INTRODUCTION AND IMPORTANCE: The spleen is normally found in the left hypochondrium and it is fixed in its place by numerous suspensory ligaments. When the ligaments are elongated or abnormally developed, it causes a rare medical condition called Wandering spleen. A persistent ascending and descending mesocolon is also a congenital anomaly, resulting from the failure of fusion of the primitive dorsal mesocolon.
CASE PRESENTATION: Herein, a 5-year-old male child with sudden and acute onset of abdominal pain, constipation, nausea, tachycardia, and low urine output, imaging and blood tests revea…

BACKGROUND: The diagnostic criteria and effect of persistent descending mesocolon (PDM) on sigmoid and rectal cancers (SRCs) remain controversial. This study aims to clarify PDM patients' radiological features and short-term surgical results.
METHOD: From January 2020 to December 2021, radiological imaging data from 845 consecutive patients were retrospectively analyzed using multiplanar reconstruction (MRP) and maximum intensity projection (MIP). PDM is defined as the condition wherein the right margin of the descending colon is located medially to the left renal hilum. Propensity score matc…

Laparoscopic D3 lymph node dissection for transverse colon cancer is technically demanding because of complicated anatomy. Here, we reviewed the vascular structure of the transverse mesocolon, explored the extent of the base of the transverse mesocolon, and evaluated the feasibility and oncological safety of D3 lymph node dissection. We retrospectively reviewed the clinical records of 42 patients with advanced transverse colon cancer who underwent curative surgery and D3 dissection at Kyushu University Hospital between January 2008 and December 2015. We examined the venous and arterial anatomy…

Ectopic pancreas is a relatively common congenital anomaly and is usually asymptomatic. Pancreatitis in the ectopic site and pseudocyst formation is extremely rare. To date, only 2 cases have been reported. We present a case of a 3-year-old girl with recurrent pancreatitis and unilateral hydronephrosis of the horseshoe kidney, which was produced by pancreatic pseudocyst arising from ectopic pancreas and isolated intestinal duplication in mesocolon. This is the first case of pancreatic pseudocyst that expanded to the retroperitoneal space and caused urinary tract obstruction.

BACKGROUND: Right colectomy with complete mesocolic excision (CME) is considered the standard for right-sided colon cancer. While laparoscopic CME (LCME) is widely performed, robotic CME (RCME) has emerged as an alternative approach. This systematic review and meta-analysis compared the perioperative, pathological, and oncological outcomes of RCME and LCME.
METHODS: Following PRISMA 2020 guidelines, randomized and controlled clinical trials comparing RCME and LCME were analyzed. Primary outcomes included operative morbidity, blood loss, bleeding, conversion to open surgery, and operative time

A persistent descending mesocolon is defined as a congenital fixation anomaly caused by the defective membrane fusion of the descending colon and the lateral abdominal wall. Anatomically, in persistent descending mesocolon, the left colonic artery is often shortened, and joins the marginal artery soon after its bifurcation from the inferior mesenteric artery, while the colonic mesentery often adheres firmly to the mesentery of the small intestine. As a result of these characteristics, anatomical knowledge of the persistent descending mesocolon and preservation of bowel blood flow are important…

PURPOSE: Persistent descending mesocolon (PDM) is a rare congenital anomaly that may complicate minimally invasive surgery (MIS) for colorectal cancer (CRC). This systematic review and meta-analysis evaluated the perioperative impact of PDM on MIS for CRC.
METHODS: This study followed the PRISMA guidelines and was registered in PROSPERO (CRD420251055757). A systematic search was performed in PubMed, Cochrane, and Scopus (January, 2000 to April, 2025). Observational studies (OBSs) compared MIS for CRC in patients with and patients without PDM. The primary outcome was operative time and the sec…

