, are not covered by Israel's universal healthcare coverage. We initiated a Public-Private Partnership (PPP) to handle this public health and humanitarian challenge. The PPP venture included the Ministry of Health (MoH), pharmaceutical companies, pharmacies, and specialized HIV clinics, the Israeli HIV Medical Society (from the Israel Medical Association), and non-governmental organizations. This study describes the national policy process in conceptualizing and implementing access to HIV services for und

Trends in pelvic inflammatory disease emergency department visits, United States, 2006-2013.

BACKGROUND: Pelvic inflammatory disease is a female genital tract disorder with severe reproductive sequelae. Because of the difficulties in diagnosing pelvic inflammatory disease, it is not a reportable condition in many states. Females seeking care in emergency departments are a sentinel population for pelvic inflammatory disease surveillance. OBJECTIVE: The objective of the study was to determine trends in diagnoses of acute pelvic inflammatory disease in a nationally representative sample of emergency departments. STUDY DESIGN: All emergency department visits among females aged 15-44 yea
