Ethical competence: the cornerstone of care in the cardiac operating room, a phenomenological study.

BACKGROUND: Ethical competence leads to the provision of principled and ethical care, which subsequently improves health and increases patient satisfaction. Among these, the cardiac operating room is one of the most challenging and stressful therapeutic environments, in which the ethical competencies of healthcare professionals can significantly impact the performance and quality of care. Therefore, it is critical to describe healthcare professionals' experiences in the operating room regarding ethical competency. The present study aimed to investigate the experiences and perceptions of ethica

Psychotherapeutic Competencies in Blended Care Psychotherapy: Development of a Competence Framework.

BACKGROUND: Blended care psychotherapy (BCP) combines face-to-face psychotherapy with digital interventions, such as videoconferencing, apps, or virtual reality, which can be incorporated in various ways and at different stages of the therapeutic process. Despite its increasing implementation, conceptual and terminological inconsistencies continue to impede a shared understanding of the competencies required for its effective delivery. OBJECTIVE: This theoretical paper introduces a framework of psychotherapeutic competencies in BCP, designed to provide conceptual clarity. METHODS: The framew

When a Patient Is at Foreseeable Risk of Losing Decisional and Functional Capacity.

The four-skills model of decisional capacity for providing informed consent for medical treatment developed by Appelbaum and Grisso is codified into most state statutes in articulating the legal criteria for establishing capacity. Decisional capacity is traditionally determined at a point in time based on a narrow clinical question; however, there are clinical scenarios in which patients may currently have decisional capacity but their recurrent nonadherence to care places them at foreseeable risk of being acutely incapacitated, both decisionally and functionally, in the near future. There is

Introduction to the special issue: Culturally sensitive approaches to conceptualization and treatment of psychiatric disorders.

Cognitive-behavioral therapy (CBT) is an effective treatment for a variety of psychiatric disorders. However, historic underrepresentation, misapplication of techniques, and neglected consideration for the unique experiences of marginalized groups-including racial, ethnic, sexual, and gender minorities-have led to mistrust of mental health treatment among these communities and decreased access to quality, evidence-based care. Although these treatments are not inherently harmful to individuals with marginalized identities, clinicians can cause harm if they do not consider the role of culture in

INTRODUCTION: Supported Employment, based on the Individual Placement and Support (IPS) model, is increasingly implemented in France to promote the professional inclusion of people with mental health disabilities. This study examines its effects on employment outcomes and certain dimensions of mental health, as well as its implementation modalities. METHODS: The study is based on a mixed-methods approach combining quantitative and qualitative analyses at three levels: organizations (N = 27), employment specialists (N = 71), and service users (N =&a

A cyclical path to recovery: Calling into question the wisdom of incarceration after restoration.

Around 20-25% of the current offenders in Cook County Jail of Chicago Illinois are mentally ill. Each one of these offenders had to be named competent to stand trial when they were first being tried in court. The majority of these offenders that were considered incompetent to stand trial (IST) had to go through the competency restoration process where they were housed in a state hospital and received treatment until the court could deem them to be competent to stand trial. Many defendants with minor offenses that were eventually deemed competent to stand trial, stood trial and were found guilt

[Differences in psychiatric expertise of responsibility: Assessment and initial hypotheses through a review of literature].

INTRODUCTION: Forensic psychiatric assessment regarding liability ensures a balance between justice and psychiatry. In France, criminal assessment is not contradictory. The psychiatric expert is commissioned by judges to determine whether or not the accused has a mental disorder and specify whether it affects discernment and control of actions at the time of offense. Its mission focuses on the mental element required to constitute an offense, and is structured around Article 122-1 of the Criminal Code. This article, composed of two paragraphs, distinguishes the framework of the abolition of di

Polyglandular autoimmune syndrome disguised as mental illness.

Our case acts to highlight the numerous presentations of polyglandular autoimmune syndromes. A 62-year-old Taiwanese woman with a history of schizophrenia presented to our emergency department with a brain tumour causing her headaches. She was admitted due to severe anaemia, and after further investigation, the patient was discovered to have pernicious anaemia and autoimmune thyroiditis-consistent with the diagnosis of polyglandular autoimmune syndrome IIIb. Her underlying primary psychiatric diagnosis was then questioned. The diagnosis of her endocrinopathies were likely delayed for many year

Indicators of Social Competence and Social Participation Among US Children With Tourette Syndrome.

Children with Tourette syndrome often have behavioral and social difficulties, which may be associated with co-occurring mental, emotional, or behavioral disorders. This study investigated social competence, including behavioral problems and social skills, and social activities between children with and without Tourette syndrome using a nationally representative sample. In the 2007 National Survey of Children's Health, parents reported on health care provider diagnosis of Tourette syndrome, co-occurring mental, emotional, or behavioral disorders, and indicators of social competence. Children a

"Built on Respect and Good Honest Communication:" A Study of Partnerships Between Mental Health Providers and Community Corrections.

The prevailing approach to managing persons with criminal histories involves community supervision professionals like probation and parole officers partnering with other mental health providers to address clients' needs. The relationships between individual professionals are seldom researched, though, and the current study aims to address this deficit in the empirical literature. This study utilized interviews about professionals' perceptions of their work experiences, analyzed open-endedly to identify major themes. Mental health providers' themes included appreciation and process of collabora

African migrant men's experiences and preferences for formal mental health help-seeking: meta-synthesis and recommendations.

OBJECTIVE: Despite high rates of mental illness and significant barriers to accessing assistance, little is known about African migrant men's views on formal mental health help-seeking (i.e. from a health professional) in their new countries. We aimed to synthesise qualitative literature on African migrant men's experiences and preferences regarding formal mental health help-seeking in their new countries, and develop recommendations from the findings. METHOD: Systematic searches of six databases (nil date restrictions) for qualitative data from adult men who had migrated from any of the 16 c

A safe bet? Transgender and gender diverse experiences with inclusive therapists.

Historically, the global mental health system has pathologized transgender and gender diverse (TGD) identities, imposing spurious diagnoses and nocuous "cures." Recently, the World Health Organization declassified "transgender" as a mental health disorder, and psychological organizations have acknowledged the harmful practice of conversion therapy and other culturally inept interventions. Some practitioners have begun to advertise their therapeutic services as "trans-affirming" or "trans-competent," but to date, there are no published studies delineating their efficacy. The following content a

Increasing participation of people with thought disorder in clinical research.

BACKGROUND: Thought disorder (TD) is a core feature of severe mental illnesses such as schizophrenia, characterized by disruptions in speech, language, and communication. People with TD face unique barriers that hinder their involvement in research, both as participants and as partners. Their systematic underrepresentation in psychiatric research is driven by pervasive assumptions about their decisional capacity, willingness to participate, and ability to engage in research. This perpetuates a biased evidence base, likely hindering the therapeutic progress toward addressing this core problem.

Is assisted dying a treatment?

This paper focuses on the nature of the assisted dying (AD) decision including the object of the mental capacity test. Specifically, we inquire into whether AD is a treatment decision. In Part I, we analyse how the AD decision is characterised in all international AD statutes also analysing what government guidance says when the primary legislation is ambiguous. In Part II, we address the question normatively: firstly, from the perspective of clinical ethics and secondly from the perspective of legal rules (the doctrine of informed consent and the duties of governments in states with socialise
