Neoplastic meningitis.

Neoplastic meningitis (NM) occurs in approximately 5% of all patients with cancer. NM is a disease affecting the entire neuraxis, so staging and treatment need to encompass all cerebrospinal fluid (CSF) compartments. Central nervous system staging of NM includes contrast-enhanced brain and spine imaging and radionuclide CSF flow study. Treatment of NM incorporates involved-field radiotherapy of bulky or symptomatic disease sites and intra-CSF drug therapy. The inclusion of concomitant systemic therapy may benefit patients with NM and may obviate the need for intra-CSF chemotherapy. At present,…

Mollaret's meningitis.

Pierre Mollaret is mainly known for his contributions to infectious diseases and their prevention. He also described benign, recurrent endothelio-leukocytic meningitis in three patients who had short-lived recurrent attacks of fever, headache and vomiting caused by sterile meningitis, with 'fantomes cellulaires' (cell ghosts) in the cerebrospinal fluid. Identical symptoms are caused by Herpes simplex virus-2 and other viruses. The term Mollaret's meningitis should be restricted to idiopathic recurrent aseptic meningitis. This paper briefly outlines the syndrome and its discoverer.

Neoplastic meningitis.

Источник пометил публикацию как отозванную

Neoplastic meningitis (NM) is a common problem in neuro-oncology occurring in approximately 5% of all patients with cancer, and is the third most common site of CNS metastases. NM is a disease affecting the entire neuraxis, and therefore clinical manifestations are pleomorphic affecting the spine, cranial nerves, and cerebral hemispheres. Because of craniospinal disease involvement, staging and treatment need to encompass all CSF compartments. Treatment of NM utilizes involved-field radiotherapy of bulky or symptomatic disease sites and intra-CSF drug therapy. The inclusion of concomitant syst…

Cryptococcal meningitis.

Cryptococcal meningitis is a common opportunistic infection in AIDS patients, particularly in Southeast Asia and Africa. Cases also occur in patients with other forms of immunosupression and in apparently immunocompetent individuals. Mortality from HIV-associated cryptococcal meningitis remains high (10-30%), even in developed countries, because of the inadequacy of current antifungal drugs and the complication of raised intracranial pressure. In cohorts of HIV-infected patients from sub-Saharan Africa, cryptococcosis has accounted for 13-44% of all deaths. Optimal current therapy is with amph…

Chronic meningitis.

Chronic meningitis is a clinical syndrome that is characterized by persistent or progressive signs and symptoms of meningitis associated with cerebrospinal fluid pleocytosis and elevated protein concentrations, lasting for a duration of at least 4 weeks without improvement. A large number of infectious and noninfectious diseases, many of which are difficult to diagnose, can cause chronic meningitis. Careful attention to exposure and travel history, physical examination findings, and certain patterns of CSF parameters, coupled with other specific laboratory evaluations, all play important roles…

Tuberculosis Meningitis.

Treatment of tuberculous meningitis should begin with an anti-tuberculous regimen of at least three drugs: isoniazid, pyrazinamide, and rifampin. Early in the course of therapy, ethambutol or streptomycin may be of some added benefit. If the local incidence of drug resistance to Mycobacterium tuberculosis is greater than 4%, or is unknown, then a fourth drug (ethambutol or streptomycin) should be added. If the patient is from an area with organisms resistant to multiple drugs, or is likely to be infected with a multiply resistant organism for any reason, then the patient should be on enough dr…

Viral meningitis.

Enteroviruses account for 85 to 95% of all cases of aseptic meningitis, but the arboviruses and herpes simplex virus are also important etiologic agents. Mumps, lymphocytic choriomeningitis virus, herpes zoster, human herpesvirus type 6, and influenza viruses are rare causes of meningitis. The virology, pathogenesis, epidemiology, clinical manifestations, diagnostic studies, and established and potential antiviral therapies for viral meningitis are discussed. A differential diagnosis of the aseptic meningitis syndrome is provided.

Neonatal meningitis.

OBJECTIVE: To determine the prevalent bacterial agents of neonatal meningitis and their antibiotic susceptibility in a referral intensive care unit in Assir Central Hospital, Saudi Arabia, during the years 1993-1998. METHODS: Records of newborn infants with positive cerebrospinal fluid culture during the period were retrospectively studied. RESULTS: There were 1473 nursery admissions, of which 32 episodes of meningitis occurred amongst 31 neonates. Klebsiella pneumoniae (31%) and Serratia marcescens (21%) were the main pathogens. The incidence of concurrent septicemia among these infants was…

[Chronic meningitis].

The paper presents the past knowledge of the etiology of chronic meningeal syndrome. The causes are divided into those of infective and those of noninfective etiology. Basic recommendations for general diagnostic approach, laboratory, clinical and radiological follow up of the patients have been given. A wide spectrum of differential-diagnostic possibilities has been pointed out, especially in relation to chronic systemic diseases. Characteristic syndromes of chronic meningitis have been separated and concisely described.
