Adrenocortical carcinoma (ACC) is a rare and aggressive form of cancer. This article presents a multiperspective narrative describing the experience of a patient treated for ACC, the clinical insights of an endocrinologist, and the advocacy perspective of "Let's Cure ACC", a patient association. In a retrospective, nonrandomized comparison of patients with first recurrence of adrenocortical cancer, 18 patients were treated with chemotherapy (primarily mitotane) and 15 patients were treated with surgical resection plus similar chemotherapy. The case was treated by suprarenalectomy and is currently under antiblastic treatment with mitotane (o-p'DDD). In 1966, we published the case report of a patient with adrenal carcinoma and wide spread metatasis who, after treatment with mitotane (o,p-DDD) and fluorouracil, developed Addison disease accompanied by a clinical remission. The patient's unexpected death from an unrelated cause now allows us to verify what to our knowledge is the first cure of metastatic adrenal carcinoma by chemotherapy.