INTRODUCTION: Persistent descending mesocolon is a congenital fixation abnormality where the left-sided colon deviates medially. When significantly displaced to the right, it may affect sacrocolpopexy. We report two cases of persistent descending mesocolon in which robot-assisted sacrocolpopexy was successfully performed with mesh fixation to the sacral promontory from the left side of the mesosigmoid.
CASE PRESENTATION: Two patients with pelvic organ prolapse underwent robot-assisted sacrocolpopexy. In both cases, the sigmoid colon was displaced to the right, making exposure of the sacral pr…

Persistent descending mesocolon (PDM) is a congenital failure of fusion of the descending mesocolon, resulting in a lack of retroperitoneal fixation, rightward displacement, adhesions, and vascular displacement that complicate colorectal surgery. Renal cell carcinoma (RCC) metastasis to the rectum is extremely rare. The patient was a 65-year-old man who had a history of RCC with a rectal tumour. Colonoscopy revealed a submucosal tumor-like lesion, and endoscopic ultrasound-guided fine needle aspiration confirmed metastatic RCC. The shape of the colon on the 3D CT suggested PDM. On operation, a…

OBJECTIVE: To assess the effect of high inferior mesenteric artery tie on defecatory, urinary, and sexual function after surgery for sigmoid colon cancer. Performing a sigmoidectomy poses a notable risk of causing injury to the preaortic sympathetic nerves during the high ligation of the inferior mesenteric artery, as well as to the superior hypogastric plexus during dissection at the level of the sacral promontory. Postoperative defecatory and genitourinary dysfunction after sigmoid colon resection are often underestimated and underreported.

Isolated injuries of the small bowel mesentery or mesocolon with subsequent bowel infarction due to blunt abdominal trauma are rare. Two cases are described: 1 involving the mesentery to the terminal ileum and 1 involving the transverse mesocolon and middle colic artery, both with bowel infarction. The modes of clinical presentation and management of patients with injuries to the mesentery, mesocolon and mesenteric vessels following blunt trauma are described.

The distribution of regional lymph node metastases in carcinomas of the cecum, ascending colon, and transverse colon follows the vascular distribution in the ileocolic mesentery, ascending mesocolon, and transverse mesocolon. The location of these metastatic nodes can be recognized on CT scans when the anatomy of the vessels in the ileocolic mesentery and mesocolon is well understood. This knowledge is important in the preoperative staging of carcinomas of the colon for curative surgery and in the early detection of recurrent nodal disease after curative surgery.

Foetal evolution of bowel is the result of unequal growth of its segments and abdominal constraint of pressure. Caecum and appendix on the cranial and right side of intestine torsion differentiate colic anlage. Left colon observations in 20 foetus reveal a dorsal peritoneal fusion between 12 and 17 weeks: Progressive growth of left colon and meso, without dorsal fixation, beyond left kidney and subrenal gland. First mesocolon fixation at the left kidney. A tunnel persist between this fusion and the primitive mesocolon root. Radial advance of peritoneal fusion up to sigmoid mesocolon, with disa…

Sigmoid volvulus demonstrates geographical, racial, and gender variation. This autopsy study was undertaken to establish morphological differences of the sigmoid colon and its mesocolon in which the length and other characteristics were assessed. A total of 590 cadavers were examined (403 African, 91 Indian, and 96 White). Length and height of the sigmoid colon and mesocolon were significantly longer in Africans, and mesocolon root was significantly narrower in Africans. Mesocolic ratio for Africans, Indians, and Whites was 1.1 ± 0.8, 1.8 ± 0.7, and 1.9 ± 1.0, respectively. Africans had a sign…

Although the sigmoid colon is commonly afflicted with disease, studies on its anatomical dimensions are scarce. It is suspected that dimensions of the sigmoid colon change with age. This study documents data on the anatomical measurements of the sigmoid colon in 70 Indian subjects (51 live and 19 cadavers). Seven parameters of sigmoid colon anatomy measured included length and width of the sigmoid colon and mesocolon at specific points. Three mesocolic indices (width to length ratios) were calculated. Comparisons of measurements in the live and cadaver subjects and in the two sexes were made. …
